A RESOLUTION AUTHORIZING THE EXTENSION OF THE
EXISTING CONTRACT FOR PHYSICAL EXAMINATIONS
TO CEDARS ME0ICAL CENTER, INC. APPROVED
ORIGINALLY BY RESOLUTION NO. 83-506 (BID NO.
81-82-135) TO PROVIDE ANNUAL PHYSICAL
EXAMINATION TO THE POLICE DEPARTMENT FOR ONE
(1) YEAR RENEWABLE ANNUALLY AT A TOTAL
PROPOSED COST OF $110,000.00,ALLOCATING FUNDS
THEREFOR FROM THE 1987-88 OPERATING BUDGET
INDEX CODE NO. 290201-260,, AUTHORIZING THE
CITY MANAGER TO INSTRUCT THE CHIEF
PROCUREMENT OFFICER TO ISSUE A PURCHASE ORDER
FOR THIS SERVICE , SUBJECT TO THE AVAILABILITY
I OF FUNDS.
WHEREAS, the Police Department has been utilizing the
professional services of Cedars Medical Center, Inc. to provide
annual medical evaluation/examinations of sworn personnel in
order to ascertain that these personnel are in satisfactory
physical condition; and
WHEREAS, funds for this service will be available from the
1987-88 Operating Budget, Index Code No. 290201-260; and
WHEREAS, this contract was originally approved on
Resolution No. 83-506 on June 9, 1983, Bid No. 81-82-135; and
WHEREAS, the City Manager and the Director of the Department
of Police recommend that the extension for one (1) year for
physical examinations provided by Cedars Medical Center, Inc. be
approved;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The extension of the existing contract from
Cedars Medical Center, Inc. approved originally by Resolution No.
83-506 (Bid No. 81-82-135) to provide annual medical
evaluations/examination of sworn personnel renewable annually for
the Department of Police at a total proposed cost of $110,000.00
is hereby authorized with funds therefor hereby allocated from the
Operating Budget Index Code No. 290201-260.

Section 2. The City Manager is hereby authorized to
instruct the Chief Procurement Off icet to issue a Purchase Order
for this service, subject to the availability of funds.

RECOMMt$DATION:
It is respectfully recommended that a resolution be passed
extending the existing contract for Annual Medical Physicals with
Cedars Medical Center, Inc. through September 30, 19880 under the
same terms, conditions, and prices as authorized in Bid No. 81 -
82-135 for a total proposed amount of $110,000.
BACKGROUND:
The Police Department has utilized the professional services of
Cedars Medical Center, Inc. since 1983. They have been providing
medical evaluations/examinations of sworn personnel in order to
ascertain that these personnel are in satisfactory physical
condition. The complete basic physical examination for police
officers includes the following: Complete blood analysis;
urinalysis; chest x-ray and pulmonary function;
electrocardiogram; vital statistics, i.e. weight, height, etc;
medical history; audiometry; vision; tonometry; physical
examination by a qualified physician; and, for female officers,
pelvic examination and pap smear.
In addition, Cedars Medical Center, Inc. also provides physicals
for executive staff members which include all of the items/test
listed above for police officers, as well as sigmoidoscopy and a
cardiac treadmill stress test performed by a board certified
cardiologist.
It is, therefore, recommended that an extension of the contract
for Annual Medical Physicals with Cedars Medical Center, Inc. be
approved through September 30, 1988- The funding source will be
$110,000 from the Fiscal Year 1987-88 Police Operating Budget,
Index Code 290201-260.
