Medical Direction Services for EMS
7. COSTIFEE PROPOSAL
Cost/Fee Proposal
EMS medical direction services will be provided by the Emergency Care Center of Jackson
Memorial Hospital In consideration of the following: (Detail outlined in Figure 1.)

All costs include the costs of staffing the Medical Director and lower tier positions at the levels
outlined, all administrative services, all facilities and materials necessary for the duties
outlined, and all program supervision costs.
Optional Associate Medical Director:
Not Included In the above proposal or in the detailed budget In Figure 1 are costs
associated with an Associate Medical Director. Costs are outlined below as per
request.

Medical Direction Services for EMS
The above mentioned fee -proposal Includes the foilowlna:
> Medical Director's service to the City of Miami and the Village of Key Biscayne Fire
Rescue Departments for EMS -related activities.
A Second-f/sr and 7'hlyd-tler physicians, including salaries and mandatory institutional fees
and employer -paid fringe benefits, and administrative support (as described in section 7).
➢ Working capital required for meeting all out-of-pocket costs associated with rendering the
services.
Calculated from the 1997-8 Composite Fringe Benefit Rate (CFBR) of the institution issuing the paycheck to the Individual
physician. The CFBR is 20% for JMH hires. For the University Of Miami School Of Medicine, the CFBR is 22.5°/x; the Dean's
Office Tax on all professional Income is 2,6%. Salary for the Medical Director is paid by the PHT to the University under the
Annual Operating Agreement between the Institutions. UM then issues d single paycheck to the. physician that includes her
compensation for services rendered via the UM Medical Group Practice.
B. MINORITYIWOMEN PARTICIPATION
The Public Health Trust of Miami -Dade County 1 Jackson Health System as a public health
resource is compliant with EEOC and Afflrmative Action and is proposing a female Medical
Director, Kathleen Schrank, MD.

The primary office location of the proposer is:
The Public Health Trust/Jackson Memorial Hospital is located within the city limits of Miami at
1611 NW 12 Avenue, Miami, Florida 33136-1099.
The primary office location of the proposed Medical Director, Dr. Kathleen Schrank, and the
location from which she will be based to perform the work is in the Emergency Care Center of
--the-University-of-GVtiarniC,lackson Memorial Medical Center. The physical address is consistent
with the one given above. ----

Not applicable. There are no trade secrets in the Proposal.
