Monitoring Only Plan Proposal
Dear Mr. Fenton:
Bureau Veritas North America, Inc. (Bureau Veritas) proposes to provide the services identified
below for the project entitled Grapeland Heights Park.
I. GENERAL
Pursuant to your request, Bureau Veritas understands that the Pollution Remediation
Section of the Department of Environmental Resources Management (DERM) requires
Groundwater Monitoring at select monitoring wells for a minimum of one-year for the above -
referenced facility. On November 15, 2007, DERM approved quarterly groundwater
monitoring for arsenic from monitoring wells MW -3, MW -9, MW -29, MW -30, MW -31, and
MW -32 (see attached Figure 1). Bureau Veritas field verified that only MW -3 still remained
on site. However, the integrity of this well needs to be determined. There are two wells on-
site (MW -8 and an unknown well) which Bureau Veritas may be able to substitute for MW -9
and MW -29. However, the integrity of these wells must also be determined. Bureau Veritas
will discuss this option with DERM. Once DERM has decided about using the on-site wells
and Bureau Veritas has verified the integrity of the on-site wells, Bureau Veritas may need
to reinstall a total of six monitoring wells. If the monitoring wells can not be placed in the
same location(s), then Bureau Veritas will submit to DERM a site map which proposes the
new location(s) and install the new well(s) upon DERM approval.
II. SCOPE OF WORK
Based on Bureau Veritas's understanding of the project requirements, Bureau Veritas
proposes the following:
A. Task 1 -Quarterly Groundwater Monitoring
1. Determine the integrity of MW -3, MW -9, MW -29, MW -30, MW -31, and MW -32.

2. Discuss with DERM the option to utilize MW -9 and the unknown monitoring well
as a sampling location provided the integrity of the wells prove to be viable.
3. Based on the viability and integrity of the wells, install up to 6 shallow
monitoring wells as approved by DERM.
4. Install six monitoring wells with a mobilization cost of $375/day, which will take
2 days. The cost per monitoring well installation is $475. Bureau Veritas will
only charge the City of Miami based on the actual number of monitoring wells
that have been reinstalled.
5. Sample each monitoring well quarterly for arsenic via the EPA Method 200.7.
6. Place the purge water in one fifty-five gallon drum for each quarterly sampling
event and disposed of properly.
B. Task 2 -Reporting
1. Prepare the report for each sampling event for DERM review and approval.
2. Each report will include the sample results, tables, figures, and historical data.

Immediately upon receipt of authorization to proceed, the outlined scope of services will be initiated. The
referenced quarterly reports will be submitted to the City of Miami within three business days of receipt of
laboratory data, which will be requested utilizing standard turnaround times. An updated schedule, indicating
actual delivery dates, based on the above durations, will be provided to the City upon receipt of the NTP.
V. COMPENSATION
Consultant shall perform the Work detailed in this Proposal for a Total fee of $18,615.77. The
City shall not be liable for any fee, cost, expense or reimbursable expense or other
compensation beyond this amount. Said fee includes an allowance for Reimbursable Expenses
required in connection with the Work, which shall not exceed $100.00. Said Reimbursable

Expenses shall be used in accordance with the Agreement Provisions and shall conform to the
limitations of Florida Statutes § 112.061.

VI. ADDITIONAL SERVICES
The City may establish an allowance for additional services requested by the City and for
unforeseen circumstances, which shall be utilized at the sole discretion of the City.
