Action Community Center, Inc., a Non -Profit Community Based Organization,
established for the purpose of serving the Elderly and/or Handicapped, since
its inception in 1971.
Action has been providing for over thirty fourth years, door to door, free of charge
fundamental transportation frequently driving them to medical and hospital
appointments and ongoing treatments, food procurements outlets, specialty
stores, social security offices and a long list of governmental agencies and
offices, for their overall well-being and necessities.
Transportation services for the older population make good sense for several
reasons, May of the riders are not physically able or drive an automobile,
including operation expenses and insurance.
Transportation is a necessary part of any effort to deepen the involvement of
older people in community life.
Our agency has established procedures for transportation services and may be
used for assessing clients status, conditions and needs for establishing priorities
for transportation services, some older persons with high probabilities of
services are:
1. Advance age
2. Low and/or moderate income
3. Living along or with non -relatives
4. Functional impairment or disability
5. Insulation and lack of access to social and recreational activities, etc....
To be eligible for transportation services you must be:
1. Sixty (60) years of age or older
2. Handicapped any age
3. Low and/or moderate income
4. Resident to the City of Miami
PROGRAM OBJECTIVES:
Access to most health and social services is dependent on the availability
of transportation services, there is a need to provides transportation
services that links the elderly and handicapped and low income residents
with medical and other basic and urgent family needs.
Action Community Center, Inc., has been serving the City of Miami residents
for the past thirty years, providing door to door, free of charge transportation
services to doctor's and hospital's appointments, food procurement,
physical and medical therapy, hot meals programs, rehabilitation and dialysis
centers, nursing and boarding homes, radiation and cancer therapy, government
Offices, activities centers, restaurants and a long list of agencies for their well
being a necessities.

DESCRIPTION OF THE PROGRAM:
Transportation is the key to the independent of older persons, and their access
to health, social leisure and other essential services to reach the unmet need
and to assist frail older and handicapped persons in our community for_ their
well being a necessities.
Our agency headquarter are located in the Little Havana are where a high level
of social and economic activity is synonymous with the health of the neighborhood
Fife conditions have change in the Little Havana and the disposable income of the
residents has decreased in the last eighteen years due in part to the out migration
of the higher residents and immigration of lower income thus affecting the neighborhood,
the target is economic divestment is also the result if image deterioration, relative un
attractiveness, crime and social problems. All this can be attributed to the arrival of the
immigrants (many undocumented), and an increase in the number of poor including a
large older community.,
Action Community Center, Inc., has development a close working relationship
with agencies in our community to ensure that its elderly and handicap participants
utilize the resources.
Elderly and handicapped who are referred for services or who request transportation
service will be screened by our staff (by phone) during an intake process, and we have
the flexibility to establish reasonable and consistent procedure for prioritizing the clients
needs.
Action Community Center, Inc_, provides transportation services to different hot meal programs
in the community,.such as Allapattah Community Action, De Hostos Senior Center, UTD Towers,
Carroll Manor, etc.
PROGRAM TIME LINE.
During Fiscal Year 2008-2009 we will provides transportation services to 500 clients
And 65,000 one-way trips, from Monday to Friday and occasional trips on Saturday
And Sunday.
TARGET AREA: CITY-WIDE
ORGANIZATIONAL EXPERIENCE:
Action Community Center, Inc. has been serving the City of Miami
Residents for the past thirty fourth years, has been providing door to door,
free of charge transportation services to the elderly and handicapped
residents for their a being necessities.

TARGET POPULATION:
Our Target Population are Elderly and Handicapped (any age) with low and/or
moderate income, most of them immigrants.
DEMOGRAPHIC OR OTHER FACTOR:
Action Community Center, Inc., provides for the past thrity four years transpor-
tation services to the elderly and handicapped persons without regard of sex,
race, national origen or creed with medical, food procurment and other basic
and urgent needs.
RECRUITMENT STREATEGIES:
We are inform to our participants with flyers, govermental programs, froi from
doctors offices, the signs in our buses, friends, etc.
PARTICIPANT'S ELIGIBILITY:
We are select the eligible for transportatio9n services a person must be sixty
year of age or older, handicapped (any age) resident in the City of Miami with
low and/or moderate income.
OUR GOALS:
Our purpose is to help our elders and handicapped to age with dignity, and to
help them to maintain their independence.
PROGRAM STAFF DO WITH PARTICIPANTS:
Dispatcher supervises the staff and organizes the transportation services
includings intakes and phone calls from participans. Our Drivers transport
them from their homes to specific location, to fill their necessities.
