AN ORDINANCE ESTABLISHING A NEW SPECIAL
REVENUE FUND ENTITLED: "DADE COUNTY EMS
GRANT AWARD (FY'93)" AND APPROPRIATING FUNDS
FOR ITS OPERATION IN THE TOTAL AMOUNT OF
$223,937, CONSISTING OF A $136,703 GRANT
APPORTIONED BY METROPOLITAN DADE COUNTY FROM
THE STATE OF FLORIDA DEPARTMENT OF HEALTH AND
REHABILITATIVE SERVICES UNDER THE "FLORIDA
EMERGENCY MEDICAL SERVICES GRANT PROGRAM FOR
COUNTIES", AND $87,234 IN CARRY-OVER FUND
BALANCE FROM PREVIOUS EMS GRANT AWARDS;
AUTHORIZING THE CITY MANAGER TO ACCEPT THE
AFOREMENTIONED GRANT AWARD AND ENTER INTO THE
NECESSARY LETTER OF UNDERSTANDING WITH THE
FLORIDA DEPARTMENT OF HEALTH AND
REHABILITATIVE SERVICES AND/OR METROPOLITAN
DADE COUNTY, IN A FORM ACCEPTABLE TO THE CITY
ATTORNEY AND SUBJECT TO APPLICABLE CITY CODE
PROVISIONS; CONTAINING A REPEALER PROVISION
AND A SEVERABILITY CLAUSE.
WHEREAS, the Florida Department of Health and Rehabilitative
Services is authorized by Chapter 401, Part II, Florida Statutes,
to provide grants to boards of county commissioners to improve
and expand pre -hospital emergency medical services; and
WHEREAS, it is the intent of the members of the Dade Board
of County Commissioners that the 1992-93 funding for Dade County
from the Florida Department of Health and Rehabilitative
Services, "Florida Emergency Medical Services Grant Program for
Counties" in the amount of $593,861 be apportioned and passed
through to participating municipal
individual projects; and

WHEREAS, the City of Miami's allocation of this total grant
for 1992-93 is $1-36,703, with a carry-over fund balance from
previous grant awards of $87,234; and
WHEREAS, the City of Miami Department of Fire, Rescue and
Inspection Services' Work Plan has been approved by Dade County
and now desires to implement the following projects under a total
grant appropriation of $223,937: (1) Purchase of Safety
Literature - $4,752; (2) Purchase of a Rescue Vehicle -Ambulance-
$40,000; (3) Purchase of a Mass Casualty/Firefighter/Paramedic
Rehabilitation Vehicle - $30,000; (4) Purchase of EMS and Video
Equipment - $21,484; (5) Establishment of an Injury Control
Program - $37,601; (6) Attendance of EMS Personnel at National,
Regional and Local Conferences and Seminars - $10,000;
(7) Provision of Infection Control Clothing - $3,000;
(8) Purchase of Five (5) EKG Monitor Defibrillators - $70,000;
and (9) Provide Training for Department Personnel to be Qualified
and Certified as Emergency Medical Dispatchers - $7,100;
NOW, THEREFORE, BE IT ORDAINED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the
Preamble to this Ordinance are hereby adopted by reference
thereto and incorporated herein as if fully set forth in this
Section.
Section 2. The following New Special Revenue Fund is
hereby established and resources are hereby appropriated as
described herein:

Section 3. This Appropriation is contingent upon final
approval of the State of Florida Department of Health and
Rehabilitative Services and the City Manager's acceptance
thereof.
Section 4. The City Manager is hereby authorized to
accept the aforementioned grant and enter into the necessary
Letter of Understanding with the Florida Department of Health and
Rehabilitative Services and/or Metropolitan Dade County, in a
form acceptable to the City Attorney and subject to applicable
City Code provisions.
Section 5. All ordinances or parts of ordinances insofar
as they are inconsistent or in conflict with the provisions of
this Ordinance are hereby repealed.
Section 6. If any section, part of section, paragraph,
clause, phrase or word of this Ordinance is declared invalid, the
remaining provisions of this Ordinance shall not be affected.
Section 7. This Ordinance shall become effective thirty
(30) days after final reading and adoption thereof.

