REQUEST FOR PROPOSAL (RFP)
1.0 PURPOSE
The Florida Department of Transportation (FDOT) requires professional consultant services in
connection with the two (2) separately advertised projects listed below:

CONSULTANT # 1 AND # 2
Additional Information is necessary to complete the evaluation of each shortlisted
consultant's ability to provide the desired services. As a result, the consultants shortlisted
below are requested to submit a Written Technical Proposal that also includes required
Certification / Standard Consultant Technical Forms for the anticipated work.

2.0 SCOPE OF CONSULTANT SERVICES AND MEETING (IF REQUIRED)
For a detailed description of the required consultant services, refer to Attachment "A", Scope of
Services. A Scope of Services meeting has been scheduled for this project. A representative
from each shortlisted firm should be present at the meeting. It will be held at 2:30 P.M. E.S. T.
on November 7, 2001 in Conference Room B at the District Office located at 1000 N.W. 111'h
Avenue in Miami, FL.
3.0 LENGTH OF CONSULTANT SERVICES
The initial term of the agreement is 24 months with the option of extending it to a maximum
of five (5) years at the option of the Department. The Consultant services shall begin only
upon the receipt of a written Notice to Proceed (NTP) from the District Secretary.
4.0 CONTRACT RESTRICTIONS & PROPOSED METHOD OF COMPENSATION
A standard consultant contract will be executed by the selected consultant for this project.
The standard contract terms are provided as Attachment "B". The following additional
information is furnished to the consultant firm for consideration:
4.1 Professional Liability Insurance: $1.000,000.00
The insurance coverage must extend three (3) years beyond the completion of the
consultant services.

4.2 Computer Us.
The selected Consultant firm may desire to use the Department's mainframe computer
(not CADD) for project related tasks. The consultant should identify the specific FDOT
computer programs and an estimate of system usage that is proposed for the projec
in the technical proposal, if applicable. Requested computer services will be
considered during contract negotiation.
4.3 Public Entity Crime Information Statement
A person or affiliate who has been placed on the convicted vendor list following a
conviction for a public entity crime may not submit a bid on a contract to provide any
goods or services to a pubic entity, may not submit a bid on a contract with a public
entity for the construction or repair of a public building or public work, may not submit
bids on leases of real property to a public entity, may not be awarded or perform work
as a contractor, supplier, subcontractor or consultant under a contract with any public
entity and may not transact business with any public entity in excess of the threshold
amount provided In Section 287.017, Florida Statutes, for Category Two for a period of
36 months from the date of being placed on the convicted vendor list.
4.4 Method of Compensation and Invoicing
The Consultant will be compensated for services satisfactorily performed under the
Professional Services agreement, based on a negotiated lump sum amount or limiting
amount as stated in Exhibit "B" of the negotiated agreement.
Invoices for this agreement may be prepared by the Consultant and submitted to the
Department via the Department's Computerized Invoice Transmittal System (CITS).

4.5 Overhead Allowance
The allowance for administrative overhead and fringe benefits, expressed as a
percentage of direct salary and wages, shall be based on the firm's most recent
overhead audit by a State or Federal agency and shall not exceed 168% home office
or 116% field office (including subconsultants).
4.6 Subcontractual Services
Services assigned to subconsultants must be approved in advance by the Department.
Subconsultants performing the standard types of work covered by Rule Chapter 14-75,
F.A.C., must be technically prequalified with the Department or have an application for
prequallfication under review at the time that they are proposed.

