I. The Provider agrees:
A, Services to be Provided.
To provide adult non work oriented developmental training
(self -care, daily living skills r'training ,":community awareness
fine and gross motor -development, human growth and development,
recreation, pre -work readiness skills, functional academics, etc.)

E. - RetentiOn of Records.
To retain retain allfiriandial records, _supporting documents, statistical
records, and any other-doc.urnents 'pertinent to this agreement for a
period of five (5) Years,.after starting date of the applicable
retention period, or if audit findings have not been resolved at the
end of the five year period, the records shall be retained until
resolution of the audit findings. Federal auditors and any persons
duly authorized by the'DIMS/ItP shall have full access to, and the right
. to examine any Of. the said materials during said period.

Indemnification.

II. The DIIRS/RP agrees:

A. 14etItod of Payment for Servi.ces.
1. That payment will be made on the basis of tnonthly invoices
submitted to the District DliRS/RP in quintuplicate on forms
furnished by the DHRS/RP. Reports are due within five working
days after the end of the month.

2. That the DIIRS/RP shall not reimburse the Provider .for a cost
greater than the cost for an ineligible client. This cost will
be determined through an 'IRS audit.
3. That subject to the availability of funds, the DIIRS/RP will

(a) This service agreement shall begin on

Developmental Training: Enrollment and Attendance Requirements.``

• For the, purpose. of payment and program development, enrollmen
is�';related, to.continuin attendance.'No_days of unexcused absence
will be reimbursable. Absence due to any cause other then illness,
such as vacation. -justified,, twill be allowed as an excused 'absence
up 'to a 'maximum of Live�,,,program days per' month. -
Should an individual becomcill, absence will be :excused up to
five days. Documentation of. family.. contact is required for
ra cured bsences. If a i illness; is extended, and there is
reasonable assurance ,thereturn to. the program,
