Failure to provide the requested information may result in the consideration of your request being either delayed or denied. Requests brought before the committee must be directed toward dismantling or disrupting Drug Trafficking Organizations (DTOs) connected with violent activities. You will be required to appear before the VCDTO Sub -Committee to discuss this funding request. Yes ❑ No X The following are requirements for the submission of a request before the Sub -Committee: t Requesting agency must have at least one full-time member presently assigned to a H DTA initiative. j The head of the requesting agency certifies the need for the requested funds and that sufficient funds are not available from other sources. The head of the agency must also agree that H DTA funds will not be used to supplant existing funds. f Applications must be concurred with by a State or Federal prosecutorial entity, depending on the nature of the case. f Once the above step is accomplished, the application must be submitted to the Office of the VCDTO Coordinator for processing and follow-up. f A detailed narrative description of the case must be provided along with the application as indicated below in the General Information & Reimbursement Funding Procedures section. GENERAL INFORMATION & REIMBURSEMENT FUNDING PROCEDURES The maximum annual funding for a single initiative is $30,000. This funding shall be utilized by the requesting agency for the purpose of officer overtime only and will not be used to pay for officer fringe benefits. Reimbursement Funding Procedures (CONTINUED) In addition to completing all of the information detailed on this application, please include a narrative description of the case for which funding is being requested by completing the Narrative Description of Investigation Form located below. A monthly accountability report detailing the progress of the investigation including statistical information depicting arrests and seizures must be forwarded to the Office of the Initiative Coordinator. This report should arrive at the aforementioned location no later than the 5'b day of the month. A claim for the overtime expenses (with supporting documentation consisting of copies of officer overtime slips, timesheets and payroll documentation) will be forwarded to the South Florida H DTA Fiscal Officer within 45 days of the end of the month. Once the investigation is completed, a detailed report depicting the level of damage caused to the criminal DTO i.e. dismantled, disrupted, number of arrests, type of arrests, subjects names, addresses, police case numbers, assets seized or confiscated and any and all pertinent information that may be required by the VCDTO Initiative Coordinator must be submitted. BACKGROUND AND FACTS OF INVESTIGATION: The subjects that are mentioned in this initiative have an extensive history in dealing with the sale and distribution of narcotics. Several of the subjects have been involved directly or indirectly with numerous homicides and shootings within the Overtown area. 2. DESCRIPTIVES: a. Description of Organization: The "Guess Who's Back" initiative consist of several organizations that are responsible for the drug trade that is being facilitated in the Overtown area. With the widespread of drugs within the community, the citizens are becoming victims and spectators of the on going violence. b. Geographic Scope: The Overtown area is considered a historical site located within the City of Miami jurisdiction. It is bordered by tourist establishments such. as Bayside, the Center of the Performing Arts and Port of Miami. c. Number of Participants: See power -point presentation d. Iflegal Activities: The majority of the subjects mentioned in this initiative began their criminal career by selling marijuana and committing violent crimes.