4.7 Copyrighted Materials
Copyrighted material will be accepted as part of a technical proposal only if
accompanied by a waiver that will allow the FDOT to make paper or electronic copies
necessary for the use of Department staff and agents. It is noted that copyrighted
materials is not exempted from the Public Records Law, Chapter 119, Florida Statutes.
Therefore, such material will be subject to viewing by the public, but copies of the
material will not be provided the public.
5.0 EVALUATION PROCESS
The firms being considered for this project are required to submit a Written Technical
Proposal that contains all required certification forms and address(es) the evaluation criteria
presented in Section 5.1 below for the anticipated work.
5.1 EVALUATION CRITERIA:
SkG)ven that the Department will provide three (3) work sample projects to each shortlisted firm at the
op of Services Meeting, the evaluation criteria used for grading the written proposal will consist
of the following:
(Refer to Attachment "C" for the evaluation form)
1. Awareness of Project Issues: The Consultant should demonstrate their
understanding of the scope of services and of any unique issues involved in
the project.
2. Approach to the Project: The Consultant should present the proposed
approach for completing the desired work. This will include a brief synopsis of
the critical management activities involved with the project, and a discussion
of critical project issues. These issues could include technical or managerial
issues that are unique to this project or are particularly challenging. The
consultants approach to dealing with these issues should be described.
3. Project Staffing: The Consultant should submit a staffing plan which
clearly shows the key elements of the organizational structure that is proposed
to accomplish the work. The plan should address the administrative and
technical aspects of the project. Key personnel by discipline must be identified,
Past work experience of the key personnel should be discussed as they relate
to this particular project. The following specific items should be included in
this section:
Staffing requirements for the project (both by quantity and job
classification).
Availability of key project staff. This section should clearly show the
availability of the proposed team members at the time of the
proposed start of this contract. The firm should also clearly show
how conflicts with ongoing projects for the availability of the key
members will be resolved. This must be done for all key members of

the proposed staff, whether currently employed by the consultant or
those offered employment for this project.
Resumes of key project staff, including subconsultants. Resume
shall be complete, with no lapse of service. All prior work on FDG
projects will include references and contact numbers. Resumes of
senior managers and/or ancillary staff not proposed to work on this
project should not be included.
4. Other Considerations: Other data demonstrating communication ability, use
of specialized equipment, innovative ideas, commitment to satisfy the
Departments need, past performance on similar projects as well as:
Disadvantaged Business Enterprise (DBE): The use of DBE sub -
consultants is not mandatory and no preference points will be given in
the selection process for DBE participation. The Consultant is required
to complete the DBE Participation Statement Form and submit this form
with their technical proposals.
Schedule of Events: - (Not required for thisfirolect) The consultant
should identify the major project events anticipated with an "atypical"
project and discuss how they will be handled by the proposed staff.
Responsible Office: The consultant should identify the office location
which will provide office support to the construction field office.
Project Coordination: The consultant should describe the manner it
which the firm will manage the proper project coordination among th
consultant, Department, and other agencies and groups.
Quality Control: The consultant should describe, in detail, the firms
quality control program, specifically in relation to the requirements
specified in the Scope of Services.
Constructability Issues: - (Not required for this project) The consultant
should identify constructability issues and discuss how they are to be
handled.
5.2 REQUIRED CERTIFICATIONS / STANDARD CONSULTANT FORMS —
(Refer to Attachments "D" and "D.1 " for forms)
Estimate of Work Effort: - (Not required for this project) The consultant
shall submit an estimate of effort hours. This estimate may be given on
the standard forms that are provided by the Department as Exhibit
"D.1". Work effort hours should be specified by job classification for
each of the major project activities specified in the scope of work by the
Department. Any work to be done by subconsultant firms should be
clearly identified.

• Truth In Negotiations Certification: This form is primarily used for any
lump sum or cost -plus -a -fixed fee professional service agreement over
$60,000, and should be executed by an officer of the consultant firm.
DBE Participation Statement: The Consultant is required to complete
the following information and submit this form with their technical
proposals:
The Department encourages DBE firms to compete for Department
professional services projects, and also encourages Non -DBE
Consultants to use DBE firms as sub -consultants. However, use of DBE
sub -consultants is not mandatory and no preference points will be given
in the selection process for DBE participation. Consultants are required
to indicate their intention regarding DBE participation in the DBE
Participation Statement contained in Attachment "D" to this Request for
Proposal and to submit that statement with their technical proposals.
Bid Opportunity List: Federal Law requires states to maintain a
database of all firms that are participating or attempting to participate in
DOT -assisted contracts. To assist the Department in this endeavor,
consultants are requested to submit the Bidder's Opportunity List
contained in Attachment "D" to this Request for Proposal with their
technical proposal. The list should include yourself as well as any
prospective sub -consultant that you contacted or who has contacted
you regarding this project. However, any firm previously shown on
such a list need not be included.
Proposed Subconsultant List: This form is primarily used when
Interviews are required. The consultant shall complete this form and
list all subconsultants being proposed on this project.
Environmental Management Services Certification: - (Not reQuired for
this prolect) Only for EMO services, a certification is required that
indicates neither the firm nor any principals of the firm have any
financial or other interest in the outcome of this project.
Construction Engineering Inspection (CEI) Certification: - (Not required
for this project) Only for CEI services, a certification advising that a
consultant under contract with the Department to perform CEI or
material sampling and testing work on a project may not subcontract
with the construction contractor of the same project.
Federal Funded Projects: - (Not required for this project) The
following certification forms for federal -aid contracts shall be signed
and dated by an officer of the firm, association or corporation
submitting the proposal:

