A RESOLUTION ACCEPTING THE PLAT ENTITLED
"SECOND ADDITION TO MEDICAL CENTER", A
SUBDIVISION IN THE CITY OF MIAMI; AND ACCEPT-
ING THE DEDICATIONS SHOWN ON SAID PLAT; AND
AUTHORIZING AND DIRECTING THE CITY MANAGER
AND CITY CLERK TO EXECUTE THE PLAT AND PRO-
VIDING FOR THE RECORDATION OF SAID PLAT IN
THE PUBLIC RECORDS OF DADE COUNTY, FLORIDA.
WHEREAS, the City Zoning Beard and the Department of Public
Works have recommended the acceptance of the plat:
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The plat entitled "SECOND ADDITION TO MEDICAL
CENTER", is a resubdivision of Tract "A", PROFESSIONAL ARTS CENTER
(81-3), a portion of Lot 14, FIRST ADDITION TO SUNNYBROOK (43 -5),
a portion of Tract 10, of CIVIC CENTER SECTION TWO (75-79), Tract
"C", FIRST ADDITION TO MEDICAL CENTER (113-69), of the Public Records
of Dade County, Florida, and that portion of N.W. 11 Avenue -Spring
Garden Road between the south right-of-way line of N.W. 14 Street
and the north right-of-way line of State Road #836, all lying in
Section 35, Township 53 South, Range 41 East, City of Miami, Dade
County, Florida which subject plat by reference is made a part hereof
as if fully incorporated herein and which plat, together with the
dedications shown thereon, and the dedications to the perpetual use
of the public of all existing and future planting, trees and shrubbery
on said property, is hereby accepted and confirmed by the City of
Miami, Florida.
Section 2. The City Manager and City Clerk are hereby authorized
and directed to execute the plat and cause the same to be recorded in
the Public Records of Dade County, Florida.

The Department of Public Works recommends
the approval of the plat by the City
Commission of Miami, Florida, and certifies
its correctness as to form.
The propose' record plat entitled "SECOND ADDITION TO MEDICAL CENTER",
is a replat of Tract "A", PROFESSIONAL ARTS CENTER (81-3), a portion
of Lot 14, FIRST ADDITION TO SUNNYBROOK (43 -5), a portion of Tract
10, of CIVIC CENTER SECTION TWO (75-79), Tract "C", FIRST ADDITION
TO MEDICAL CENTER (113-69)•, and that portion of N.W. 11 Avenue -Spring
Garden Road between the south right-of-way line of N.W. 14 Street and
the north right-of-way line of State Road #836. It will consist of
one tract containing 3.59t acres and is zoned R-C.
This plat is being .forward to your office for City Commission action.
Also attached are the following itemized papers necessary to present
the plat to the City Commission of Miami:

The accompanying plat entitled "SECOND ADDITION TO MEDICAL CENTER", was
prepared by Biscayne Engineering. It is in correct form for submission
to the City Commission and is forwarded with a recommendation that it
be approved.
PERTINENT INFORMATION REGARDING THE PLAT:
(1) The property platted is a replat of Tract "A",
PROFESSIONAL ARTS CENTER (81-3), a portion of Lot 14,
FIRST ADDITION TO SUNNYBROOK (43 -5), a portion of
Tract 10, of CIVIC CENTER SECTION TWO (75-79), Tract
"C", FIRST ADDITION TO MEDICAL CENTER (113-69), and
that portion of N.W. 11 Avenue -Spring Garden Road
between the south right-of-way line of N.W. 14 Street
and the north right-of-way line of State Road #836.
It will consist of one tract containing 3.59t acres
and is zoned R-C.
(2) The location of the streets and the widths conform
with the standards of the department of Public •Works
of the City of Miami, Florida.
(3) As certified to by Alfred B. Mohr, Registered Land
Surveyor, this plat complies with the plat filing
laws of the State of Florida.
(4) The City Zoning Board of Miami, Florida, after Public
Hearing, has recommended the closing of N.W. 11 Avenue -
Spring Garden Road, between the south right-of-way line
of N.W. 14 Street and the north right-of-way line of
State Road #836. This action was confirmed by City
Commission Resolution No. 80-572.
(5) By authority of Resolution No. 175-82, adopted 10-4-82,
the plat was recommended for acceptance by the City
Zoning Board of Miami, Florida.
(6) The attached Certificates of Title Examination dated
August 29, 1980, April 7, 1981 and May 9, 1983, signed
by Pedro A. Martin, Sandra K. Michel, and Marilyn D.
Greenblatt, Attorneys, indicates that the fee simple
title to the property platted is correctly vested in
Morton Halpern and Jacob Colsky, as Trustees, and the
plat has been correctly executed.
(7) The area platted is encumbered by a mortgage and.the
mortgage holders have executed the plat and joined in
its dedications.
(8) Since all the required subdivision improvements on this
property have been previously placed and accepted, this
- plat will be forwarded to the City Commission of Miami,
Florida, without the usual formality of providing Bond
and Contract.

( 9) The location of the property is shown colored
in red on the accompanying copy of a portion of
City of Miami, Florida, Zoning Sheet No. 24.
(10) The attached Resolution has been prepared for the
acceptance of the plat by the City Commission of

SPECIAL EXCEPTIONS
6. Mortgage affecting a portion of the property described
in Exhibit "A" in the original principal amount of
$230,000.00 given by Jacob Colsky and Morton Halpern,
individually and as trustees to the First National Bank
of Miami, dated April 15, 1966 and filed May 17, 1966,
under Clerk's File No. 66R-79907 which was amended by a
Consolidation Modification and Spreading Agreement dated
July 20, 1967, filed July 20, 1967, under Clerk's File
No. 67R-110373. This Mortgage was assigned by First National
Bank of Miami to Metropolitan Life Insurance Company by
instrument dated July 20, 1967, filed July 20, 1967, under
Clerk's File No. 67R-110374. Said Mortgage was thereafter
amended by an Amendment to the Consolidation, Modification
and Spreading Agreement dated May 31, 1968, recorded June
14, 1968, under Clerk's File No. 6811-101618.
7. Lots 18 and 19, Block 2, SUNNYBROOK, are subject to a mortgage
in the original principal amount of $54,000.00 given by Morton
Halpern and Jacob Colsky as Trustees to John V. Williams and
Garland A. Williams, his wife, dated October 13, 1972, filed
October 25, 1972 under Clerk's File No. 72R-239433.
8. All matters shown on the Plat of MEDICAL CENTER SUBDIVISION,
recorded in Plat Book 95, at Page 54 of the Public Records
of Dade County, Florida.
9. All matters shown on the Plat of SUNNYB ROOK, recorded in
Plat Book 43, at Page 5 of the Public Records of Dade County,
Florida.
10. All matters shown on the Plat of FIRST ADDITION TO MEDICAL
CENTER, recorded in Plat Book 113, at Page 69 of the Public
Records of Dade County, Florida.
