I II"B NO. 105049 October 2, 2008 Request for Proposals (RI=P) for Drug Screening Services TO: ALL PROSPECTIVE PROPOSERS: The following changes, additions, clarifications, and deletions amend the Proposals Documents of the above captioned RFP, and shall become an integral part of the Contract Documents. Please note the contents herein and reflect same on the documents you have on hand. The following is the City's answers to questions received from prospective proposers: Q1: Who is the current vendor(s)? Al: Mercy Medical Development d/b/a Mercy Outpatient Center and Preventive Medicine Testing Centers Inc. d/b/a Global MRO. Q2: What are the prices for services under the current Drug Screening Services contract? A2: See Contract Award Sheet attached. Q3: Can a copy of the existing contract(s) be obtained? A3: Yes. A copy of the contracts made with Mercy Medical Development and Preventive Medicine Centers have been attached Q4: Now many collection sites are required for the contract? A4: One collection site for each part of the contract. Q5: What are the addresses of the current collection sites? A5: Mercy Medical Development: 3641 S. Miami Avenue. Miami, FL 33131 Preventive Medicine Testing Centers. 5201 Ravenswood Road, Suite 121, Ft. Lauderdale, FL 33312 (self- contained mobile unit) Q6: Are expert witnesses required to appear in court? A6: No, only personnel involved in the administration of drug screens, if required. Q7: Are the quantities in each line item estimated annual quantities? A7: Yes. Q8: Must the fees be separated by the line items in the proposal package? A8: Yes. Fees must be submitted along with your response to the Request for Proposal. Q9: Can a prospective proposer submit a bundled price for each part to include collection, testing, and MRO services? A9: No. A prospective proposer must submit their fee proposal following the requirements stipulated under Section 4.1, subsection 7, "Fee Proposal". Q10: Pages 12 and 15 of the RFP document seem to have mistakes. There is no Drug Testing Facility involved in Evidential Breath Testing just as there is no MRO involved. Can you confirm if this is true? A10: This is true, breath alcohol testing falls under Collection Services only. Q11: Section 1.11 (Bid Bond/Bid Security) states that "a cashier's or certified check, or Bid Bond signed by a recognized surety company that is licensed to do business in the State of Florida, payable to the City of Miami, for the amount bid is required from all bidders/proposers, if so indicated under the Special Conditions". Upon review of the Special Conditions there is no mention of a Bid Bond requirement. Please confirm that a bid bond is a requirement of this RFP (105049 - Request for Proposals for Drug Screening Services) and if so provide the dollar amount needed. A11: A bid bond is not required for this proposal. Q12: Under Section 2.6, it mentions that the proposer shall hold a current SAMSHA and/or AHCA license. Are you only accepting bids from Laboratories themselves? The way this reads it seems that all Third Party Administrators are being excluded from bidding on this contract. Al2: SAMSHA and/or ARCA license is only required for the drug testing facility. Third Party Administrators may bid on this contract as long as the drug testing facility performing the testing holds a current SAMSHA and/or AHCA license. Q13: On Pg 68 of the RFP, the cut off for Methaqualone is 750. Is that a typo error (normally it is 150)? A13: Under Part IV Services, "Initial Test Level (ng/ml)", the cut-off level should be 300. Q14: The cutoffs for Methaqualone in Part I and Part IV are different. Is this correct? A14: Yes, this is correct. Q15: In each of the five (5) groups, in the Drug Testing Facility section, it states: "State of Florida certificate must be for forensic toxicology". Does this mean that in order to be eligible for this bid, you must have a State of Florida Toxicology License and SAMHSA certification. Or is it one or the other? A15: For Parts 1, ii, IV, and V you must have either or both SAMSHA or AHCA certification. For Part III the City will only accept SAMSHA certification. For all parts, you must also have a State license where the drug testing facility is located if required. For example, if you are a drug testing facility located in the State of Florida you must have a State of Florida toxicology license. Q16: Section 4.1, subsection 6b) "Proposer's Ability and Capability to Provide Required Services" states: "Provide copy of most current audited Financial Statement, or current unaudited Financial Statement, for the past year." Can a prospective proposer submit, instead of their Financial Statement a Balance Statement? A16: No. Prospective proposers must submit along with their proposal their Financial Statement.
