RFP #03-04-079 Pre -Employment
Physical Examination Services
RECOMMENDATION
It is respectfully recommended that the City Commission adopt the attached resolution to award a
contract to Mercy Medical Development d/b/a Mercy Outpatient Center, 3661 S. Miami Avenue, a local
non -minority vendor, to provide pre -employment physical examination services for designated personnel,
for the Department of Employee Relations, for two (2) years, with the option to renew for two additional
two (2) year periods, at a proposed first year annual cost not to exceed $136,695; further, authorizing an
increase in compensation for an amount not to exceed 5% for any optional renewal term, subject to the
availability of funds.
BACKGROUND
Section 2-268 of the City of Miami Code mandates the Department of Employee Relations establish and
maintain medical services which shall include a medical examination of employees and potential
employees. To fulfill this requirement, RFP 03-04-079 was issued to secure a provider to perform pre-
employment physical examination services for the City of Miami. In addition to physical examinations,
this RFP incorporated various other medical services such as x-rays, EKGs, and Hepatitis A, B and C
screenings. Three (3) proposals were received by April 12, 2004, and deemed responsive.
Based on an evaluation of the three Proposals by the Evaluation Committee, and approval by the City
Manager, it is recommended that the City Commission accept the findings of the Committee and
authorize the City Manager to execute a contract, in substantially the attached form, with Mercy Medical
Development d/b/a Mercy Outpatient Center, for two (2) years, with the option to extend for two
additional two year periods, with a first year cost not to exceed $136,695. We are additionally seeking an
escalation in fees charged by Mercy Medical Development d/b/a Mercy Outpatient Center, in an amount
not to exceed 5% for any extension term.
Funding is available to cover the cost of these services in the general operating budgets of the
Department of Employee Relations, Account Code No. 001000.270101.6.260; Department of Fire -
Rescue, Account Code No. 001000.280401.6.260; and Department of Police, Account Code No.
001000.290201.6.260.
