A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
AUTHORIZING THE CITY MANAGER TO EXECUTE THE FIRST AMENDMENT TO
THE CONTRACT, IN SUBSTANTIALLY THE ATTACHED FORM, WITH CHI
ALARMS, INC., AWARDED BY INFORMAL SOLICITATION AUTHORIZED BY THE
CITY MANAGER ON SEPTEMBER 2, 2002, PURSUANT TO RESOLUTION NO.
03-906, ADOPTED JULY 24, 2003; EXTENDING THE TERM OF THE CONTRACT
FOR ONE (1) ADDITIONAL MONTH AND INCREASING THE CONTRACT, IN AN
AMOUNT NOT TO EXCEED $10,000, FROM $99,928 TO $109,928, TO COVER
COSTS OF ADDITIONAL WORK AND MATERIALS REQUIRED TO COMPLETE
THE PROJECT ENTITLED,
"CITY OF MIAMI M.M.P.D. FIRE ALARM MODIFICATIONS, B-6433;" ALLOCATING
FUNDS FROM CAPITAL IMPROVEMENTS PROJECT NO. 312025, ENTITLED
"HEADQUARTER BUILDING RENOVATIONS;" AMENDING RESOLUTION NO.
03-906 TO REFLECT SAID INCREASE.
WHEREAS, pursuant to the informal solicitation authorized by the City Manager on September
2, 2002, the City of Miami and Chi Alarms, Inc. entered into a contract on June 12, 2003 for the
project entitled "City of Miami M. M. P. D. Fire Alarm Modifications, B-6433;" and
WHEREAS, the initial terms of the contract was 45 days completion time after beginning the
construction, and the contract sum, subject to additions and deductions and based on lump sum
and unit prices, all as provided in the proposal and other contract documents, was $99,928, and
WHEREAS, the proposed work consists of the furnishing and installation of a new fire alarm
system for the 400 Northwest 2nd Avenue Police Headquarter Building; and
WHEREAS, there are additional work and materials that need to be incorporated in the
forensic laboratory, located at the Department of Police parking garage, as part of the new fire
alarm system in an estimated amount of $10,000 and an estimated completion time of one
(1) additional month; and
WHEREAS, this increase has been approved on an emergency basis by the City Manager as
per the Emergency Memorandum dated January 27, 2004; and
WHEREAS, the City Manager may administratively approve emergencies that do
not exceed $25,000, and
WHEREAS, funds for the additional work are available from Capital Improvement Project
Account No. 312025 entitled "Headquarter Building Renovations;"
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,

FLORIDA:
Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted
by reference and incorporated as if fully set forth in this Section.
Section 2. The City Manager is authorized {1 } to execute the First Amendment to the Contract,
in substantially the attached form, with Chi Alarms, Inc., to extend the term of the contract for one
(1) additional month, and to increase the contract, in an amount not to exceed $10,000, from
$99,928 to $109,928 to cover the costs of additional work and materials required to complete the
project entitled "City of Miami M.M.P.D. Fire Alarm Modifications, B-6433."
Section 3. Resolution No. 03-906 is amended in the following particulars to reflect said
increase in an amount not to exceed $10,000.{1}

Section 1. By a four -fifths (4/5ths) affirmative vote, the City Manager's
finding of an emergency, the requirements for competitive bidding
procedures, and authorizing the acquisition of services from CHI Alarms,
Inc. for the project entitled "City of Miami M.M.P.D. Fire Alarm
Modifications, B-6433", in the amount of $99,928 $109,928, for project
costs plus $16,000 for estimated expenses incurred by the City, for a total
estimated project costs of $115,928 $125,928 are ratified, approved and
confirmed, with funds allocated from Capital Improvements Project No.
312025, entitled "Headquarter Building Renovations."

Section 4. This Resolution shall become effective immediately upon its adoption and signature
of the Mayor.{2}
Footnotes:
{1} The herein authorization is further subject to compliance with all requirements that may be
imposed by the City Attorney, including but not limited to those prescribed by applicable City
Charter and Code provisions.
{2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar
days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall
become effective immediately upon override of the veto by the City Commission.
