There are five Boys & Girls Kip Rabin Club facilities that now serve 13,000 members, participants, and C. Fred Rebozo spectators in Miami. Boys & Girls Clubs of Miami-Dade is an established Richard Sepler 'President Emeritus entity, well known by all the surrounding neighborhood areas. Over 60% EXECUTIVE DIRECTOR of youth crime is committed between the hours of 2-6pm. With Our Alex Rodriguez-Roig programs, we help decrease the number of children in the streets in the DIRECTOR OF OPERATIONS after-school hours, as well as during the weekends. Saavedra & SW 32nd Avenue (2805 SW 32nd Ave.), provides year-round after- DIRECTOR OF SPECIAL EVENTS School, sports and summer programs to over 1,500 youth from Miami and AND DEVELOPMENT neighboring communities. Our Hank Kline facility enjoys well-regarded Mayra Gonzalez Rey and established baseball & basketball programs, which bring many family COMPTROLLER members and residents to our address on a nightly basis. Additionally our Thomas M. Winston, Jr. annual pumpkin sales and Christmas tree sales bring many more Miami DIRECTORS residents to our location during these seasons. The location of our Hank Roger N. Arsham Richard Baker Kline Club also attracts local media exposure year-round. This past year Alan S. Berg we have hosted events where ESPN, Channels 4, 6, 7 & 10 and The Matt Bernhardt Thomas J. Bezold Miami Herald have sent correspondents to cover newsworthy events. How the applicant's long-term goals and organizational mission are met by the program. Our Hank Kline Club is Boys & Girls Clubs of Miami-Dade's original club and has been part of the Miami landscape since 1940. We have been at the Northeast corner of US 1 & SW 32nd Avenue for nearly 70 years and plan on being here for many more years. Boys & Girls Clubs of Miami -Dade, Inc.'s mission is to enable all young people, especially those who need us most, to reach their full potential as productive, caring and responsible citizens. Our programs help get kids off the street and get them involved in programs like after-school care, academic improvement and homework assistance, organized leagues, such as: basketball, baseball, football and cheerleading. The above programs are offered on a year-round basis in order to keep the children occupied as long as possible in a healthy, safe, productive, and enjoyable environment. The after-school and academic improvement and homework assistance programs are designed to assist the children of predominantly blue collar working families. With the requested CDBG funding we shall be able to continue to provide the highest quality youth services in an area of the community where parents will continue to consider us a safe haven for their children. The beautification & renovation project will be a nonverbal message to local residents and government that Boys & Girls Clubs of Miami -Dade is committed to this city block in Miami for the long-term. 3. How the proposed Public Service, Economic Development and/or Emergency Shelter Grant activity furthers CDBG and/or ESG Program objectives and priorities as well as addresses the needs identified by the applicant. This proposed beautification project directly addresses objectives and priorities identified by the City of Miami's Economic Development component of the CDBG. In accordance with the 2004-2009 City of Miami Consolidated Plan, this project would address one of its core concepts, Neighborhood -based community development. Additionally under the terms stated in the city's 2008-09 CDBG Economic Development Program Overview, we shall be specifically addressing "Public Facilities and Improvements" by undertaking this beautification project on our own. Furthermore Boys & Girls Clubs of Miami -Dade, Inc. is a private non-profit Community Based Organization (CBO) and qualifies as a company eligible for funding under the Economic Development program for activities that benefit underserved areas and/or very low-, low- and moderate -income persons. Finally in accordance with the purpose of this CDBG grant we will meet the City of Miami's objective: "To create quality of life enhancements that increase access to quality facilities and services, improve the safety and livability of neighborhoods, restore and preserve natural and physical features of special value for historic or architectural reasons and conserve energy resources." 