A RESOLUTION OF THE MIAMI CITY COMMISSION, WITH ATTACHMENT(S),
AMENDING RESOLUTION NO. 08-0428, ADOPTED JULY 24, 2008,
ESTABLISHING A SPECIAL REVENUE PROJECT ENTITLED: "PROGRAMS FOR
THE DEVELOPMENTALLY DISABLED, 2008-2011," BY APPROPRIATING FUNDS
FOR ITS CONTINUED OPERATION IN THE ESTIMATED ADDITIONAL AMOUNT OF
$199,418 TO THE PREVIOUSLY APPROPRIATED AMOUNT OF $600,582 FORA
TOTAL OF $800,000; AUTHORIZING THE CITY MANAGER TO ACCEPT
ANTICIPATED PAYMENT FOR SERVICES IN THE ESTIMATED TOTAL AMOUNT
OF EIGHT HUNDRED ($800,000) FROM THE STATE OF FLORIDAAGENCY FOR
HEALTH CARE ADMINISTRATION ("AHCA") FOR MEDICAID WAIVER SERVICES
MEDICAID PROGRAM DEVELOPMENT, THE STATE OF FLORIDA, DEPARTMENT
OF EDUCATION, DIVISION OF VOCATIONAL REHABILITATION ("DOE/VR") FOR
EMPLOYMENT AND SUPPORTED EMPLOYMENT SERVICES, AND THE SOCIAL
SECURITY ADMINISTRATION ("SSA") FOR EMPLOYMENT AND SUPPORTED
EMPLOYMENT SERVICES AS AN EMPLOYMENT NETWORK CONTRACTOR FOR
THE TICKET TO WORK AND SELF SUFFICIENCY PROGRAM; WITH SAID
INCREASE BASED ON THE CITY OF MIAMI'S ABILITY TO PROVIDE ADDITIONAL
SERVICES THROUGH THE DOE/VR AND SSAAND ELIGIBLE DISABLED
PARTICIPANTS; FURTHER AUTHORIZING THE CITY MANAGER TO APPLY FOR,
RENEW, NEGOTIATE AND EXECUTE MEDICAID WAIVER SERVICES
AGREEMENT(S), IN SUBSTANTIALLY THE ATTACHED FORM(S), AND ALL
OTHER NECESSARY DOCUMENTS TO ACCEPT SAID PAYMENTS, IN A FORM
ACCEPTABLE TO THE CITY ATTORNEY AND THE RISK MANAGEMENT
DIRECTOR; WITH SAID PAYMENTS TO BE MADE BY THE AHCA, DOE/VR AND
THE SSA ON A REIMBURSEMENT BASIS, SUBJECT TO STATE
APPROPRIATIONS, FOR SERVICES AND PROGRAMS FOR THE
DEVELOPMENTALLY DISABLED WHICH ARE ADMINISTERED BY THE CITY OF
MIAMI'S DEPARTMENT OF PARKS AND RECREATION THROUGH ITS
PROGRAMS FOR PERSONS WITH DISABILITIES DIVISION, DURING THE
PERIOD OF JULY 1, 2008 THROUGH JULY 1, 2011.
WHEREAS, the City of Miami's ("City") Department of Parks and Recreation's Program for
Persons with Disabilities Division provides some of itr services and programs for the developmentally
disabled through payments, on a reimbursement basis, from the following agencies: for Medicaid
waiver program services the State of Florida Agency for Health Care Administration ("AHCA"), for
employment and supported employment services, the State of Florida Department of Education,
Vocational Rehabilitation ("DOE/VR"), and the Social Security Administration ("SSA") as an
Employment Network ("EN") Contractor for the Ticket to Work and Self Sufficiency Program; and
WHEREAS, the AHCA, the DOE/VR and the SSA is providing to the City funds, on a
reimbursement basis, in the estimated amount of eight hundred thousand dollars ($800,000) for a

three-year period, commencing July 1, 2008 through July 1, 2011, subject to state appropriations, in
payment for the previously specified services and programs being provided and to be provided to
developmentally disabled individuals, through the City's Department of Parks and Recreation's
Programs for Persons with Disabilities; and
WHEREAS, in order to provide said services, the City must submit new applications, renew
existing applications and enter into agreements with the AHCA, the DOE/VR and the SSA to maintain
the eligibility status required by said agencies to service and provide programs to the developmentally
disabled;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,
FLORIDA:
Section 1. The recitals and finding contained in the Preamble to this Resolution are adopted
by referenced and incorporated as if fully set forth in this Section.
Section 2. The following Special Revenue project is amended as described herein:
FUND TITLE: Program for the Developmentally Disabled 2008-2011
RESOURCES: State of Florida Agency for Health Care Administration Medicaid
Program Development through the State of Florida Agency for Persons with
Disabilities for Medicaid waiver services $600,582
The State of Florida, Department of Education, Division of Vocational
Rehabilitation for employment and supported employment
services $150,000
The Social Security Administration for Employment and Supported
Employment as and Employment Network Contractor for the ticket
to work and self sufficiency program $49,418
APPROPRIATIONS: Program for the Developmentally Disabled 2008-2011 $800,000
Section 3. The City Manager is authorized {1} to accept anticipated payment for services in
the estimated total amount of eight hundred thousand dollars ($800,000) from the AHCA, for
Medicaid waiver program services; the DOE/VR for employment and supported employment services;
the SSA for employment and supported employment services as an EN Contractor for the Ticket to
Work and Self Sufficiency Program to fund on a reimbursement basis, subject to state appropriations,
services and programs for the developmentally disabled which are administered by the City's
Department of Parks and Recreation through its Programs for Persons with Disabilities Division,
during the period of July 1, 2008 through July 1, 2011.
Section 4. The City Manager and all other necessary City officials are authorized{1 } to apply
for, renew, negotiate and execute Medicaid Waiver Services Agreement(s), in substantially the
attached form(s), and all other necessary documents to accept said payments, in a form acceptable
to the City Attorney and the Risk Management Director.
Section 5. This Resolution shall become effective immediately upon its adoption and
signature of the Mayor.{2}

{1} The herein authorization is further subject to compliance with all requirements that may be impose
the City Attorney, including, but not limited to, those prescribed by applicable City Charter and Code
provisions
{2} If the Mayor does not sign this Resolution, it shall become effective at the end of ten calendar day:
the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission
