LOWERY & ASSOCIATES, FOR PROVIDING ACCIDENTAL DEATH INSURANCE IN THE AMOUNT OF TWENTY THOUSAND ($20,000) DOLLARS FOR CITY OF MIAMI EMPLOYEES FOR THE PERIOD FROM OCTOBER 1, 1978 THRU SEPTEMBER 30, 1979, AT AN ESTIMATED ANNUAL PREMIUM OF TWENTY SEVEN THOUSAND ($27,000.00) DOLLARS, WHICH IS BASED UPON FORTY-SIX (46) CENTS PER EMPLOYEE PER MONTH TIMES THE NUMBER OF EMPLOYEES WORKING FOR THE CITY OF MIAMI, FLORIDA ON OCTOBER 1, 1978, WITIH AN OPTION TO RENEW FOR AN ADDITIONAL TWO (2) YEAR PERIOD, AT THE SAME PRICE, BY MUTUAL AGREEMENT. WHEREAS, pursuant to Public Notice, sealed Bids were received on August 29, 1978, providing for Accidental Death insurance in the amount of Twenty Thousand ($20,000.00) Dollars per employee for the period from October 1, 1978 thru September 30, 1979, with an option to renew by mutual consent of both parties for an additional two (2) year period; and WHEREAS, invitations were mailed to seventy-four (74) insurance agents in Dade County who represent insurance companies who might write this type of coverage; and WHEREAS, ten (10) Bids were received at the office of the City Clerk of the City of Miami, Florida, in response to the abovementioned invitations to Bid; and WHEREAS, the City Manager recommends that the award be made to BANKER'S LIFE & CASUALTY COMPANY, which is the most advantageous bid to the City, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF MIAMI, FLORIDA: "DOCUr,1Et ITEM NO. 1�1 t) LA Section 1. The Bid received from BANKER'S LIFE & CASUALTY COMPANY is accepted. This policy provides Twenty Thousand ($20,000.00) Dollars per employee for Accidental Death benefits. September 11, 1978 Acceptance of Bid Accidental Death insurance -)CL0ZU1Rr5:Copy of Resolution and Chatt Pursuant to Public Notice, sealed Bids were received August 29, 1978 to provide Accidental Death insurance in the amount of Twenty Thousand ($20,000.00) Dollars for each City of Miami employee for the period from October l, 1978 tbro September 30, 1979, with an option to renew for an additional two (2) year period by mutual agreement at the same premium. In addition to the invitations to Bid being published in five (5) newspapers, ^ Specifications were mailed to all insurance agencies in Dade County, some seventy-four (74) in number, who represent insurance companies who might write this type of coverage. ` N� N� @� N� m� Ten (lO) Bids were received, which is an excellent response considering that Municipalities are not a favored risk by insurance companies. This Accidental Death insurance is a requirement under Florida Statutes 112,19 and 112'191 for Policemen and Firemen' In order to reduce the premium, all employees of the City of Miami have been included in this bene- fit. It is recommended that the award be made to Banker's Life a Casualty Company through its local agent Joseph E. Lowery a Associates at a cost of forty-six (460 cents per employee per month. The estimated annual premium is approxi- mately Twenty Seven Thousand <$27,000.00> Dollars, said premium being predicated on the number of employees working for the City as of October l, 1978. The policy period is October l, 1978 tbru September 30, 1979, with an option to renew for an additional period of two (2) years with the mutual consent of both parties.
