The application for the Emergency Medical Services (EMS)
County Grant #C8013 for FY 2008-09 has been approved by the
Board of County Commissioners and the Florida Department of
Health -Bureau of EMS. A total new grant revenue amount of
$82,907.00 has been approved for your 2008-09 work plan
projects. The new revenue allocation was based on the
revenue distribution schedule revised on 10-15-2008 (copy
enclosed).
We are planning again this year to disburse the new funds
designated for your grant work plan projects directly to
your municipality- In order to expedite the process, we are
enclosing a Letter of Understanding/Agreement that provides
the basis for the disbursement. Please obtain the
appropriate signatures and return the completed form to:

Copies of the required JUNE 2002 FLORIDA EMS COUNTY GRANT
PROGRAM, State EMS Award Letter, CSFA number sheet,

Carryover Schedule, County Resolution, Grant Application and
City's Expenditure and Work Plans (Revised on 10-16-08) are
enclosed for your information and future reference.
