City of Miami man dates the Department of Personnel Management to establish and maintain medical services which shall include a medical examina- tion of employees and prospective employees. Additionally, agreements between the City of Miami and the Fraternal Order' of Police (FOP) and the International Association of Fire Fighters (IAFF) bargaining units allow for a scheduled physical examina- tion program. Screen Medical Diagnostic, Inc. - Minority Vendor, low bidder. An inspection of the low bidders' facility was conducted on i March 31, 1989 by the following individuals: Name Department z Angela R. A follow-up inspection was conducted on April 6, 1989, by Renee S. Jones and Lt. P.J. Mount. — It was the consensus of the entire group that the low bidder be eliminated based on the following: 1. Staffing Only one (1) physician to examine approximately 35 - employees per day. Insufficient staff for administration of Cardiac Stress Testing. Did not submit educational credentials of all staff as requested twice. 2. Facility Size Only two (2) examination rooms available `to handle City employees plus private patients.a- Insufficient space for collection of drug testing samples. Very small reception area which could only accommodate approximately 20 individuals. Insufficient parking.spaces (approximately 12). 3. Equipment Vision and Audiological Equipment not.available for inspection. Bidder indicated he would purchase equipment if awarded bid.
