This is to request time on the September 15, City Commission
Agenda for a personal appearance for Jim Hirschman., M.D. and
Sanford Dernis Esq. Dr. Hirschman and Mr. Dernis are officers
of the South Florida Emergency Medical Services Coordinating
Council, Inc.
The Council has been approved by the State E.M.S. unit and the
Federal E.M.S. program as the organization to develop a regional
E.M.S. System (Dade and Monroe Counties). Federal Authorities
have awarded the Council $400,000 grant conditioned on raising
an additional amount of $50,000 local fundinq. Dade County has
provided $31,000, Monroe County has committeed $5,000. There
have also been contributions from the Dade County Medical
Association and individuals. The purpose of these personal
appearances is to request the support of the City Commission,
both programatic and financially in the amount of $5,000, by
passage of a Resolution similar to the suggested enclosure.
The City of Miami Fire Rescue has been a leader in the develop-
ment of Paramedics, Swat-meds.and the Cooperative utilization
of these life savers with Emergency Departments.
Now is the time to take a further step and with formal agreements
establish a regional system.

Rescue Units. Chief Larry Kilburn has been a member of the
Council since its inception. Fire Chief Herman Brice was a

RESOLUTION
Resolution approving in principle the concepts of the
South Florida Emergency Medical Services Coordinating
Council, Inc., and authorizing the funding of $5,000
for assisting in the financing process.
WHEREAS, the South Florida Emergency Medical Services Coordinating
Council, Inc., has offered a means of planning, evaluating and coordinating
the various elements and components of the emergency medical services in
the geographic area of Dade County; and
WHEREAS, while we have elements and components of the emergency
medical services system of high quality, it is appropriate to study and
evaluate our emergency medical services in order to seek more cost effective
means to deliver services and to plan for better ways of providing quality
and cost effective services; and
WHEAREAS, it is essential to the effectiveness of the goals of the
South Florida Emergency Medical Services Coordinating Council, Inc., and
the emergency medical services provided by Metropolitan Dade County that the
process of study, evaluation and planning be a cooperative one; and
WHEREAS, the South Florida Emergency Medical Services Coordinating
Council, Inc., has applied for and been awarded $400,000 of federal funds
to finance the beginning of the process and thereby provide equipment, train-
ing, education, data gathering and management information; and
WHEREAS, South Florida Emergency Medical Services Coordinating Council,
Inc., has requested and received the endorsement of the Dade County Commission
yin -the amount of $31,000 and the Monroe County Commission in the amount of
$5,000 and has further requested that the City of Miami assist in the financing
of the process and encourage the granting of federal funds.
NOW, THEREFORE, RE IT RESOLVED BY THE COMMISSIONERS OF THE CITY OF
MIAMI, FLORIDA, that:

Section 1. The process of study, evaluating,.planning, education
and training as proposed by South Florida Emergency Medical Services
Coordinating Council, Inc., is hereby endorsed in principle.
Section 2. The City of Miami and or the City of Miami Fire
Department shall be involved in the process of study, evaluation, planning
and implementation.
Section 3. The funds requested, $5,000, shall be granted and
utilized with Dade, Monroe Counties and other Public and private funds
approved in the grant application to the Public Health Service, to establish
a regional E.M.S. System as under Public Law 95-626, for the grant period
beginning July 1, 1980.

