HomeMy WebLinkAboutR-04-0253City of Miami Legislation Resolution: R-04-0253 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-00356 Final Action Date: 4/22/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION AMENDING RESOLUTION NO. 03-1222, ADOPTED DECEMBER 18, 2003, AUTHORIZING AN INCREASE IN THE CONTRACT AWARDED TO CARDIAC SCIENCE, INC., IN AN ESTIMATED AMOUNT NOT TO EXCEED $28,000, FROM $91,779.33 TO $119,779.33, FOR THE ACQUISITION OF ADDITIONAL POWERHEART G3 AUTOMATED EXTERNAL DEFIBRILLATORS ("AED") AND ACCESSORIES WITH A FIVE-YEAR WARRANTY, FOR THE DEPARTMENT OF FIRE -RESCUE, UTILIZING EXISTING STATE OF FLORIDA SNAPS III CONTRACT NO. 4651951-4, EFFECTIVE THROUGH MAY 17, 2004, SUBJECT TO ANY EXTENSIONS OR REPLACEMENT CONTRACTS BY THE STATE OF FLORIDA; ALLOCATING FUNDS FROM THE STATE OF FLORIDA EMERGENCY MEDICAL SERVICES ("EMS") MATCHING GRANT M3004 AWARD (FISCAL YEAR 2003), PROJECT NO. 110118, ACCOUNT CODE NO. 280212.6.840. WHEREAS, pursuant to Resolution No. 03-1222, adopted December 18, 2003, the City Commission approved the purchase of 41 each of Powerheart G3 automated external defibrillators ("AED"), G3 carrying cases, wall mount units and extended life lithium batteries, 47 ready kits and 129 sets of electrodes including a five-year warranty from Cardiac Science, Inc., for the Department of Fire -Rescue ("Fire -Rescue"), utilizing existing State of Florida Snaps III Contract No. 4651951-4, effective through May 17, 2004, subject to any extensions or replacement contracts by the State of Florida, in a total amount not to exceed $91,779.33, with funds allocated from the State of Florida Emergency Medical Services ("EMS") Matching Grant M3004 Award (Fiscal Year 2003), Project No. 110118, Account Code No. 280212.6.840; and WHEREAS, since it's initial award, Fire -Rescue has determined additional AEDs and accessories are required to enhance and expand the City of Miami's ("City's") Public Access Defibrillation program for Citywide use; and WHEREAS, additional equipment would allow Fire -Rescue to train additional City personnel to respond to calls of "person -down," and the ability to dispense these life savings devices throughout various City facilities; and WHEREAS, most importantly, for the safety and well being of the citizens and visitors of the City, the aforementioned equipment will give someone a second chance at life; and WHEREAS, due to the time restrictions of the EMS Matching Grant M3004 Award, and in accordance with the policies and procedures, it is necessary to increase the contract awarded with Cardiac Science, Inc., in an amount not to exceed $28,000, from $91,779.33 to $119,779.33, without delay to avoid the prospect of losing our funding source and said monies earmarked by City of Miami Page 1 of 2 File Id: 04-00356 (Version: 11 Printed On: 9/8/2016 File Number: 04-00356 Enactment Number: R-04-0253 Fire -Rescue's Coordinator; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The increase in the contract awarded to Cardiac Science, Inc., in an estimated amount not to exceed $28,000, is authorized and Resolution No. 03-1222, adopted December 18, 2003, is amended in the following particulars: {1} "RESOLUTION NO. 03-1222 Section 2. The acquisition of additional /11 each of Powerheart G3 AEDs, G3 carrying cases, wall mount units, and extended life lithium batteries, 47 ready kits, and 129 sets of electrodes accessories including a five-year warranty from Cardiac Science, Inc., for the Department of Fire -Rescue, utilizing existing State of Florida SNAPS III Contract No. 4651951-4, effective through May 17, 2004, subject to any extensions or replacement contracts by the State of Florida, in a total amount not to exceed $91,779.33 $119,779.33, is authorized, with funds allocated from the State of Florida Emergency Medical Services ("EMS") Matching Grant M3004 Award (FY 2003), Project No. 110118, Account Code No. 280212.6.840. * Section 2. This Resolution shall become effective immediately upon its adoption and signature of the Mayor. {2} Footnotes: {1} Words and/or figures stricken through shall be deleted. Underscored words and/or figures shall be added. The remaining provisions are now in effect and remain unchanged. Asterisks indicate omitted and unchanged material. {2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
