HomeMy WebLinkAboutexhibit 3- proposalPROPOSAL CITY OF MIAMI Biscayne Boulevard Cross Streets Two Way Conversions (9th Street, 10t' Street, 11th Street) City of Miami Florida A. GENERAL The proposed project consists of the conversion of 9th Street, 10h Street, and 11th Street from "one way" to "two way". The limits of the project are as follows: 1) 9th and 11th Street between Biscayne Blvd. and N.E. 2rd Avenue and 2) 10th Street between Biscayne Blvd. and N.W. 3r° Avenue. The proposed "two way" conversion of the above mentioned streets shall be consistent with the Florida Department of Transportation (FDOT) District 8 Project Improvements for Biscayne Blvd. (from N.E. 5th Street to N.E. 13th Street) (FPID No.: 249835-2-52-01). The scope of the work includes the final design and preparation of contract documents (Task 1) and post design services (Task 2); as related to the proposed two-way conversion of 9thStreet, 10'" Street and 11th Street within the above mentioned project limits. B. TASK 1 (FINAL DESIGN) 1. Survey a) Metric Engineering, Inc. Miami Ave, N.W. 2"d Ave. and N.W. 3rd Ave c. 11th Street at N.E. 2nd Ave All existing elevations shall be based on CITY OFMIAMI DATUM. d) The Surveyor shall prepare plan view of survey to contain ALL survey information gathered in the field and baseline of survey stationing proceeding from South to North and West to East, as applicable for each site. e) Survey shall be prepared by the SURVEYOR using 24" x 36" sheets. f) The Surveyor shall provide two (2) signed and sealed hardcopies to Metric. Metric shall provide one (1) hardcopy to the City. g) Metric shall provide a typical section submittal to the City along with the survey. Materials Testing a) Metric will retain the services of OEOSO of Inc. to obtain pavement minimum recommended of the existing roadway pavement surface for the purpose g depth. ns or if b) Actual field locationshall be obtained by the Materials Testing Engineer prior to performance of Utility des the tests. c) The Materials Testing Engineer shall prepare a Materials Testing Report to include field test results and recommendations. d) The Testing Lab performing the tests shall fill all holes, cuts, or other excavations or vehicular traffic. during the tests to avoid the possibility of accidents or damage to any pedestrian e) Two (2) ,signed and sealed sets of the test report shall be submitted by the Materials Testing Engineer to Metric. Metric shall submit One (1) signed and sealed copy of the test report to the City. 3. Design Development/Construction Documents a) The work to be addressed by the construction contract documents shall include the reconstruction of N.W. 1011' St from N.W. 2"d to 31t1 Ave and the milling and resurfacing of the remaining streets as welt as signing and pavement markings and signalization as related to the two-way conversion of 91' Street, 10t.Street and 1111" Street within the project limits. b) Develop Preliminary Engineering Plans based on all required engineering/design coordination decisions for City of Miami to review and validate project definition prior to completion of final construction documents. Submit two (2) copies of the preliminary engineering plans to the City for review and approval. Provide preliminary construction cost estimate. c) Complete construction . documents showing all of the proposed roadway Improvements as related to the conversion to "two war of 91" Street, 101" Street, and 1111 Street within the project limits. d) Unless otherwise directed by the City Project Manager, Standard Contract documents and Specification for Public Works Projects mOcepd�rovsse made art of the act Documents project current edition (supplemental technical specifications) to be developed by Metric. e) Provide updated construction cost estimates with each submittal as directed by the City Project Manager. f) Submit two sets of progress drawings and/or documentation to the City Project Manager at intervals designated by the City Project Manager. NE 11"' Project Definition Meetings a) This proposal includes 40 hours for meetings in support of work effort by City of Miami to define future improvements to NE 1111' St. west of NE 2nd Ave. C. TA$K 2 f POST DEIGN SERVICES) a) Metric Engineering, Ina. will provide assistance to the City of Miami during the construction phase of the project based on a twenty (20) week construction period. b) Attend pre -construction meeting with the City and selected Contractor. c) Respond to Requests for Information (BFI's) submitted by the Contractor and issue clarification of the construction documents. d) Review shop drawings of the proposed improvements and submit copies of approved shop drawings to the City. • e) Perform construction site visits for the purpose of max.)observing construction site project of the te (bahallsbeed on 20 weeks at 6 hours per week = 120 hours coordinated with the Contractor's schedule to observe the general status and progress of the work. Write and distribute the construction observation report after each site visit. f) Perform a substantial completion observation and prepare, in conjunction with the City's Project Manager, a punch list of any discrepancies. g) Perform a Final Construction Observation to ensure satisfactory completion of all items in the punch list. D. DESIGN SCHEDULe The work shall be performed based on the attached Project Design Schedule. (Attachment `C"). Task 2 (Post Design Services) compensation shall be based on Hourly Fee Basis. F. ASSUMPJIONS. EXCLUSIONS AND LIMITATION4 a. The design for installation or relocation of existing utilities are not Included In this proposal. b. The design of the signalized intersection at NE 11th St. and NE 2nd Ave. will be implemented by the Florida Department of Transportation. -END- 4 Exhibit A
