HomeMy WebLinkAboutR-04-0059City of Miami Legislation Resolution: R-04-0059 City Hall 3500 Pan American Drive Miami, FL 33133 www.miamigov.com File Number: 04-00031 Final Action Date: 2/12/2004 A RESOLUTION OF THE MIAMI CITY COMMISSION AUTHORIZING AN INCREASE IN THE CONTRACT AWARDED TO MEDTRONIC PHYSIO-CONTROL CORPORATION, SOLE SOURCE PROVIDER, PURSUANT TO RESOLUTION NO. 02-980, ADOPTED SEPTEMBER 11, 2002, IN AN ESTIMATED ANNUAL AMOUNT NOT TO EXCEED $5,224.85, FROM $34,104 TO $39,328.85, TO INCLUDE MAINTENANCE SERVICES FOR ADDITIONAL PHYSIO-CONTROL CARDIAC LIFE SUPPORT EQUIPMENT ACQUIRED FOR THE DEPARTMENT OF FIRE -RESCUE; ALLOCATING FUNDS FOR SAID INCREASE FROM THE DEPARTMENT OF FIRE -RESCUE ACCOUNT CODE NO. 001000.280601.6.270, SUBJECT TO BUDGETARY APPROVAL; AMENDING RESOLUTION NO 02-980 TO REFLECT SAID INCREASE. WHEREAS, Resolution No. 02-980, adopted September 11, 2002, approved the procurement of Physio-Control Cardiac Life Support Equipment maintenance service for the Department of Fire -Rescue ("Fire -Rescue"), from Medtronic Physio-Control Corporation, sole source provider for said service, for an initial three-year term commencing October 1, 2002, and expiring September 30, 2005, at a total annual amount not to exceed $34,104, with the option to renew for two additional one-year periods and annual maintenance increases after the initial three-year period limited to no more than ten percent (10%) per year subject to negotiations; and WHEREAS, since this initial award, Fire -Rescue determined an increase is essential in the contract with Medtronic Physio-Control Corporation, the sole source provider, to add on-line six (6) Lifepak automated external defibrillation/monitor (AED) systems model No. 12, and twenty (20) Lifepak Model No. 500 to the Physio-Control Cardiac Life Support Equipment maintenance service contract, in an annual amount not to exceed $5,224.85, from $34,104 to $39,328.85, with funds allocated from the Department of Fire -Rescue, Account Code No. 001000.280601.6.270, subject to budgetary approval; and WHEREAS, this increase is necessary in order to ensure uninterrupted maintenance services for aforementioned equipment beyond the original manufacturer warranty date and to ensure a lower risk of field failures; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are adopted by reference and incorporated as if fully set forth in this Section. Section 2. An increase in the contract awarded to Medtronic Physio-Control Corporation, sole source provider, pursuant to Resolution No. 02-980, adopted September 11, 2002, in an annual City of Miami Page 1 of 2 File Id: 04-00031 (Version: 1) Printed On: 8/30/2016 File Number: 04-00031 Enactment Number: R-04-0059 amount not to exceed $5,224.85, from $34,104 to $39,328.85 is authorized, to include maintenance services for additional Physio-Control Cardiac Life Support equipment acquired for the Department of Fire -Rescue, with funds for said increase allocated from Account Code No. 001000.280601.6.270, subject to budgetary approval. Section 3. Resolution No. 02-980, adoted September 11, 2002, is amended. Section 4. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{1} Footnotes: {1} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective immediately upon override of the veto by the City Commission.
