AGREEMENT FOR MAINTENANCE SERVICES
Unisys Corporation, a Delaware corporation qualified to do business in the state of Florida
("Unisys") and the City of Miami, a municipal corporation of the State of Florida ("City")
hereby agree that Unisys shall provide to the City the services described in this Agreement. The
Request For Proposal Number 03-04-052R, together with the following listed attachments
constitute the entire agreement that governs the terms and conditions for the provision of
Network Backbone Maintenance Services ("Services") by Unisys. The documents comprising
the Agreement shall be governed in the order of precedence listed below.
Attachment 1 Infrastructure Support Services
Attachment 2 Program and Project Management
Attachment 3 Change Request and Issue Resolution Process
Attachment 4 Exceptions/Clarifications to RFP and Additional Terms and Conditions
Attachment 5 Supported Equipment List (Appendix F -- Pricing Matrix)
Attachment 6 Unisys Public Sector Surety Bond
Attachment 7 Insurance Certificate
RFP Number 03-04-052R (excluding all provisions regarding SmartNet)
This Agreement is entered into this day of 2004, effective as of , 2004,