4. The applicant's track record for successfully performing the Public Service, Economic Development and/or Emergency Shelter Grant related service/activity. Boys & Girls Clubs of Miami -Dade, Inc. has a very successful track record for successfully performing Economic Development related services &/or activities. In the past five years we have received funding for capital projects from Miami -Dade County, the City of Miami Beach and the United States Tennis Association (LISTA). The projects for Miami -Dade County's Parks & Recreation Department have involved the following: fencing for baseball fields & upgrading retractable rows at our Gwen Cherry Park Club's basketball gym; creation of a shade structure/pavilion at our Northwest Club; and most recently funding to fix lightpoles at our Northwest Club. The capital improvement grant we received from the LISTA in 2007 helped us refurbish the tennis courts at our Hank Kline Club for our now successful tennis program. Lastly we have been awarded from both Miami -Dade County's Building Better Communities General Obligation Bond ($750,000) and the City of Miami Beach's CDBG ($300,000) funding toward our South Beach Club Capital Campaign due to begin prior to the end of 2009. DESCRIPTION OF PROGRAM 1. Carry out the details of the program, including verification and compliance requirements. In detail the proposed beautification project for the south side of our Boys & Girls Clubs of Miami-Dade's Hank Kline Club that faces US 1 between SW 31St & 32"d Avenues consists of: ■ Removal of the Brazilian Pepper trees and all accumulated trash & debris- $20,000; • Replacement of chain link fence with wrought iron security fence work- $75,000; ■ Installation of lights- $15,000; ■ Benches- $5,000; ■ Sidewalks- $20,000; ■ Landscaping, including green space for parents & children alike to enjoy- $15,000; ■ Total- $150,000. Boys & Girls Clubs of Miami -Dade, Inc. will be responsible for obtaining all necessary permitting and will oversee all subcontracted personnel to assure all aspects of the project satisfactorily comply with city & county codes. 2. The Public Service, Economic Development and/or Emergency Shelter services that will be provided, the number of clients who, or units which, will be served, who will provide the services and how the services will be provided. Boys & Girls Clubs of Miami-Dade's Hank Kline Club Beautification Project is an Economic Development project since it is in accordance with the 2004-2009 City of Miami Consolidated Plan. This beautification project addresses the Neighborhood -based community development core concept. Also under the terms stated in the city's 2008-09 CDBG Economic Development Program Overview, we are specifically addressing "Public Facilities and Improvements" by undertaking this beautification project on our own. We can state that we are directly impacting the 1,500 youth we serve annually with our programs and their families, but indirectly this project will serve the 12,814 residents of US Census Tracks 70.02 and 71.00 who reside within or across the proposed project site. The subcontractors to be used will all have the proper licenses to do business in the City of Miami this includes landscapers, iron/metal works, electricians and concrete slab construction. If funded, project will begin immediately and adhere to agreed upon timelines with the City of Miami. 3. Track and monitor the use of funds provide regular reports and program evaluation. Boys & Girls Clubs of Miami -Dade, Inc. has an excellent track record of submitting grant funding reports to all of its funding sources. Our organization shall adhere to any reporting schedule developed by the City of Miami. Whether reports are monthly, quarterly or bi-annually, they will be thorough and provide all project accomplishments through the report's end date. If reports require a detailed expenditure itemization, this too shall be provided. Additionally we shall supply the city with a copy of our annual audit for the finance department's records. As to program evaluation, we shall provide, if requested, a project synopsis after completion that details the successes and unanticipated difficulties. This will serve as a learning tool for any similar projects undertaken in the future. 4. Implement quality assurance measures and performance measures, coordinate all related services and coordinate the program with other agencies. Please list outcome measurements for the proposed Public Service, Economic Development and/or Emergency Shelter services. As far as quality assurance measures, our agency shall work closely with any & all city officials (building & zoning personnel, code compliance officials, city engineers and law enforcement) needed for project approval as to the 2004-2009 City of Miami Consolidated Plan, specifically regarding Public Facilities and Improvements. Our outcome measurements for this project are: ■ Successful removal of Brazilian Pepper trees (an invasive species) and debris on the south side of the Hank Kline Club between SW 31' & 32nd Avenues facing US 1 in accordance with the City of Miami's 2004-09 Consolidated Plan. ■ Successful removal of existing chain link fencing along the south side of the Hank Kline Club between SW 31St & 32nd Avenues facing US 1 and construction & installation of wrought -iron fence work in accordance with the City of Miami's 2004-09 Consolidated Plan. ■ Successful landscaping and sidewalk reconstruction and installation along the south side of the Hank Kline Club between SW 31St & 32nd Avenues facing US 1 in accordance with the City of Miami's 2004-09 Consolidated Plan. All proposed outcome measurements also address the city's 2008-09 CDBG Economic Development Program Overview, "Public Facilities