Gary City Manager F N..LOsuRES FILE This memorandum responds to the City Commission's specific request for a procedure that will be applied to all organizations or individuals requesting fundinq through the agenda process. This proposed procedure incorporates the requirements of Resolution 81-421 (fundino guidelines for the Festival Advisory Committee) and information that is necessary for the City Commission to properly evaluate the request). (1) Requests for event or festival funding must be considered by the Metropolitan Dade County Tourist Development Council and City of Miami Festival Advisory Committee prior to consideration by the City Commission. (2) Copies of the organization's charter, by-laws and articles of incorporation (not applicable for individual applications). ,,(3) Request presented at least 45 days in advance of the event. (4) All applicants must have a valid non-profit status in good standing. (5) Objectives of the event must be clearly defined in the preliminary proposal and show evidence of the need for the program proposed as well as an evaluation of the benefits to the communities. (6) A detailed description of the proposed activity including the total number of individuals to be served by each activity, proposed schedule of activities and a listing of staff personnel — -' who will plan and supervise programs along with their qualifi- cations and experiences in similar programs. (7) A complete schedule of proposed events with indications of pro- jected audience attendance, sites, crowd control measures, central planning and audience flow charts. n (16 Mayor and Members of the City Commission -2- (8) Rain dates and alternate sites, if applicable. (9) A detailed line -item budqet for the proposed program. (10) Identification of sources of funding by organization, amount and percent of total. (11) Statement by a Certified Professional Accountant as to the organization's financial status, includinq assets, liabilities, revenues and expenditures for the most recent five years of existence. The form letter to be sent to all applicants incorporating the above re- quirements is attached. �A1W 11 De ar This will acknowledge receipt of your correspondence requesting scheduling before the City Commission for funding consideration. It is the policy of the City Commission for the City Manager to review all such requests and provide a recommendation based upon specific criteria. Therefore, the following information is required for consideration and must be received in a timely fashion.