Disclosure of Lobbying Actin ..ies: If any funds other than
federally appropriated funds have been paid or will be paid to
any person for influencing or attempting to influence an officer
or employee of any Federal agency, a member of Congress, an
officer or employee of a member of Congress in connection wit!
this Federal contract, grant, loan, or cooperative agreement, the
prime consultant shall complete and submit the standard form
"Disclosure Form to Report Lobbying" in accordance with its
Instructions.
Compliance with 49 CFR, Section 29.510: Appendix B &
Appendix I Federal Regulation 49 CFR, Part 29 requires that on
all federally -funded contracts, consultants provide a certification
as to their current history regarding suspension, debarment,
ineligibility, voluntary exclusion, convictions, or civil judgments.
6.0 WRITTEN PROPOSAL & CERTIFICATION FORMS SUBMITTAL
Written Proposals and Certification Forms are due: November 28, 2001 by 3:00 P.M. E.S.T.
Certification Forms that are received after this deadline will be considered non-
responsive.
Five (5) Written Proposals consisting of one (1) original and four (4) copies are required.

The proposals should not exceed 15 standard size pages (8.5" X 11") in length.
Font size will be restricted to 10 pitch or larger.
Exhibits may be of larger size, but must be folded to the standard size when submitted.
Pages measuring 8.5" X 14" or 11" X 17" will be counted as two pages.
The maximum page size shall be 11" x 17".
Note: Tabs with pictures and words or pages containing pictures with added words MAY be counted as a page.

7.0 DEADLINE FOR QUESTIONS
The question cut-off date is needed to assure that the Department has adequate time to notify
all firms in writing about any additional questions, issues, or information that are raised by
any of the other competing firms.
The Deadline for Questions is: November 21, 2001 by 3:00 pm E.S.T.

8.0 CONSULTANT SELECTION
An, 'agreement will be awarded to two (2) of the four (4) firms shortlisted for the two (2)
General Consultant Engineering Services projects (District Contracts # 02644 and 02645)
advertised by the Department. A written technical proposal submitted by the four (4)
shortlisted firms will be reviewed by a Department Technical Review Committee (TRC).
Individual score sheets and comments will be submitted by the Technical Review Committee
to the District Professional Services Unit. The final weighted scores and comments will be
combined with other appropriate information and forwarded to the Selection Committee
Members. The final weighted scores will be determined as follows:

The Selection Committee Members, consisting of the Director of Planning & Programs; the
Director of Operations; the Director of Production; and a representative of the Professional
Services Unit (non -voting), will meet to discuss the capabilities of the consultant firms. At
the discretion of the Committee, other persons may be requested to provide information at
the meeting.
Immediately after the selection, the consultant firms will be notified of the ranking order
selection results.

9.0 CONTRACT NEGOTh1. i'IONS
Contract discussions will be initiated with the firms ranked number one and two by the
Selection Committee. The negotiations will focus on the technical proposal, scope r
services, the wages and billing rates.
The consultants shall prepare a price proposal with backup information to support the
proposed wage and expense costs for the services. The consultant's audited overhead will
be used to determine the allowance for administrative overhead and fringe benefits for the
project. If subconsultants are proposed to work on the project, Information to substantiate
each iirm's overhead rate, proposed wage rate and expenses is also needed.
Should the Department be unable to negotiate a fair, reasonable, and competitive price for the
consultant services with the number one ranked firm, the Department will terminate the
negotiations. The Department will then begin contract negotiations with the next ranked firm.