It is respectfully recommended that an Ordinance be passed
establishing a New Special Revenue Fund entitled "Dade County EMS
Grant Award (FY192)", appropriating funds in the amount of
$223,937; consisting of a $136,703 Grant, apportioned by
Metropolitan Dade County, from the State of Florida Department of
Health and Rehabilitative Services under the "Florida Emergency
Medical services Grant Program for Counties" and $87,234 in
carry-over fund balance from previous EMS Grant Awards; and
authorizing the City Manager to accept the Grant Award and to
enter into the necessary Letter of Understanding with the Florida
Department of Health and Rehabilitative Services and/or
Metropolitan Dade County.

The Department of Fire, Rescue and Inspection Services has been
awarded a grant under the State's "Florida Emergency Medical
Services Grant Program for Counties, for the following work

TO APPLY FOR, RECEIVE AND EXPEND EMERGENCY
MEDICAL SERVICES GRANT AWARD FUNDS FOR
IMPROVED AND EXPANDED PRE -HOSPITAL EMERGENCY
MEDICAL SERVICES (EMS) PROGRAM
wuF,REAS, this Board desires to accomplish the purposes
outlined in the acconpanying memorandum and attachments, copies of
which are incorporated herein by reference,
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COIV,ISSIONBRS OF D;+DE COUNTY, FLORIDA, that this Board authorizes
the County Manager to file a 1992-93 grant application for
Emergency Medical Services Award funds to be used to improve and
expand the pre -hospital Emergency Medical System in Dade County,
in substantially the form attached hereto and made a part hereof;
to receive and expend any and all monies received for such
purposes described in the grant application; to execute such
co^tracts and agreements that are required by this governmental
body, following approval by the County Attorney's office, for and
on behalf o- Dade County, Florida; and to file and execute any
amendments to the application.

it is recommended that the Board authorize the County Manager to
apply for, receive and expend Emergency Medical Services Grant
Award funds in an amount not to exceed $1,294,302 in the Fiscal
Year 1992-93, to be used to improve and expand the Countywide
Pre -Hospital EMS System.

Each year the Department of Health and Rehabilitative Services
(HRS), Office of Emergency Medical Services (EMS)aistributecrida
County Grant Funds as authorized by Chapter 401, p
Statutes (E.S.). These funds are made available to eligible
County governments to improve and expand their pre -hospital
emergency medical services. The funds are,derived from
surcharges on various traffic violations.
A stipulation of the grant is that municipal fire departments are
to apply for and receive funds through their respective county
Government or county fire department. The Paramedic Coordinating
Council, which includes members of the four municipal fire
departments, as well as Metro -Dade Fire Department, conducts Awardant
an
annual needs assessment to formulate the Dade County
application. Coordination of the grant application with this
area EMS Council is a requirement of the application process.
Approximately 5500,000 of new Grant Award monies is anticipated
for 1992-93. The new revenue (S500,000) combined with prior year
funds and interest earned forms the basis for the 1992-93 Work
Plan amount of $1,294,302. A detailed description of the
proposed projects is provided in the attached Work Plan.
The attached resolution certifies that the activities included in
our application will improve or expand the pre -hospital EMS
system in Dade County and that funds will not be used to supplant
existing budget allocation for these services.
The Fire Board has reviewed and approved this item.

APPLICATION FOR EMS GRANT PROGRAM FOR COUNTIES
WORK PLAN FOR FY 1992-93
It is the intent of the members of the Dade Board of County
Commissioners that the 1992-93 funding for Dade County,
estimated to be $593,861.06 (Pending future adjustments per
final HRS-EMS letter of approval), plus any monies carried
forward from Grant #C9113 (FY 1991-92) be apportioned and
passed through to the participating municipal fire departments
in support of the projects herein proposed. Performance and
financial reports, as described in the 1992-93 EMS County Grant
Application, will be assembled and forwarded to HRS by Dade
County. However, HRS agrees to conduct performance and
financial compliance audits directly with the municipal fire
department responsible for the individual project.

1. ADDITIONAL RESCUE VEHICLE -AMBULANCE:
Project# MF.92-93.01 Total Budget $40,000.00

Purchase or upgrade and place into service an additional Rescue
Vehicle -Ambulance within the grant period, upon formal approval and

2. MASS CASUALTY/FIREFIGHTER/PARAMEDIC REHAB VEHICLE:
Project# MF.92-93.02 . Total Budget $25,000.00
Provide a Mass Casualty Incident (MIC) Vehicle to be in service daily
for the on -duty Rescue Battalion Captain that will respond with
proper equipment and supplies to all medical disasters, mass casualty
incidents and second alarms or larger fires, to improve the level of
service provided to the residents of the City of Miami.
Actions and Time Frames:
Purchase and place into service an MCI (van type) Vehicle within the
grant period, upon formal approval and actual receipt of grant funds.

