HomeMy WebLinkAboutR-04-0095City of Miami Legislation Resolution: R-04-0095 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-00111 Final Action Date: 2/26/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S), ACCEPTING THE CITY MANAGER'S RECOMMENDATION TO APPROVE THE FINDINGS OF THE EVALUATION COMMITTEE PURSUANT TO REQUEST FOR PROPOSALS ("RFP") NO. 03-04-004, THAT THE TOP -RANKED FIRM TO PROVIDE GROUP HEALTH BENEFITS FOR PART-TIME/TEMPORARY EMPLOYEES CONSULTING SERVICES FOR THE DEPARTMENT OF RISK MANAGEMENT, IS STAR HRG; AUTHORIZING THE CITY MANAGER TO NEGOTIATE A PROFESSIONAL SERVICES AGREEMENT ("AGREEMENT"), IN SUBSTANTIALLY THE ATTACHED FORM, AT AN ESTIMATED ANNUAL COST NOT TO EXCEED $150,000, FORA ONE-YEAR PERIOD, WITH THE OPTION TO EXTEND FOR THREE ADDITIONAL ONE-YEAR PERIODS; ALLOCATING SAID FUNDS FOR THE FIRST YEAR FROM NON -DEPARTMENTAL ACCOUNT CODE NO. 921002.510, WITH SUBSEQUENT ALLOCATION AND RENEWAL YEARS SUBJECT TO BUDGETARY APPROVAL; DIRECTING THE CITY MANAGER TO PRESENT THE NEGOTIATED AGREEMENT TO THE CITY COMMISSION FOR CONSIDERATION. WHEREAS, Request for Proposals ("RFP") No. 03-04-004 was issued to secure a provider to provide Group Health benefits for part Time/Temporary Employees, for the Department of Risk Management; and WHEREAS, two Proposals were received, deemed responsive and responsible, and evaluated by the Evaluation Committee; and WHEREAS, following evaluation, the Committee recommended the City negotiate a Professional Services Agreement ("Agreement") with the top -ranked firm, Star HRG for a one-year period, with the option to extend for three additional one-year periods if necessary; and WHEREAS, funding, in the amount of $150,000, for the first year will be provided from Non -Departmental Account Code No. 921002.510 for this project, with subsequent allocation and renewal years subject to budgetary approval; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. The City Manager's recommendation to approve the findings of the Evaluation Committee pursuant to RFP No. 03-04-004, that Star HRG is the top -ranked firm to provide Group City of Miami Page 1 of 2 File Id: 04-00111 (Version: 11 Printed On: 9/1/2016 File Number: 04-00111 Enactment Number: R-04-0095 Health Benefits for Part-time/Temporary Employees Consulting Services for the Department of Risk Management, is accepted. Section 3. The City Manager is authorized{1} to negotiate an Agreement, in substantially the attached form, with Star HRG, in an estimated annual amount not to exceed $150,000, for a one-year period with the option to extend for three additional one-year periods, with said funds for the first year allocated from Non -Departmental Account Code No. 921002.510, with allocation for subsequent renewal years subject to budgetary approval. Section 4. The City Manager is directed to present the negotiated Agreement to the City Commission for consideration. Section 5. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{2} Footnotes: {1} The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions. {2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten (10) calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
