As Chairperson of the Evaluation Committee ("Committee") for the above services for the City of Miami, it is my
responsibility to offer the findings and recommendation of the Conunittee.
The City issued RFP 105049 for the provision of Drug Screening Services, and five (5) Proposals were received,
one (1) Proposal was deemed non-responsive. The EvahGa.tion Coriurlittee ("Committee"), appointed by the City
Manager, was comprised of the following indi ;iduals:
I. Marjorie H. Adler, Human Resources Director, City of Coral Gables.
2. Rebecca Jones, Human Resources Director, City of North Miami.
3. Craig Dunn, Assistant Fire Chief, City of Miami.
The Evaluation/Selection Committee deemed the proposals submitted by Mercy Medical Development d/b/a Mercy
Outpatient Center, and Work Injury Solutions of Dade County Inc., d/b/a Health Care Center of Miami, as the
highest ranked firms, in the manner cited below, and recommends that the City negotiates with both vendors for the
respective parts, Part I through Part V. The Evaluation/S elect] on Committee further recommends that should
negotiations fail with the top ranked firms, that negotiations be made with the second highest ranked firms Should
negotiations then fail with the second highest ranked firms, then the Evaluation/Selection Committee recommends
that the Purchasing Department issue a new request for proposal.

Upon successful contract negotiations, the recommendation from the City Manager to the City Commission seeking
permission to authorize and execute the professional services agreement will be presented at the next available
meeting.
