Subject: Amendment to First Renewal Agreement Between City of Miami and Public Health Trust of
Miami -Dade County, Florida
Purpose of Item:
A Resolution amending the aforementioned, pursuant to Resolution No. 05-0658, dated November 17,
2005, for a term of three (3) years, with an option to extend for two (2) additional two (2) year
periods. On October 1, 2008 on mutual agreement and City Manager's approval, the agreement was
extended for a two (2) year period through September 30, 2010. It is now necessary to include
additional medical direction duties and responsibilities; hence, increasing the amount of compensation
agreement to $242,608.00 from $226,192.00, an increase of $16,416.00, for the first year 2008-2009,
and a cost increase equal to the Consumer Price Index but no more than 5% for 2009-2010.

Background Information: (continued)
On November 17, 2005, pursuant to Resolution 05-0658, the City Commission authorized the City
Manager to execute an Agreement with The Public Health Trust of Miami -Dade County d/b/a Jackson
Health Systems ("The Trust") for Medical Direction Services at a first —year cost not to exceed
$203,588, a second -year cost not to exceed $209,420 and a third -year rate cost not to exceed $215,427
with an option to renew for two (2) additional two-year periods, with incremental annual increases
after the initial three (3) year period limited to the recent year's annual increase of the Consumer Price
Index, but no more that 5% per year.
On November, 2008, City initiated an emergency medical services training program in Miami -Dade
County public schools and certain medical direction services are required by state law and are not part
of the First Renewal Agreement.
On October 1, 2008, on mutual agreement and City Manager's approval, the agreement was extended
for a two (2) year period effective retroactively from October 1, 2008 to September 30, 2010. Based
on the additional duties being required of the Medical Director there is a need of an increase in the
amount of compensation agreement to $242,608.00 from $226,192.00, an increase of $16,416.00 for
the first year, 2008-2009, and a cost increase equal to the Consumer Price Index but no more than 5%
for the second year, 2009-2010, allocating funds from Account No. 00001.184010.5310000.0.0.
