Box 82-2103 Miami, Florida 33082 SENT VIA FAX RE: Complaints regarding Bid No. 02-03-015 Dear Mr. Fender: jC}E ARRIOLA City Manager On June 1, 2004 a meeting was head between PRIT, Inc. dba US Lawns of Miami and representatives from the City of Miami's Dinner Key Marina, the Coconut Grove Expo Center and the Purchasing Department. The meeting was set as a result of a complaint received as it relates to the poor quality of services provided by US Lawns of Miami at the Dinner Key Marina and the Coconut Grove Expo Center, pursuant to Bid No. 02-03- 015. The following issues were discussed and agreed upon by all parties present: • US Lawns of Miami will remove all the accumulated leaves from both facilities by no later than Friday. June 4, 2004, at 4:00 p.m. • US Lawns of Miami will provide the necessary crew to be able to perform the required services (including mowing, trimming, edging and pickup and removal of debris and leaves) on the same visit. • US Lawns of Miami will assign an "on duty" supervisor, who shall serve as the City's main contact, capable of receiving information and/or complaints in relation to contract performance, and who shall take action where needed. Due to the lack of satisfaction from the Marina's manager, Mr. Stephen Bogner, and from the Expo Center Manager, Mr. Gregory Wright with the quality of service being performed by US Lawns of Miami, it was agreed that a probationary period of one (1) month be set to closely monitor US Lawns of Miami's performance. A meeting has been scheduled for Friday, July 2, 2004 at 9:30 a.m. to discuss US Lawns of Miami's progress and continuation of the contract. Notice is hereby given that if during this probationary period of one (1) month, the Purchasing Department receives any complaints regarding the level of service or your failure to adhere to the above agreed conditions, US Lawns of Miami will be found in default of contract, and in accordance with Sections 2.42 and 3.15 of the bid document, your contract with the City of Miami for the furnishing of grounds maintenance services for the Dinner Key Marina and the Coconut Grove Expo Center will be terminated. Please sign and return this letter as acknowledgement of receipt and your concurrence with the above facts and your intent to fully comply with the City's expectations. In an effort to address this issue, a meeting has been scheduled for iefo June,'2004 at 9:00 a.m. The meeting will take at the Miami Riverside Center Building, located at 444 SW 2nd Avenue, Miami, Florida in the 6th Floor Conference Room, and your attendance is mandatory. Box 82-2103 Miami, Florida 33082 Dear Mr. Pender December 22, 2003 VIA REGULAR AND CERTIFIED MAIL RE: Complaints regarding Bid No. 02-03-015 JOE ARRIOLA City Manager As you are aware, PRITT dba US Lawns of Miami has been awarded the City-wide Term Contract to provide grounds maintenance services to various city facilities, pursuant to Resolution No. 03-415. Please be advised the Purchasing Department has received a formai complaint regarding the quality of service being provided by PRITT dba US Lawns of Miami at the Dinner Key Marina under Purchase Order #40107. Dinner Key Marina has specifically indicated your failure to adhere to the number of visits required by contract specifications, and your failure to properly dispose of all debris after edging and trimming. In an effort to address these issues by the City and to ensure the full understanding of all of the responsibilities required of PRITT dba US Lawns of Miami in the performance of this contract, it is recommended a meeting be convened with all pertinent parties in an effort to discuss the situation and to resolve any problems which may exist to the satisfaction of all parties. Upon receipt of this letter, you are requested to immediately contact Ms. Maritza Suarez at 416-1907 to set up a meeting date and time so that these issues can be discussed and resolved. Failure to agree to a mutually convened meeting may result in the cancellation of this contract.
