HomeMy WebLinkAboutR-04-0617City of Miami Legislation Resolution: R-04-0617 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-01023 Final Action Date: 9/23/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S), ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER APPROVING THE FINDINGS OF THE EVALUATION COMMITTEE, ACCEPTING THE PROPOSAL OF MERCY MEDICAL DEVELOPMENT D/B/A MERCY OUTPATIENT CENTER ("MERCY") TO PERFORM SWORN POLICE ANNUAL PHYSICAL EXAMINATION SERVICES FOR DESIGNATED PERSONNEL, FOR THE DEPARTMENT OF EMPLOYEE RELATIONS, FORATWO-YEAR PERIOD WITH THE OPTION TO RENEW FOR TWO ADDITIONAL TWO-YEAR PERIODS, AT AN ESTIMATED ANNUAL AMOUNT NOT TO EXCEED $415,240, WITH INCREASES NOT TO EXCEED FIVE PERCENT (5%) FOR EACH RENEWABLE PERIOD FOR AN ESTIMATED TOTAL CONTRACT AMOUNT OF $2,757,536; ALLOCATING FUNDS FROM THE GENERAL OPERATING BUDGET OF THE DEPARTMENT OF POLICE, ACCOUNT CODE NO. 001000.290201.6.260; REQUIRING CITY COMMISSION APPROVAL FOR FURTHER INCREASES; AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE AN AGREEMENT, IN SUBSTANTIALLY THE ATTACHED FORM, WITH MERCY, FOR SAID SERVICES. WHEREAS, pursuant to public notice, Request For Proposals No. 03-04-081 was issued to perform sworn police annual physical examination services for designated personnel, on a contract basis for two years, with the option to renew for two additional two-year periods, for the Department of Employee Relations; and WHEREAS, RFPs were received on April 14, 2004, and only one Proposer was deemed responsive; and WHEREAS, the proposal was evaluated by an Evaluation Committee appointed by the City Manager which recommended that the proposal received from Mercy Medical Development, d/b/a Mercy Outpatient Center ("Mercy") for sworn police physical examination services, be accepted as the proposal most advantageous to the City, and WHEREAS, funds for these services, for an estimated annual cost not to exceed $415,240, with increases not to exceed five percent (5%) for each renewable period for an estimated total contract amount of $2,757,536 are available from the General Operating Budget for the Department of Police, Account Code No. 001000.290201.6.260; and WHEREAS, the City Manager and the Director of Employee Relations concurs with the recommendation that the proposal received from Mercy for the above services, be accepted as the proposal most advantageous to the City; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: City of Miami Page 1 of 2 File Id: 04-01023 (Version: 1) Printed On: 9/30/2016 File Number: 04-01023 Enactment Number: R-04-0617 Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. The recommendation of the City Manager approving the findings of the Evaluation Committee, accepting the proposal of Mercy to perform sworn police annual physical examination services for designated personnel, for the Department of Employee Relations, is accepted. Section 3. The City Manager is authorized{1} to negotiate and execute an Agreement with Mercy for a two-year period with the option to renew for two additional two-year periods, at an estimated annual amount not to exceed $415,240, with increases not to exceed five percent (5%) for each renewable period for an estimated total contract amount of $2,757,536, with funds allocated from the General Operating Budget of the Department of Police, Account Code No. 001000.290201.6.260. Section 4. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{2} Footnotes: {1} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {1} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
