A RESOLUTION, BY A 4/5THS AFFIRtIA.TIVE VOTE OF THE MEMBERS OF THE CITY COMMISSION AFTER A DULY ADVERTISED PUBLIC HEARING, RiATIFYING THE CITY MANAGER'S FINDING OF SOLE SOURCE; WAIVING THE REQUIREMENTS FOR COMPETITIVE SEALED BIDS AND APPROVING THE PURCHASE OF MAINTENANCE SERVICES AND RELATED SUPPLIES FOR THE PHYSIO CONTROL LIFE SUPPORT EQUIPMENT FOR THE DEPARTMENT OF FIRE, RESCUE AND INSPECTION SERVICES ON A CONTRACT BASIS FOR ONE (1) YEAR RENEWABLE ANNUALLY FROM PHYSIO CONTROL CORPORATION AT A PROPOSED FIRST YEAR COST OF $24,000.00; ALLOCATING FUNDS THEREFOR FROM THE GENERAL } FUND AND TELEPHONE FRANCHISE BUDGETS AUTHORIZING THE CITY MANAGER TO INSTRUCT THE CHIEF PROCUREMENT OFFICER TO ISSUE A PURCHASE ORDER FOR THIS SERVICE, SUBJECT TO THE AVAILABILITY OF FUNDS. WHEREAS, proper and efficient functioning of the existing Life Support Equipment used by each Rescue unit in the Department of Fire, Rescue and Inspection Services is of upmost ,t importance; and WHEREAS, the existing Life Support Equipment was �a manufactured by Physio-Control Corporation; and WHEREAS, Physio-Control Corporation is the only known supplier who can provide this service to the Physio-Control Life .k ..s Support System; and WHEREAS, funds for this purchase are available in the '5 General Fund and Telephone Franchise Budgets; and WHEREAS, the Chief Procurement officer has adopted a finding wherein that Physio-Control Corp. is the Sole Source of Supply to the City for this service, and that the pricing offered is both reasonable and favorable; and WHEREAS, the findings of the Chief Procurement Officer have been approved and adopted as the finding of the City Manager; and WHEREAS, The City Manager and the Director of the Department of Fire, Rescue and Inspection Services recommend that the requirement for competitive formal sealed bids be waived and the purchase from Physio-Control Corporation, as the sole source, be approved= THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA% Section 1. By an affirmative vote of 4/5ths of the Members of the City Commission, after a duly advertised public hearing, the City Manager's written finding that Physio-Control Corporation is the Sole Source of supply to the City for furnishing maintenance service and related supplies for the Physio Control Life Support Equipment on a contract basis for one (1) year renewable annually to the Department of Fire, Rescue and A i Inspection Services at a proposed first year cost of $2 4,000.00 is hereby accepted with funds therefor hereby allocated from the General Fund and Telephone Franchise Budgets. Section 2. The City Manager is hereby authorized to instruct the Chief procurement Officer to issue a purchase order for this service, subject to the availability of funds.
