Miami, Florida 33130 RE: Supplemental Proposal for Engineering Services, No. 1 Liberty City Drainage Improvement Project Ph. I (B-5692-A) Dear Mr. Lago: As per your Department's request of February 25th, 2004 enclosed, please find the submittal of the proposal for supplemental engineering services on the subject, to include the new site located at N.W. 59 Street between N.W. 2nd and N.W. 3rd. Avenues. This project was initiated with (15) sites (See Attachment), as the result of various meetings between the City of Miami and MEI as described in our original proposal dated May 2n . 2003. as part of the Professional Services Agreement between the City and MEI entered in May 16, 2002. As of April 11, 2003 MEI was instructed to present the proposal for the aforementioned (15) sites following a particular format, which was submitted, approved and implemented from July 17, 2003 to December 15, 2003. SURVEYING SERVICES By • Provide Land Surveying services withint' - g, topographic survey with cross sections at 100 feet intervals, trees, driveway locations, and other utility and visual features that might impact the engineering design work. • Provide one (1) signed and sealed hard copy, and one (1) copy of the electronic file. 2. GEOTECHNICAL EXPLORATION SERVICES a. Borehole percolation testing will be performed at (2) location. These tests will be done to depths ranging from 10 to 15 feet below existing grade. b. The borehole percolation tests will be performed in accordance with the South Florida Water Management District method, Usual Open -Hole "Constant Head". c. Report will include, results of the borehole exfiltration tests (K= values) letter, summarizing results of the field tests, and One (1) report signed and sealed by the Professional Engineer. 3. „ DATA GATHERING AND CIVIL ENGINEERING AND LAND SURVEYING SERVICES • MEI will provide professional engineering services in accordance with Section 6, Scope of Work of the Professional Services Agreement between the City of Miami and MEI. As follows: A. Phase I Programming and Schematic Design B. Phase II Design Development C. Phase III Construction Documents (to include new site) D. Phase IV Bidding and Negotiation Phase (NOT APPLICABLE) E. Phase V Administration of the Construction Contract (including New site) F. Phase VI Post Construction Administration (NOT APPLICABLE) 2 The approved Sub -Consultant as indicated in Paragraph 16 D of the Professional services Agreement between Marlin Engineering, Inc. and the City of Miami is: GEOSOL, INC. Geotechnical Consulting Engineering, Testing and Inspections 4. NOT INCLUDED • Plans processing for approvals and/or construction permits. • Regulatory Agencies Plans Processing Fees. • As -Built Plans. (Construction plans are the responsibility of the Contractor) • Printing and reproduction costs, over and above the ones described under the Scope of Work as described in the Professional Services Agreement. In general, this proposal will provide site drainage design and calculations, to be included as part of construction documents preparation including construction specifications, bid packages, and all other pertinent services already completed for the original (15) sites, in accordance 'with the Professional Services Agreement between MEI and the City of Miami. 5. SUBMITTALS The Engineer will submit to the City, one (1) set of 24"x36" plans and one (1) copy of the specifications duly signed and sealed. One (1) set of 24"x36" reproducible plans, as well as one (1) set of 11"x17" set of plans along with two (2) CD's containing the electronic files for the final plans. It will also include an updated Estimate of Probable Construction Costs. 6. CONTRACT SUM For the services performed under paragraphs 1,2 and 3, the City will pay the Engineer on a lump sum basis the sum of Thirteen Thousand Four Hundred and Sixteen Dollars ($13,416), Refer to Exhibits "A" and "B" for breakdown per firm). Compensation shall be paid in accordance with the Section 11 of the Professional Services Agreement between the City of Miami and Marlin Engineering, Inc. dated May 16, 2003. 3 MEI will commence to work promptly, and expects to complete the professional services in forty-five calendar days from the date of the notice to proceed.
