HomeMy WebLinkAboutcover memoCITY OF MIAMI, FLORIDA INTER -OFFICE MEMORANDUM TO : FROM : Honorable Mayor rtdJ Mq of t ers of the ity Commissi Jo A riola Cit anager JAN 2 6 2004 FM.136.doc DATE : FILE SUBJECT : Amending Resolution No. 02-980 to include maintenance services to additional Physio-Control Cardiac Life Support Equipment and REFERENCES increasing contract ENCLOSURES: Resolution RECOMMENDATION It is respectfully recommended that the City Commission adopt the attached Resolution amending Resolution No. 02-980, adopted September 11, 2002, to include maintenance services to additional Physio-Control Cardiac Life Support Equipment acquired and authorizing an increase in the contract amount with Medtronic Physio-Control Corporation, a non- minority/non-local vendor, located at 11811 Willow Road NE, P.O. Box 97023, Redmond, WA 98073, the sole source provider, in an estimated annual amount not to exceed $5,224.85, from $34,104 to $39,328.85, for the Department of Fire -Rescue. Funds for this increase in an annual amount of $5,224.85 will be allocated from the Department of Fire -Rescue Operating Budget Account Code No. 001000.280601.6.270, subject to availability of funds. BACKGROUND Resolution No. 02-980, adopted on September 11, 2002, approved the procurement of Physio- Control Cardiac Life Support Equipment maintenance service for the Department of Fire - Rescue, from Medtronic Physio-Control Corporation, the sole source provider for said service, for an initial term of three years commencing October 1, 2002, and expiring September 30, 2005, at a total annual amount not to exceed $34,104, with the option to renew for two additional one-year periods and annual maintenance increases after the initial three (3) year period limited to no more than ten percent (10%) per year subject to negotiations. Since this initial award, the Department of Fire -Rescue has a need to add on-line six (6) Lifepak automated external defibrillation/monitor (AED) systems model No. 12, and twenty (20) Lifepak model No. 500 to the Physio-Control Cardiac Life Support Equipment maintenance service contract, and to increase the contract at an annual amount not to exceed $5,224.85, thereby increasing the contract amount from $34,104 to $39,328.85 with Medtronic Physio-Control Corporation. This increase is necessary in order to ensure uninterrupted maintenance services for said equipment beyond the original manufacturer warranty date and to ensure a lower risk of field failures.
