D. To provide all available placement services of PROVIDER for utilization
+' by CETA participants enrolled in training programs.
E. To provide the Premises of Datamerica Institute at 1011 S.W. 27th Avenue,
Miami# Florida, as the location of classrooms £or the courses of instruction.
• F. To.provide all necessary instructional materials for instruction of
participants to include textbooks, notebooks, papers, pens, and pencils.
G. To provide all training by.qualified instructors with minimum re-
quirements of a Bachelor's Degree from an accredited -college or university and
with significant"teaching experience in the field of their subject matter.
H. To provide an initial assessment of each participant'.at the`time
of enrollment to'evaluate the present educational level of the student in
the respective area of study.
I. To provide periodic attendance and progress reports. as, outlined
in Article <VII. .'
:ARTICLEIIe DELIVERY OF SERVICES
Services shall be provided to ninety one (91) participants of the City'`
of Miami CETA program: "Class hour"' 'for purpose of this contract shall be
defined as aperiod of instruction consisting of'at least fifty minutes
instructional training. Each participant shall'`attend.classes'for six class
hours per day, for two: consecutive%days each week. iThere will be no.instruction
on public holidays as established by; the City. Manager,7 City of 'Miami..
A total of sixty-four participants shall study a program of English as a
Second Language, and twenty-seven participants shall study a program of English
as a Second Language along with`Typing. Thirty-two. participants for English as
a Second Language and twelve participantsfor English as a Second Language along
with Typing shall meet on Mondays and'Tuesdays'of each week and theremaining
thirty-two' participants for English,as a Second Language and fifteen participants
for English as a Second Language along with Typing shall meet .on Wednesdays and
Thursdays.of each week.'
`Instruction will begin no later that ten days after the signing of the
contract by the City, Manager and notice is given to' -the PROVIDER and'.will
thereafter run for twenty consecutive, weeks.

Costs for instructional materials shall bepayabletwenty percent,,(20%)
from CETA Title II D t'unds and eighty percent (80%) from CETA Title VS Funds.
These percentages represent the ratio of participants from'Titles`II D
'. and Title VI respectively.
' ARTICLE V: METHOD OF PAYMENT
ti Payment. shall be";made in accordance -with those procedures as outlined
"below -
PROVIDER for services rendered on'a
monthly basis in'connection with this, Agreement and billing should be made
at the end of each month: All 'invoices shall include the following infor-
matione
1) Description of services'provided.l
2) Date and time services were provided.
3)' Total hours worked ,for period of invoices.
4) Names of the participants receiving services.,
5) Additional information as requested by'Division.
Separate invoices shall be 'provided to the DIVISION on a•month ly
basis for class hour".training and for cost"of instructional materials.
If an invoice is questioned•by the DIVISION, the DIVISION s}iallnotify
PROVIDER in writing within seven days of recelp,t of the invoice and 'mutual
agreement shall be reached before payment'is made.
Unless the invoice is`,questioned by DIVISION asprovded for above;-'
payment shall be made in full by the CITY within thirty (30) days after,
receipt (if invoice from PROVIDER.
ARTICLE VI: NOTICES
It. IS understood and agreed between the both parties, hereto that
all notices -which may arise in. connection ;with this 'Agreement,shall '.
be considered sufficient when made in writing and mailed or delivered-'
in person, to the appropriate address of the respective party:`,
