HomeMy WebLinkAboutdetailed listDETAILED LIST OF NON-PERFORMANCE The Purchasing Department has received numerous complaints regarding the quality of service being provided by PRIT, Inc. d/b/a U.S. Lawns of Miami, pursuant to Bid No. 02-03-015. The following is a brief history of what has transpired since our receipt of the first complaint: December 16, 2003 • Received formal complaint from Stephen Bogner, Dinner Key Marina. Complaint: Failure to meet specifications, poor workmanship. (See attached copy). December 22, 2003 • Letter was sent out by Purchasing to U.S. Lawns of Miami informing them of complaints and requesting a meeting with all parties involved. (See attached copy) January 20, 2004 • Meeting at MRC with U.S. Lawns of Miami (Joey Pender and Rick Regist); and the following City personnel: Stephen Bogner, Jose Banos, and Frank Perez from Dinner Key Marina; Mike Rath and Maritza Suarez from Purchasing. Areas of concern were discussed with vendor, who agreed to remedy the problems. May 21, 2004 • Received another complaint, along with pictures, from Dinner Key Marina (Jose Banos) regarding poor quality of service provided by U.S. Lawns of Miami. (See attached copies) • Contacted Coconut Grove Expo Center (Alina Haas) to inquire about U.S. Lawns of Miami's performance at their facility. Alina advises that they are not happy with their services. May 24, 2004 • Letter was sent out by Purchasing to U.S. Lawns of Miami advising of complaints and of a meeting scheduled for June 1, 2004 to discuss the new complaints received. (See attached copy) June 1, 2004 • Meeting at MRC with U.S. Lawns of Miami (Rick Regist) and the following City personnel: Jose Banos and Frank Perez from Dinner Key Marina; Gregory Wright from Coconut Grove Expo Center; and Mike Rath and Martiza Suarez from Purchasing. Vendor was placed on a probationary period of one (1) month, and vendor was placed on notice that if any complaints were received during this probationary period would deem them in default of contract. • Letter was sent out to U.S. Lawns of Miami as a follow-up to the meeting detailing the issues discussed and agreed upon by all parties present. The letter had to be faxed back as acknowledgement of receipt, concurrence with the issues cited, and intent to comply with the City's expectations. (See attached copy) Page 1 of 2 June 1,2 and 3, 2004 • Called U.S. Lawns of Miami and left messages on their answering machine to call me regarding the letter that was faxed. No calls back. June 4, 2004 • Received e-mail complaint, along with pictures from Dinner Key Marina (Jose Banos) advising that U.S. Lawns of Miami did not meet the deadline given to them at the last meeting, requiring them to clean-up the marina and the expo center. (See attached copy). June 8, 2004 • Called Parks and Recreation Department to inquire about U.S. Lawns of Miami's performance at their facilities. S.A. Bastos advises that they have been having a lot of problems with U.S. Lawns of Miami and were just getting ready to contact us to have their contract cancelled. • Received phone call from Raul Garcia, who confirmed what Mr. Bastos had said. • Received, via courier, backup documentation from Parks and Recreation regarding numerous complaints about poor or lack of performance from U.S. Lawns of Miami, dating back to July 3, 2003. (See attached copies) • Received phone call (3:00 p.m.) from Rick Regist returning my messages from the 1st, 2nd, and 3rd. He explained he had some family problems and had just gotten my messages. He said he would fax the signed letter today. July 21, 2004 • Letter sent by Coconut Grove Expo Center requesting immediate service to the facility. • No response by U.S. Lawns of Miami. July 27, 2004 • Received phone call complaint from Raul Garcia of the Parks and Recreation Department regarding U.S. Lawns of Miami's failure to show up for work leaving the City with no other option but to utilize City personnel to perform the required work. • Received phone call complaint, followed by e-mail pictures supporting base of complaints from the Coconut Grove Expo Center (Tony Rodriguez).
