MIGOYA ChierProcuremeul0l`ti(ei L'it) l"Ianaacr � df Std;; rJ ADDENDUM NO. 1 IFB No. 211224 June 22, 2010 Invitation For Bid (IFB) for Bunker Gear Inspection, Cleaning, and Repairs TO: ALL PROSPECTIVE BIDDERS: The following changes, additions, clarifications, and deletions amend the Bid Documents of the above captioned IFB, and shall become an integral part of the Contract Documents. Words and/or figures stricken through shall be deleted. Underscored words and/or figures shall be added. The remaining provisions are now in effect and remain unchanged. Please note the contents herein and reflect same on the documents you have on hand. The bid closing date and time has been changed to: Wednesday, June 30, 2010 at 12:00 PM. The following line # 3.3, Pant Shell— Replace Wristlet Only, has been deleted from the IFB. Section 3.1, Specifications/Scope of Work, has been amended as follows: SERVICE: Successful Bidder shall be responsible for the inspection, cleaning and repairs of ensembles or any part thereof, in accordance with the NFPA 1851 guidelines. The Successful Bidder shall: • Provide secure 1eGka le storage containers for shipping. Incur all cost of shipping from and to the City. Scan all gear (those that have barcodes already) using a barcode scanner or some other approved method of identification. The Successful Bidder shall also email a pickup list to the Contract Administrator or his designee listing the bunker pear that is picked up from the Department -of Fire -fescue- and- received- at -#he- Successful --Biddees_facility_._4hat_,.,;n .,„1,,d9 the p;Gk Lip Gl.,+o RaMe and SGhed ied deliyeF„ date Said list will be used by the Contract Administrator or his designee to verify that the Successful Bidder received all gear that was shipped. The pickup list must include the following information: o Quantity of secure storage containers picked up and received by the Successful Bidder. o Description of the contents in each container, such as: ■ Bunker gear component (i.e. coat, pants ... etc) • Quantity of each bunker gear component ■ Bunker gear component serial number or name of firefighter if serial number is not legible - • Date bunker gear was picked up from the Department of Fire -Rescue ■ Scheduled delivery date of bunker gear that has been inspected, cleaned and repaired. • Have a turn -around time to original location, with finished gear, of no more than 96 hours after r-esefviflg picking up contaminated gear and/or gear in need of repairs from the Department of Fire -Rescue. Specialized cleaning may be needed to decontaminate gear exposed to blood-borne pathogens, chemicals, and other substances. Page I Q1. Please clarify the requirements of being classified as a local office, pursuant to Section 1.49, Local Preference Al. The requirements of being classified as a "Local Office" are found in Section 18-73, of the City of Miami Procurement Ordinance, which states: "Local office means a Business which conducts all or a portion of its operations from a permanent structure that is located within the corporate limits of the City of Miami, and has operated legally pursuant to all applicable zoning and licensing laws for a minimum of six months prior to the date Bids or Proposals were received for the Purchase or Contract at issue. A Post Office Box shall not be sufficient to constitute a Local Office within the City of Miami. If the Business is located in the permanent structure pursuant to a lease, such lease must be in writing, for a term of no less than one year, have been in effect for no less than six months prior to date Bid or Proposal was received, and be available for review and approval by the Chief Procurement Officer or its Designee." 02. Section 2.14, Liquidated Damages, states: "Bidder shall provide all items at the guaranteed delivery time specified by the City in the Specifications Section of this solicitation. If the successful bidder fails to deliver within the specified delivery time, it is understood that 1% of the unit price per calendar day per item will be deducted, as liquidated damages, for each day beyond the specified delivery time (not to exceed the total amount of the contract), not as a penalty but as a form of compensation for the loss of not having the ability to use the product." Please clarify as this RFQ is a service contract not a product contract so a unit price is not applicable. A2. Although this is a service contract, a unit price is still required for the inspection, cleaning, and repairs of the bunker gear. Therefore, if a successful bidder fails to deliver items within the specified time, 1% of the unit price per calendar day will be deducted, as liquidated damages, for each day beyond the specified delivery time (not to exceed the total amount of the contract). For example, if the cost for Advance Cleaning is $50.00, prospective bidder will be deducted .50 cents (1% of $50.00) for each calendar day beyond the specified delivery time. Q3. Upon reviewing RFQ 210224 - Bunker Gear Inspection, Cleaning, and Repairs, we have a question regarding Line Item 3.3 Pant Shell - Replace Wristlet Only. This appears to be an error since the pants do not have a wristlet. Should we ignore this line item or do you want to re -define it? A3. This line item has been removed from the Invitation for Bid. - -Q4. -On-May ll-,-2010-RFQ 208196, -1 --was- issued -by-Ramon Berges for the_.same services was this RF -Q- not accepted thus the reason for issuing another RFQ? A4. Invitation for Quotation 208196,1 (RFQ 208196,1) was accepted and was issued as an informal bid to address an immediate need of the Department of Fire -Rescue for bunker gear inspection, cleaning, and repairs. Invitation for Bid No. 210224 (RFQ 210224) is a formal bid issued to establish a term contract for the provision of these same services. Q5: Section 3.1, Specifications/Scope-of Work, states: "SERVICE: Successful Bidder shall be responsible for the inspection, cleaning and repairs of ensembles or any part thereof, in accordance with the NFPA 1851 guidelines. The Successful Bidder shall: • Provide secure lockable storage containers for shipping." Page 2 Please clarify does this mean the use of containers secured with padlocks or with normal packing tape? A5: Successful Bidder will be allowed to provide containers secured with normal packing tape. Q6: Section 3.1, Specifications/Scope of Work, states: "The Successful Bidder shall also email a pickup list to the Contract Administrator or his designee that will include the pick-up date, name, and scheduled delivery date." The previous statement implies the use of UPS / FedEx would be acceptable as long as lockable containers are used, but the statement supplying a pick up list implies the vendor would be doing the pick-up and delivery. Will the RFQ allow for the use of shipping agents such as UPS or FedEx? A6: Yes, successful bidder can use shipping agents such as UPS, FedEx, or any other common commercial carrier. 07: Section 3.1, Specifications/Scope of Work, states: "Have a turn -around time to original location, with finished gear, of no more than 96 hours after receiving contaminated gear and/or gear in need of repairs." Does the 96 hour turn time begin when the vendor receives delivery of the shipment through freight forwarding company? A7: No. As amended through this addendum, the 96 hour turn -around time begins when the gear is picked up from the Department of Fire -Rescue.
