Institutional Control Proposal
Dear Mr. Fenton:
Bureau Veritas North America, Inc. (Bureau Veritas) proposes to provide the services identified
below for the project entitled Grapeland Heights Park.
I. GENERAL
Bureau Veritas understands that the Pollution Remediation Section of the Department of
Environmental Resources Management (DERM) requires the preparation of the supporting
documents that will be attached with the Institutional Control for the above -referenced facility.
ll. SCOPE OF WORK
Based on Bureau Veritas's understanding of the project requirements and DERM's approval
that the groundwater monitoring is no longer required, Bureau Veritas proposes the following:
A. Task 1 -Institutional Control
Bureau Veritas will prepare the following documents that are required attachments to record
the Institutional Control as per DERM's Guidance Document 7F:
1. A copy of the proposed covenant, in the standard form approved by Miami -Dade County
Board of County Commissioners (be advised that the language and format of the
covenant cannot be modified from the approved form).
2. Attachment A- Summary of the contamination assessment report including summary
tables of analytical results, site maps (indicating the north direction, drawn to scale, and
including geographical representation of the scale) with property boundaries, institutional
control boundaries, sample locations, and sample results.
3. Attachment B -Summary of the DERM approved engineering control plan, including the
contaminant of concern plume map, drawn to scale, with the engineering control
superimposed and a detailed description of the construction, maintenance, and as
appropriate, monitoring of the engineering control.

B. Task 2 -Documents not prepared for the Institutional Control
Bureau Veritas will not prepare the following documents that are required attachments for
the Institutional Control
1. Exhibit A -A copy of the legal description of the property
2. A copy of the title search (for ownership). NOTE: The owner name on the proposed
covenant must match the name of the current property owner at the time the covenant is
filed in the public records.
3. Tax lien information
4. List of easements (copies of recorded easements and diagram of locations)
5. Mortgage holder (completed and signed subordination of mortgage for each financial
institution or lender of existing mortgages)
6. Leases/tenants (copy of leases/subleases or assigned leases)
7. Any Uniform Commercial Code (UCC) liens (copies of and releases of any liens).
C. Task 3 -Prepare the Institutional Control for final signature
Bureau Veritas will combine all required documentation for final signature. This includes the
work performed under this proposal and the items listed in Task 2, above, which will be
provided by the City of Miami.
The City of Miami is responsible for recording the executed Institutional Control with the
Clerk of the Courts as public records of Miami -Dade County. The City of Miami is
responsible for providing a copy of the covenant stamped with the book and page number in
the upper right hand corner indicating where and when the covenant was recorded to DERM
prior to issuance of the Risk -Based Corrective Action site closure permit and subsequent No
Further Action with Conditions letter.
III. SUBCONSULTANTS
The below listed Sub -Consultants will assist in the performance of the Work.

Immediately upon receipt of authorization to proceed, the outlined scope of services will be initiated. The
referenced quarterly reports will be submitted to the City of Miami within three business days of receipt of
laboratory data, which will be requested utilizing standard turnaround times. An updated schedule, indicating
actual delivery dates, based on the above durations, will be provided to the City upon receipt of the NTP.
V. COMPENSATION
Consultant shall perform the Work detailed in this Proposal for a Total fee of $16,272.23. The
City shall not be liable for any fee, cost, expense or reimbursable expense or other
compensation beyond this amount. Said fee includes an allowance for Reimbursable Expenses
required in connection with the Work, which shall not exceed $100.00. Said Reimbursable
Expenses shall be used in accordance with the Agreement Provisions and shall conform to the
limitations of Florida Statutes § 112.061.

VI. ADDITIONAL SERVICES
The City may establish an allowance for additional services requested by the City and for
unforeseen circumstances, which shall be utilized at the sole discretion of the City.