and Improvements", by our agency undertaking this beautification project on our own. 5. Program Time Line: Applicant must be able to demonstrate the capacity to implement the program quickly and provide a detailed start-up schedule. Should Boys & Girls Clubs of Miami -Dade, Inc. be awarded CDBG funds to undertake our Hank Kline Club Beautification Project by June 2009, our timeline will begin immediately so that we may complete the proposed project by December 31, 2009. This is our proposed project timeline for the project: ■ April 2009- Submit proposed Beautification Project to City of Miami for review and approval. ■ June 2009- If awarded, meet with all subcontractors to recertify that all needed licenses are current and begin the process of obtaining required permits. ■ July 2009- Meet with any city government officials with the detailed plan and scope of project. ■ August 2009- After obtaining permits, have landscaping subcontractor remove all Brazilian Pepper trees and accumulated debris. ■ September 2009- Concrete subcontractor to repair or replace damaged sidewalks with new concrete sidewalks. • October 2009- Chain link fencing removed and brand new iron fence work installed on south side of our Hank Kline Club property from SW 31St Avenue to SW 32"d Avenue facing US 1. November 2009- Electrician to install new lighting and landscaping subcontractor to install new sod in designated areas where concrete was not poured and paved. December 2009- City of Miami Code Compliance Officials to review project and approve final beautification project. Also special event to be coordinated with City of Miami's Mayor's and Commission Office to celebrate joint effort. PROJECT READINESS AND ABILITY TO PROCEED 1. Applicant must be able to demonstrate the capacity to implement the project on a timely basis and provide a detailed construction -related timeline/schedule. As stated previously Boys & Girls Clubs of Miami -Dade, Inc. has a very successful track record for successfully performing Economic Development related services &/or activities on a timely basis. We have repeatedly demonstrated to Miami -Dade County, the City of Miami Beach and the United States Tennis Association (USTA), our ability to successfully complete funded capital campaigns. Miami -Dade County's Parks & Recreation Department has funded our agency to provide fencing for baseball fields & upgrading retractable basketball gym rows; creating a shade structure at our Northwest Club; and to fix lightpoles at our Northwest Club. The capital improvement grant from USTA in 2007 helped us refurbish the tennis courts at our Hank Kline Club for our now successful tennis program. Each of these grants had strict deadlines by which we were to have our capital project completed. In each case we were able to complete each capital project within the timeframe provided to our agency. In regards to a detailed construction -related timeline/schedule, since this project is an outdoor beautification project, the detailed timeline shall not greatly differ than the aforementioned timeline previously provided (Description of Program, Answer # 5). 2. Applicant must provide a Cost Allocation Report of total project costs which includes a detailed breakdown of all hard costs, soft costs, financing costs and reserves by funding source. All detailed breakdown of costs will be provided with each and all payment requests and progress reports, if funded. These reports will contain copies of vendor invoices, cancelled checks and subcontractor agreements for the City of Miami to review. For now the following represents all associated costs with the proposed beautification project: Line Item City of Miami CDBG Cost Boys & Girls Clubs of Miami - Dade, Inc. Should this beautification project require additional funding, other than the proposed budget, Boys & Girls Clubs of Miami -Dade, Inc. shall provide the funding. We will use private donations and dedicated proceeds from various fundraising events, such as our annual golf tournament and gala dinner, to supplement any additional costs. 3. Applicant must be able to demonstrate that organization has site control and provide supportive documentation (i.e., Title, deed, lease, etc.) Our proposed beautification project is located on land owned by our foundation arm, Greater Miami Boys' Clubs Foundation, Inc. The existing property is approximately 10 acres in size. We have been incorporated since 1940 and have occupied this land since 1961. Copies of the owner & building information from www.miamidade.gov and a site map have been included in this proposal narrative for review. A copy of our actual deed may also be supplied if requested. The proposed beautification project will be overseen by our Executive Director, Alex Rodriguez-Roig, who has served in his capacity for nearly 6 years. The day-to-day supervision & management of the beautification project will be handled by Rachel Fernandez, Director of Operations. She will be agency representative to our various vendors and subcontractors during the project timeframe.
