MIGOYA City Manal-ler ADDENDUM NO. 1 IFB No. 206189 April 26, 2010 Invitation For Bid (IFB) for Child Care Food Program TO: ALL PROSPECTIVE BIDDERS: The following changes, additions, clarifications, and deletions amend the Bid Documents of the above captioned IFB, and shall become an integral part of the Contract Documents. The remaining provisions are now in effect and remain unchanged. Please note the contents herein and reflect same on the documents you have on hand. Q1. Attachment 6 (page 31) indicates there are three locations, each serving 79 breakfasts, 105 lunches, and 105 afternoon snacks daily. This conflicts with Attachment 7 (page 32) that indicates a total of 79 breakfasts, 105 lunches, and 105 snacks per day for all locations. Can you please clarify? Al. Attachment 7 of the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010 has been revised and attached to this addendum to reflect the correct estimated total number of meals per day required for all three day care centers listed in Attachment 6. Prospective Bidders shall submit with their bid response the revised Attachment 7 enclosed in this addendum. In addition, the file document titled "Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010" and located in the "Header/Notes and Attachments" section of this IFB, in the Oracle iSuppfier/Sourcing system has been updated to include the revised version of Attachment 7. Q2. Attachment 6, column 6, indicates three deliveries per day per location. is that correct or is column 6 actually indicating serving times for one delivery per day per location? A2. Yes, it is correct. Column 6, titled "Desired Delivery Time(s)", of Attachment 6 in the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010 indicates that the Successful Bidder must make a total of three (3) deliveries per day, per location. Q3. I am still not sure how to ask questions about the bid or if we submit on-line or in writing. A3. Prospective Bidders shall ask and submit questions in accordance with Section 2.2, Deadline for Receipt of Request for Additional Information/Clarification, of the City's Invitation for Bid document, which states: "Any questions or clarifications concerning this solicitation shall be submitted by email or facsimile to the Purchasing Department, Attn: Yusbe/ Gonzalez, fax. (305) 400-5104 or email: yaonzalezO)ci.miami.Aus. The solicitation title and number shall be referenced on all correspondence. All questions must be received no later than Thursday, April 22, 2010 at 5:00 PM. All responses to questions will be sent to all prospective bidders/proposers in the form on an addendum. NO QUESTIONS WILL BE RECEIVED VERBALLY OR AFTER SAID DEADLINE." Page I Prospective bidders may submit their bid responses electronically or in hard copy format in accordance with the instructions provided in Section 1.79, Submission and Receipt of Responses, of the City's Invitation for Bid document, which states: 1.79. SUBMISSION AND RECEIPT OF RESPONSES - Responses shall be submitted electronically via the Oracle System or responses may be submitted in hardcopy format to the City Clerk, City Hall, 3500 Pan American Drive, Miami, Florida 33133-5504, at or before, the specified closing date and time as designated in the IFB, RFP, RFQ, or RFLI. NO EXCEPTIONS. Bidders/Proposers are welcome to attend the solicitation closing; however, no award will be made at that time. A. Hardcopy responses shall be enclosed in a sealed envelope, box package. The face of the envelope, box or package must show the hour and date specified for receipt of responses, the solicitation number and title, and the name and return address of the BidderlProposer. Hardcopy responses not submitted on the requisite Response Forms may be rejected. Hardcopy responses received at any other location than the specified shall be deemed non-responsive. CITY HALL IS AT THE END OF PAN AMERICAN DR. PARKING IS ON RIGHT. B. Facsimile responses will not be considered. C. Failure to follow these procedures is cause for rejection of bidlproposal. D. The responsibility for obtaining and submitting a response on or before the close date is solely and strictly the responsibility of BidderlProposer. The City of Miami is not responsible for delays caused by the United States mail delivery or caused by any other occurrence. Responses received after the solicitation closing date and time will be returned unopened, and will not be considered for award. E, Late responses will be rejected. F. All responses are subject to the conditions specified herein. Those which do not comply with these conditions are subject to rejection. G. Modification of responses already submitted will be considered only if received at the City before the time and date set for closing of solicitation responses. All modifications must be submitted via the Oracle System or in writing. Once a solicitation closes (closed date andlor time expires), the City will not consider any subsequent submission which alters the responses. H, If hardcopy responses are submitted at the same time for different solicitations, each response must be placed in a separate envelope, box, or package and each envelope, box or package must contain the information previously stated in 1.82.A. Q4. Are there 105 lunches at each site every day? A4. Yes. Successful Bidder will be required to serve an estimated total amount of 105 lunches per location, per day. Q5. Do these sites take extra children in the summer? A5. No. The day care centers do not take extra children during the summer. Page 2 Q6: Are the day care centers equipped to keep foods at the required temperature or will the successful bidder be required to provide equipment to keep foods hot or cold? A6: No, the day care centers are not equipped to keep foods at the required temperature. However, as stated in page 7 of 38, Food Preparation Requirements of the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010: (...J 3. All cold foods must be held and delivered at or below 41 ' F. 4. All hot foods must be held and delivered at or above 140'F. (...J" Q7. If the successful bidder is required to provide equipment to keep foods hot or cold, will the successful bidder be required to provide equipment for all or some of the day care centers? If so, what equipment will need to be purchased and how much time will the successful bidder have to provide the equipment after an award decision is made? A7. The Successful Bidder is not required to provide the City with equipment to maintain foods hot or cold once they are delivered. However, as stated in page 7 of 38, Food Preparation Requirements of the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010: [...J 3. All cold foods must be held and delivered at or below 41 ° F. 4. All hot foods must be held and delivered at or above 140'F, (...J" Q8. With respect to the deliveries, there are three different delivery times (breakfast 7:00 am, lunch 11:00 am, snack 2:00 pm). My question is: does each day care center want each type of meal to be delivered individually or can the snack be delivered with the lunch? A8: The Successful Bidder will be required to deliver each type of meal individually at the required delivery location and time stipulated in Attachment 6, Delivery Schedule, of the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010. Successful Bidder will not be allowed to deliver more than one type of meal per delivery. Q9: Do the references have to be from schools which have been previously served the same menu? A9: No. References do not have to be from schools which have been previously served the same menu shown in Attachment 3 of the Florida Department of Health Invitation to Bid, Standard Contract FFY 2009-2010. Q10: After the City makes an award decision, when does the contract begin? A10: Pursuant to Section 1.10, subsection D), Award of Contract, of the City's Invitation for Bid document, "The term of the contract shall be specified in one of three documents which shall be issued to the successful Bidder/Proposer. These documents may either be a purchase order, notice of award and/or contract award sheet." ALL OTHER TERMS AND CONDITIONS OF THE IFB REMAIN THE SAME.