Dear Mr. Crassie:
I am writing in behalf of the South Florida Emergency Medical Services
Coordinating Council. This Council was formed for the purpose of
coordinating EMS in Dade and Monroe Counties into a regional system.
There is a seven year history that leads up to the present status of
the Council. It began by various committees and groups working together
with County officials and Itealth System Agency staff to reach the point
of developing an organization with the mandate of coordinating emergency
medical care. Both Dade County officials and Monroe County officials
and Fire/Rescue chiefs of the municipalities have been of considerable
assistance it, developing the coordination approach. There are several
Fire/Resctic departments actively engaged in providing; us with information
and assisting in the decision malting process about the work plan and
community tasks that the Council will perform,
The EMS Council is applying to be designated as n 503 W Organization,
and hopes to he so designated by July 1, 1980, For this reason, we are
writing to request that the City of Miami participate in support of this
activity in the amount of $5gn.00 monthly ($6onn culnually) beg -inning;
Jelly 1, 19a0. Ilealth organizations ;Ind othuI- governmental units, as well
'is civic groups, various municipalities and 'lonroe County are being;
requested to support the Cuuncil's grant request with dollars and with
"in -kind" support. Total federal dollars requested the first year will
I>e approximately $600,000 and a good percentage of this; will be applied
to EMS in 1),ldQ County and its cities.
Although you are generally familiar with tilt! effort of the EMS Council,
I know that you will have gticstionc;, 1, along with others on the Council
Board, are taking time out to personally contact those governmental and
agency representatives from whom commitment is needed, one of us will
Call you within the next week to arrange an appointment.
In addition, should commission approval he required for this support beyond

The members of the S.F.E.M.S.C.C. are representatives from hospitals,
physician groups, emergency room nurses, fire/rescue units, administrators
and consumers who are interested in EMS. The Council's general purpose
is to soundly increase quantity and quality of emergency medical care in
the prehospital phase as well as in follow-up at the fixed health care
establishments at reasonable costs,
Although there have been outstanding developments in EMS in Miami, greater
Dade County and beginning now in Monroe County, there has not been a
coordinated regional approach which related to the State EMS program and
to the Federal EMS unit. Also, there has been sparse disaster coordination
of our rescue systems between the two counties.
To organize and develop this program, the EMS'Council is applying for a
federal grant as mentioned above un the EMS ACT P.L. 95-626 RMSS. This
region appears eligible for several million dollars if a competetive
application can be developed.
This letter is to inform you of the application for Federal assistance and
to request a letter of support and financial commitment to the program.
Expenditures for fire/rescue services by the City can be utilized as
in kind matching;, if you agree, of which there needs to be an aggregate
sum of 50%' of the total Federal request. In addition, there must be a
"hard cash" commitment from the local area to be available in equal
monthly or quarterly installments beginning July 1980.

The concept of an Emergency Medical Service Coordinating Council
has been the focal point of discussions for a number of years.
The Health System Agency 4HSA) has been an instrument to do
regional (Dade County.and Monroe County) planning in the area of
medical services to tFe community. As a part of this responsi-
bility, there is a task.for:ce.on Emergency Medical Service (EMS).
The authority of HSA does not extend to implementation of plans
and, therefore,the need for an implementing body.
The South Florida Emergency Coordinating Council is proposing
that it will be involved in the planning, evaluating and
coordinating of the various elements and components of the
emergency medical services of Dade County and Monroe County.
This planning, evaluating and coordinating process will include
not only fire rescue but also all other phases of EMS such as
ambulances, emergency rooms and hospitals.
Throu-bout the discussions on the need for such an organization,
the Fi.:e Service has been concerned with two major areas.
1. The lack of authority this Coordinating Council would have
to implement its plans.
2. The concern of where future funding will come from if and
whe• the Federal Grant terminates.
The Fire Service has been working with others in the community
to resolve this issue primarily because of our respect for
Dr. Jim Hirschman, who was one of the pioneers of our own City
of Miami Fire Rescue Service.
The Grant Application originally for approximately $900,000 for
two years, has been amended in the regional office at Atlanta to
approximately $400,000 for one year. The proposed budget for
these funds is attached.

It is important to understand that City of Miami support of this
organization should come from the point of view of being part of
a regional improvement of the delivery of Emergency Medical Service
rather than the City of Miami Fire Rescue being. the recipient,of
major benefits.

League of Miami, the Dade County Fire Chief Officers Association,
the Health Systems Agency of South Florida,, the South Florida
Hospital Association, the Dade -Monroe Lung Association, the Public
Health Trust and others.
The Florida Emergency Medical Services program has approved this
grant application and forwarded it' to Federal emergency medical
services authorities who will make recommendations concerning the
application sometime in the later part of April. The suggested
work plan for the first year is attached as is a suggested
resolution,
