A RESOLUTION AUTHORIZING THE CITY MANAGER
TO EXECUTE THE ATTACHED AGREEMENT WITH
MERCY HOSPITAL, INC. FOR ITS PHYSICAL
EXAMINATIONS OF CITY OF MIAMI FIREFIGHTERS
IN ACCORDANCE WITH THE TERMS AND CONDITIONS
SET FORTH IN THE SAID AGREEMENT WITH FUNDS
THEREFOR ALLOCATED FROU THE FIRE DEPARTMENT
BUDGETED FUNDS TO COVER THE COST OF SAID
EXAMINATIONS.
BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:
Section 1. The City Manager is hereby authorized
to execute the attached agremeent with Mercy Hospital, Inc.
for its physical examinations of City of Miami Firefighters in
accordance with the terms and conditions set forth therein, with
funds hereby allocated from Fire Department Budged Funds to cover
the cost of said examinations.

AJ::, Request for Resolution
It is requested that a Resolution be placed on the July 13, 1978,
Commission Agenda authorizing the City Manager to enter into a
contract with Mercy Hospital to provide physical examinations for
Fire Fighters.
The main focus of these physicals will be a cardiac stress test.
Mercy Hospital was chosen as the facility to do the examinations
after careful evaluation of other area facilities. Mercy Hospital
will be providing stress electrocardiograms, resting electrocardio-
grams, chest x-ray with interpretation, urinalysis, blood chemistry-
C.B.C., S.M.A.C. 20 profile. The stress test will be monitored
and interpreted by a cardiologist.
Recommendations will be made to individual Fire Fighters concerning
weight reduction, smoking withdrawal, guidelines for exercising,
further follow-up and any other pertinent suggestions involving
patients. Results shall be forwarded to City Medical to become
part of the individual's medical records.

AGREEMENT
THIS AGREEMENT made and entered into this day
of , 1978, by and between the city of Miami, a
municipal corporation of the State of Florida, (hereinafter referred
to as the "CITY"), and Mercy Hospital, Inc., a Florida corporation,
(hereinafter referred to as "MERCY").
WITNESSETH
WHEREAS, the CITY is desirous of having its Department of Fire
Personnel undergo physical examinations, to include a cardiac stress
test; and
WHEREAS, Mercy Hospital was selected after an evaluation of
area hospitals, as being capable of performing the examinations
desired;
NOW, THEREFORE, in consideration of the mutual covenants and
agreements hereinafter set forth, the parties hereto covenant and
agree as follows:
CITY AGREES:
1. To provide compensation for MERCY'S services at the rate of
ONE HUNDRED AND TEN DOLLARS ($110.00) per examinee.
MERCY AGREES:
1. To provide physical examinations to City of Miami Fire
Department Personnel, which shall include:
(a) Cardiac stress test
(b) Complete history
(c) Blood chemistry - CBC, Urinalysis
(d) SMA 20 profile
(e) Chest x-ray
2. To make recommendations to the individual examinees that
will help them to successfully deal with their own particular stress
in a manner that will be beneficial to their jobs and personal health.
This shall include, but is not limited to, guidelines for exercie,
weight reduction, withdrawal from smoking,

8, To forward results of physicals to the City Medical Depart-
meat to become part of examinee*s medical records.
BOTH PARTIES AGREE:
1. Each examinee must execute a standard out -patient consent
form provided by MERCY prior to any examination,
2, That the term of this agreement shall be from July 17, 1978,
to September.30, 1978.
3. That this agreement may be terminated upon the mutual consent
of the parties as evidenced by a properly executed writing.
IN WITNESS WHEREOF, the parties hereto have caused this instru-
ment to be executed in their names by the persons thereto legally
qualified.
