'85-86-96 'TO PROVtbR LABORATORY SERVICES TO THE PERSONNEL MANAGEMENT DEPARMENT FOR NINETY 00) DAYS AT A 'TOTAL PROPOSED COST 6F $15 00010a ALLOCATING PONDS THEREFOR PROM THE 1987-88 OPERATING BUDGETS OF THE USING DEPARTMENTS; AUTHOR121140 'T14E CITY MANAGER TO INSTRUCT THE CHIEF PROCUREMENT OFFICER TO 1551E A PURCHASE ORDER FOR THIS 89RV1C8 SUBJECT TO THE AVAILABILITY OF PHNDS6 WHEREAS, this service will be used by the Department of Personnel Management for the purpose of performing Pre -employment urine/drug profiles for Fire, Police and any other designated applicant and to perform substance abuse testing when needed and WHEREAS, funds for this purchase will be available from the 1987-88 Operating Budgets of the using departments; and WHEREAS, this contract was originally approved on Resolution No. 86-745 on September 11, 1986, Bid No. 85-86-96; and WHEREAS, the City Manager and the Director of the Department of Personnel Management recommend that ,the extension for ninety (90) days for Laboratory Services provided by Toxicology Testing Services be approved; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The extension of the existing contract from Toxicology Testing Services approved originally on Resolution No. 83-530, Bid No. 85-86-96 to provide Laboratory Services for the Department of Personnel Management at _a total proposed cost of $15,000.00 is hereby approved with funds therefor hereby allocated from the Operating Budgets of the using departments. The proposed resolution extends the termination date of the contract by one (1) year to September 11, 1588. The extension is necessary -so that Toxicology Testing Services can continue to provide laboratory services for pre -employment I urine 'drug profiles for all prospective employees. Laboratory c services will also include 1) annual -physical drug 'screen .G profiles for Police, Fire and Day Care employees and 2) testing of employees suspected of being under the influence of alcohol or may have been using, possessing, dispensing or selling any illegal drug or controlled substance not prescribed by a licensed physician. The need for laboratory services is outlined in Section 3(f) of Ordinance No. 8526 adopted by the City Commission ,on January 20, 1976. The specifications for this mandate are for the Department of Personnel Management to establish and maintain medical services which shall include a medical examination of employees `• and potential employees.
