Execution (Laboratory

? It is respectfully recommended that the City Commission authorize
j the execution of an agreement in a form acceptable to the City
Attorney with Toxicology Testing Service, Inc., approved
originally by Resolution No. 86-745, Bid No. 85-86-96 to provide
laboratory services to the Department of Personnel Management for
one (1) year at an estimated cost of $80,500.
BACKGROUND:
The Department of Personnel Management has analyzed the need for
execution of the attached resolution. The proposed resolution
extends the contract by one (1) year to May 14, 1989.
The extension is necessary so that Toxicology Testing Services
can continue to provide laboratory services for pre -employment
and annual physical drug screenings and substance abuse testing
j when needed. Representatives from the Departments of Personnel
Management, Fire, Police and the Office of Labor Relations met
and are in agreement regarding the extension of this contract.
The need for laboratory services is outlined in Section 3(f) of
Ordinance No. 8526 adopted -by the City Commission on January 20,
1976. The specifications for this mandate are for the Department
of Personnel Management to establish and maintain medical
services which shall include a medical examination of employees
and prospective employees. Additionally, negotiated labor
agreements allow substance abuse testing.
Provisions have been made within the budgets of the various
operating departments and Special Programs and Accounts to defray
the costs associated with laboratory services.

1. SCOP6' AND PURPOSE

for the furnishing of all labor and materials. and equipment as
required for Laboratory Services -- Pro -Employment Urine drugs
profiles on all Potice and Fire Department candidates and for any
other designated applicants for the Personnel flanagement
Department, Attention is directed to Special. Condition No. 12.
2, TERM OF CONTRACT
The `cohtr`act s ,a be effective for a period of one (1) year,
con-me•ncing with the date of the C'ontractoi•'s acceptance of award
or purchase order, and shall be renewable annually by mutual
written consent#' under the same prices, t( rms. and conditions and
subject to the availability of funds for succeeding fiscal years.

In the event the contractor is unable to extend the Contract for
a subsequent one year period, advance written notice and
explanation must be submitted to the Chief Procurement officer
not later than ninety (90) days prior to the annual expiration
date.
3. PRICING AND PAYTTENT
PuMiaies w 11 be •covered by purchase orders issued against the
Contract as item(s) are required, or blanket purchase orders will
be issued.
Pricing established herein shall prevail for the term of the
contract. Payment shall be made for work performed /items
delivered upon submission of property certified invoices. No
advance payments of any kind will be made 1N the City of Hiami.
4. BLUUER QUALIFICATIONS
aj An inspection -of the Bidders' facilities and/or equipment may
be made prior to award and bidders shall provide all information
requested regarding their ability to perform in accordance with
the contract provisions.
b) Bids will be considered only from firms that are regularly
engaged in the business of providing the goods and/or services as
described in this Bid; that have a record of performance for a
reasonable period of time; and that have sufficient financial
support, equipment and organization to insure that they can
satisfactorily execc:te the services if awarded a Contract under
the terms and conditions herein stated.
The term "equipment and organization" as used herein
construed to mean a fully equipped and well established
as determined by the proper Authorities of the city
