The City of Miarni Plat and Street Committee, at its meeting of .January 7, 2010, approved the above
tentative plat subject to the following; revisions being matte to the tentative pint, additional information
Ding provided and/or variances being granted. Pleme be advised that the processing of your tentative plat
cannot proceed until these conditions have been satisfied:
1. Location Sketch
a) Draw the lot lines of the underlying plat and of the adjacent plats.
b) Label the Lot and Flock numbers, Subdivision names, and Flat Book and Page numbers
of the underlying and adjacent plats.
c) Show zoning designations and zoning district litres.

form, and provide the name and title of person signing.

application and the proposed plat on behalf of owner.
4. Show the relationship between FDG Flagler Station H LLC a Delaware limited liability company
(signing the application), and FIG Downtown LLC a Delaware limited liability company (owner
by SPECIAL WARRANTY DEED provided).
5. Provider, from the Secretary of State, the most current Certificate in Good Standing,.
f, Provide an Opinion of Title for the remnant portions of the SPECIAL WARRANTY DEED not
being platted,
7. Provide an Opinion of. Title for the lands being dedicated as Right of Way for NVQ I" Avenue not
included in the SPECIAL WARRANTY DEED provided.

13. Provide proof of ownership for the portions of NW V Stmo-414W 0 Street, and NW 10 Avenue

1.4. Indicate, the 111011T OF WAY area, TO BE DEDICATED BY THIS PLAT.
15. Indicate the RIGHT OF WAY area TO BE CLOSED AND VACATED BY THIS'PLAT.
16. Provide a breakdown of the gross ares, the area of tho proposed tracts, the area of the right of -way
to be closed and vacated by this plat, and the area of the right of, way to be dedicated by this plat,
in square feet and in acres.
17, Provide a Searing and Distance for all the boundaries of the proposed tracts.
18. Shaw the improvements and the current use ofthe land of the proposed plat.
19. Provide geometry information for the full right of way width of the portion of N'11U' I` Avenue
being realigned.
20. A letter from Comcast is rewired to determine, if any adjustments to-thoir facilities or easeme,nts
are required.
21. Verify with the Miami -Dade County that the, proposed plat name is accvptablo.
22. All encroachments across proposed plat tracts and/or lot lines must bo remQved prier to final -plat
submittal.
23. An opinion of title, in the City of. Miami opinion of title form must be provided at the time of the
final plat submittal,
24. Currant backup documentation will be required for all who execute the final plat. A resolution
for authority to execute documents and a Certificate of Good standing from the Secretary of State
are required, if applicable.
25. Tentative plat application must be made with Miami -Dade County after receiving approval from.
the City of Miami Plat and Street Committee.
26. Be advised that an incomplete final plat package will not be accepted by the City of Miami. It is
the owner's responsibility to work with his surveyor and his attorney to assure that everything is;
in order before submitting the final plat package.
27. He advised that if all the requirements for scheduling the final plat for commission action a,b not
in order, the final plat will not be scheduled for a City Commission meeting.
28. An opinion of titles as to the reversionary rights for the right-of-way to be closed and vawbd will
be required by the Zoning Board. The, opinion of title must also address whether or not there, are
individuals, in addition to the abutting property owners, having an interest in the rigpt-of--way to
be closed and vacated. A copy of the opinion of title must be provided to the, Public Works
Department.
29, The Zoning Board will require a sketch and legal description with a square footage, on an 8112"
X II" pager, for the proposed closure. Contact the Office of the Hearing Boards, at
305.416,2030. Provide a copy to the Public Works Department
30. Provide a letter from the Miami -Daft Transit Agency approving the plat.
31. Provide a zoning verification letter from the City of Mimi Zoning Administrator.
32, Provide existing elevations at the back of sidewalk, centerline of the struts and P.C. / P.T. of the
curbs.
33. Correct the curb alignment on N.W. V Avenue at the Dort of Miami railroad spur.
34. Verify if any of the tracts are located within archaeological, historic or environmental
preservation districts. Contact the City's Planning Department of requirements / regulations.
35. Add property lines as shown on the warranty doors,
36. The Flat and Stre:e,t Committee has reviewed the tentative plat of Flagler Gran Central Station and
determined that all technical requirements contained in the, Miami City Code Subdivision
Regulations have been met and has approved the, ,tentative plat. The members of the Plat and
Strut Committee have further considered the request for vacation and closure of the streets with
respect to Miami City Cade: requirements and have voted 7 in favor and 0 in denial of this
vacation and closure request.

In addition to the above requirements, you should be aware of the following,
1. State and local laws require the installation of various physical improvements in the public rights-
of-way
ights-
of way when property is platted. These subdivision improvements include paving, drainage,
landscaping, sidewalks, etc. In some cases this could repmsent a substantial. investment. on your
part. OPTION: The submission of an electronic copy of the tentative plat, in an AutoCA-D,
version, to the City of Miami Public Works Development and lzoadway Plans Section Will
EXP13DIU. the preparation and enhance the ACCURACY of the subdivision improvement
estimate letter required for the final plat approval.,
2. The alteration, relocation or installation of utilities such as storm and sanitary sewers, electric,
telephone, water, etc., caused by this plat will be at the property owner's expense. Also, utility
easements may be required on the properly being platted.
3. A building permit will not be issued on the property being platted. until the Final plat• is recd riled
or as authorized by City of Miami Cade Section 55,1 b(h). Also, the Certificate of Occupancy for
any building construction will be issued only after all the required subdivision improvements
have been completed.
4. ,Approval for fire flow requirements must be obtained from the Fire -Rescue Department prior to
the issuance of a building permit.
S. Additional items must bo provided to the City of Miami Department. of Public Works b6fore the
final plat is submitted to the City Commission for -approval. You will be %notified in writing as to
what these items are after the amount of the bond has been determined for the necessary
subdivision improvements.
G. 'Tentative plat approval is only ya lid for one (1) year and sit (G) months from they date of the Plat
and Street Committee meeting at which time is was approved.
