D/B/A SUNBELT MEDICAL/EMERGI SOURCE FOR THE DEPARTMENT OF FIRE -RESCUE, ON AN AS -NEEDED BASIS, UTILIZING MIAMI-DADE COUNTY BID/CONTRACT NO. 6262-2/06-OTR-SW, EFFECTIVE THROUGH AUGUST 31, 2004, SUBJECT TO ANY EXTENSIONS OR REPLACEMENT CONTRACTS BY MIAMI-DADE COUNTY, WITH AN OPTION TO RENEW FOR ONE TWO-YEAR PERIOD, AT AN ESTIMATED ANNUAL AMOUNT NOT TO EXCEED $48,605, FOR AN ESTIMATED TOTAL CONTRACT AMOUNT NOT TO EXCEED $145,815, ALLOCATING FUNDS FROM THE BUDGET OF THE DEPARTMENT OF FIRE -RESCUE, ACCOUNT CODE NO. 001000.280601.6.14, SUBJECT TO BUDGETARY APPROVAL. BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The procurement of Amsino administrative sets to administer medications for medical and trauma related illnesses or injuries, from Sunbelt Medical Supply, Inc., d/b/a Sunbelt Medical/Emergi Source for the Deparment of Fire -Rescue, is authorized, on an as -needed basis, utilizing existing Miami -Dade County Bid/Contract No. 6262-2/06-OTR-SW, effective through August 31, 2004, subject to any extensions or replacement contracts by Miami -Dade County, with an option to renew for one two-year period, at an estimated annual amount of $48,605, for an estimated total contract amount of $145,815, with funds allocated from the Budget of the Department of Fire -Rescue, Account Code No. 001000.280601.6.714, subject to budgetary approval. Section 2. This Resolution shall become effective immediately upon its adoption and signature of the Mayor.{1} Footnotes: {1} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar days from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shal (become effective immediately uupon override of the veto by the City Commission.
