TENTATIVE PLAT #1615-A
Located at the Northerly Corner of SW 19 Road and South Mlaml Avenue
The City of Miami Plat and Street Committee, at Its meeting of November 6, 2003,
approved the above tentative plat subject to the following revisions being made to the
tentative plat, additional information being provided and/or variances being granted.
Please be advised that the processing of your tentative plat cannot proceed until these
conditions have been satisfied.
1. An opinion of title will be required for the remnant portion of Lot 13 and all or a
portion of Lot 12.
2. The wail encroaching into the external area of the proposed 25-foot corner radius
must be removed before the time of final plat submittal.
3. Existing structures and accessory structures must be removed prior to final plat.
4. Be aware that a setback line has been established by the underlying plat and will
continue to be a platted requirement of the new plat.
5. The owner Is advised that tentative plat application must be made with Miami -Dade
County. Please contact Mr. Tony Toledo at 305-375-2141 for information.
6. Before final plat submittal to the City, a copy of the final .plat must be submitted to
Miami -Dade County for review and then retrieved and submitted to the City. Contact
Mr. Tony Toledo at abovementioned phone number.
In addition to the above requirements, you should be aware of the following:
1. State and local laws require the installation of various physical improvements in the
public rights -of -way when property is platted. These subdivision improvementsAP-
include paving, drainage, landscaping, sidewalks, etc. In some cases this could
represent a substantial Investment on your part.
2. The alteration, relocation or installation of utilities such as storm and sanitary sewers,
electric, telephone, water, etc. caused by this plat will be at the property owner's
expense. Also, utility easements may be required on the property being platted.

3. A building permit will not be issued on the property being platted until the final plat
is recorded. Also, the Certificate of Occupancy for any building construction will be
issued only after all the required subdivision improvements have been completed.
4. Approval for fire flow requirements must be obtained from the Fire -Rescue
Department prior to the issuance of a building permit.
5. Additional. Items must be provided to the City of Miami Department of Public works ---
before the final plat Is submitted to the City Commission for approval. You will be
notified in writing as to what these items are after the amount of the bond has been
determined for the necessary subdivision Improvements.
6. Tentative plat approval is only valid for one 11) year from the date of the Plat and
Street Committee meeting at which time it was approved.

REPORT OF PROPOSED RECORD PLAT OF
1900 SMA
LOCATED AT THE NORTHERLY CORNER OF
SOUTH MIAMI AVENUE AND SW 19 ROAD

The accompanying Plat entitled 1900 SMA was prepared by Biscayne Engineering
Company, Inc. It Is in correct form for submission to the City Commission and is
forwarded with a recommendation that it be approved.
PERTINENT INFORMATION REGARDING THE PLAT:
(1) The property platted Is a repiat of the property described In the attached Legal
Description. The area platted consists of two (2) tracts containing approximately
12,133 square feet (0.28± of an acre). It Is zoned R-1.
(2) The location of the streets and their widths conform to the standards of the
Department of Public Works of the City of Miami, Florida..
(3) As certified to by George Bolton, Registered Surveyor and Mapper, this Plat
compiles with the plat filing laws of the State of Florida.
(4) The Certificate of Title Examination dated February 10, 2004, signed by Manuel A.
Ramirez, Attorney, indicates that the fee simple title to the property platted is
correctly vested in 1900 South Miami Avenue, L.L.C., a Florida limited liability
company, and that the Plat has been correctly executed.
(5) The area platted is encumbered by a mortgage and the mortgage holder has
executed the plat and Joined in its dedication.
(6) In accordance with the requirements of Chapters 54 and 55 of the Code of the' City
of Miami, Florida, a Cashier's Check in the amount of 516,725.00 has been tendered
by the owner, 1900 South Miami Avenue, L.L.C. This Cashier's Check has been
deposited with the Director of Finance and will guarantee the completion of the
subdivision improvements according to the provisions of the Agreement between
the City of Miami and the aforementioned owner. The Improvements required at
the property platted'are listed as follows:
REMOVE APPROXIMATELY 267 LINEAR FEET OF EXISTING CONCRETE CURB AND
GUTTER
CONSTRUCT APPROXIMATELY 267 LINEAR FEET OF 6-INCH CONCRETE CURB AND
GUTTER
PROVIDE AND INSTALL APPROXIMATELY 3615 SQUARE FEET OF SOLID SOD
(INCLUDING SOIL AND TOP DRESSING)
REMOVE APPROXIMATELY 100 SQUARE FEET OF EXISTING CONCRETE SIDEWALK
REMOVE APPROXIMATELY 19 SQUARE YARDS OF EXISTING ASPHALTIC PAVEMENT
CONSTRUCT APPROXIMATELY 560 SQUARE FEET OF 6-INCH CONCRETE SIDEWALK
PROVIDE AND INSTALL NEW 6-INCH SANITARY LATERAL AND WYE
(7) The attached Resolution has been prepared for the acceptance of the Plat by the
City Commission of Miami, Florida. •
