Heisenbottle Architects 340 Minorca Avenue, Suite 10 Coral Gables, Florida 33134 Re: WORK ORDER AUTHORIZATION Black Police Precinct Station and Courthouse Museum Restoration, B-3299 Dear Mr. Heisenbottle: This Work Order is deemed to incorporate by reference the terms and conditions of that certain Professional Consultant Contract for Architectural Services, dated, May 24, 2002, between the City of Miami and R.J. Heisenbottle Architects. The undersigned will perform this work in compliance with foregoing Agreement. Additionally, this work has been authorized by the City Commission Resolution No. and the terms of any enabling Resolution are deemed as being incorporated by reference herein. The undersigned will perform this work in compliance with any enabling Resolution. I. Scope of Services The Scope of Services are detailed in Exhibit "A", attached hereto and made part thereof. II. Time of Performance The Schedule for performance of the work is detailed in Exhibit "B", attached hereto and made a part hereto. III. Compensation JOE ARRIOLA City Manager The work will be performed for a fee guaranteed not to exceed the sum of $69,700.00. The City shall not be liable for any cost, fee, or expenditure above what is set forth in this section. The above quoted fee includes an allowance for reimbursable expenses that comply with the requirements of the Agreement. Any eligible reimbursable expenses shall confirm to the limitations of § 112.061, Fla. Stat. V. Black Police Precinct Station and Courthouse Museum Restoration, B-3299 Page 4 of 5 Exhibit "A" Scope of Services The scope of services shall consist of complete professional A/E/ services as defined in the (6) phases described in section A through F in paragraph 6 Scope of Work in the Professional Services Agreement between R.J. Heisenbottle Architects, Inc., and The City of Miami, dated May 24, 2002 on this proposal. The Phase I — Programming and Schematic Design, Phase II — Design Development, and Phase III — Construction Document are not part of this proposal. R.J. Heisnebottle shall submit complete and fully permitted construction documents including specification and bid proposal for the above mention project. The project consists of the restoration and adaptive reuse of the Historic Black Police Precinct Building. The location of the project is at 1009 N.W. 5th Avenue, Miami, Florida. Heisenbottle Architects, P.A., is pleased to provide you with this Fee Proposal for Bid Phase, Construction Administration Phase 'and post Construction Services on the historic Black Police Precinct Station project located at 1009 N.W. 5th Avenue, Miami, Florida. As you are aware in the past R.J. Heisenbottle Architects, P.A. has prepared the design and construction documents for the project under an agreement with the City of Miami Retired Police Officers Community Benevolent Association, Inc. Those plans are now completed and are in for permit review. Since the historic precinct station is a city owned building and since the City of Miami wishes to oversee the bidding and construction phase work under the supervision of the Depaitmnent of Capital Improvements. This proposal is intended to proyide those Bidding and Construction Administration Phase Services not completed under the original agreement and perform them under the Miscellaneous Architectural Services Agreement between R.J. Heisenbottle Architects, P.A. and the City of Miami Dated May 24, 2002. The scope of the project anticipates that the existing abandoned precinct headquarters will be restored to its original condition and utilized as both a museum and community center. To restore the structure, we anticipate structural repairs and enhancements, new mechanical, electrical, plumbing, and life safety systems, new roofing, insulation, windows, doors as well as a new elevator to provide handicap accessibility to the second floor. Wherever possible R.J. Heisenbottle Architects, P.A., has attempted to recreate the original interior details, specifically at the entrance lobby, courtroom, and cellblock. The project is currently budgeted at $1,177,685.00, not including exhibit design, display fabrication and FF&E cost. These costs and the associated design fees need to be funded separately. Under this proposal R.J. Heisenbottle Architects, P.A., will provide complete Professional A/E Services for the following phases as described in Paragraph VI Scope of Work in the Professional Services Agreement between R.J. Heisenbottle Architects, P.A., and the City of Miami. Heisenbottle Architects, P.A. reimbursable expenses shall be in accord with Paragraph 14, Reimbursable Expenses in the Professional Services Agreement between R.J. Heisenbottle Architects, P.A., and the City of Miami. R.J. Heisenbottle Architects,P.A. Additional Services shall be in accord with Paragraph 13, Extra Work Expenses in the Professional Services Agreement between R.J. Heisenbottle Architects, P.A. and the City of Miami. Reimbursable Expenses and Additional Service Fees are estimated at $20,000.00. R.J. Heisenbottle Architects, P.A., Professional Services include providing drawings as listed in attached Exhibit "C". R.J. Heisenbottle Architects, P.A. Professional Services for this project are anticipated to be completed within the following timeline/schedule: Sandra Vega City of Miami 12/08/03 Page 3 Phase IV Bidding and Negotiation - 90 Calendar days from notice to proceed. Phase V Construction Administration — 240 Calendar days from G.C. notice to proceed (See Exhibit "B") attached. Should the Construction Administration of the project extend beyond 240 Calendar days through no fault of the architect, R.J. Heisenbottle Architects, P.A., shall be entitled to Additional Services for extended Construction Phase Administration. Said Additional Services would be provided in accordance with the terms and conditions of the Professional Services Agreement between R.J. Heisenbottle Architects, P.A. and the City of Miami. In general this proposal shall provide services in accordance with the Professional Services Agreement between R.J. Heisenbottle Architects, P.A., and the City of Miami. This proposal shall not be used as a substitution of the Professional Services Agreement between R.J. Heisenbottle Architects, P.A., and the City of Miami. This proposal is used as an instrument to provide specifics under Paragraph III — Subject Matter, Paragraph IV Definitions and Paragraph V — Services as indicated in the Professional Services Agreement between, R.J. Heisenbottle Architects, P.A. and the City of Miami. Provided that the above reference scope of work meets with your approval please prepare the appropriate work order authorization.