Purchase new EMS and Video equipment which will enhance the
service capabilities of the EMS System and improve the level of
service provided to the residents of the City of Miami.
Actions and Time Frames:
Identify, purchase, and place into service new EMS and Video
equipment within the grant period, -upon formal approval and actual
receipt of grant funds.
4. INJURY CONTROL PROGRAM:
Project# MF.92-93.04
Total Budget $37,601.00
Total Hours -Teacher 1820
Total Hours -Aide 1260
Provide funds to deliver an Injury Control Program targeted at high
risk children who live in the City of Miami. The Grant Resource
Teacher and Teacher Aid hired will provide training on Injury Control
to students in the Classroom and provide train -the -trainer workshops
to teach school teachers how to utilize the Injury Control Curriculum
and provide students with the life saving behaviors.
The Grant Resource Teacher and Teacher Aide hired will immediately
begin scheduling Injury Control classes at the various public and
private schools. The teacher training workshops will be scheduled
through the Dade County School Board Teacher Education Center, upon
formal approval and actual receipt of grant funds.

Provide a means to digitally transmit alarm information to laptop
computers to enable them to be used as Mobile Data Terminals. This
will serve to upgrade the existing EMS Patient Database information.
Actions and Time Frames:
Identify, purchase and place into service the necessary hardware and
software within the grant period, upon formal approval and actual
receipt of grant funds.
6. CONFERENCES AND SEMINARS:
Project# MF.92-93.06 Total Budget $10,000.00
Provide funds for EMS personnel to participate in national, regional
and local conferences and seminars, to keep the department abreast of
trends and innovations occurring in the EMS field.
Pay for registration fees and travel expenses incurred in attending
national, regional and local conferences and seminars throughout the
grant period, upon formal approval and actual receipt of grant funds.

Dear County -Commissioners:
I am pleased to provide you with Florida's 1992 emergency
medical services county grant booklet. It includes an
application and all the information needed to request your
allocation of this year's county grant funds.
No tax funds from the general public or private businesses
are used in this program. All monies are derived from surcharges
on traffic fines. Approximately $25 million was distributed to
counties in the first six years of the emergency medical services
grant program. These funds were used to improve and expand
prehospital services systems throughout the state.
Florida's innovative grant program has led to new and
better ways of providing emergency med:zal services for
Florida's residents and visitors, resulting in the saving of
lives. Working together we can continue to make a significant
difference in pursuing excellence for Florida's emergency
medical services.

DEFINITIONS
Comptroller - Comptroller of Florida, Department of
Banking and Finance.
Department - Department of Health and Rehabilitative
Services.
Div. Comm. - Florida Department of General
Services, Division of Communications.
EALS County (iron. -Funds awarded to each eligible
county according to the respective proportion of each
county's annual contribution to the EMS grants
program portion of the store EMS Trust Fund
Expand - An increase in the extent, size, or number of
existing prehospital EMS activities and services.
Expenditure Plan - A line item budget identifying
services and equipment to be purchased.
Fiscal Year - Designation of a 12-month period for
EMS county grant program is October I through
September 30.
Grant Funds - Money deposited into the state EMS
Trust Fund for distribution under Chapter 401, Pan
11. F. S.
Grantee - The legally accountable county which
submits an application, receives and assumes
responsibility and accountability for both the awarded
funds and for the performance of the grant supported
activity.
Improve - To advance or to make better the existing
quality of prehospital EMS activities and services, or
to decrease patient mortality and morbidity.
Notice of Grant Award - The letter sent by the state
EMS Office authorizing the grant award amount,
buds et, time period, work plan and any special
conditions in the grant.
Prior Approval - Written permission by the department
for the grawee to use awarded grant funds for certain
purposes not included or specified in the current
approved budget or to change certain aspects of the
budget.
Remaining Value - Tile value of the facility, vehicle or
equipment at that point in time which it ceased to be
used solely for EMS.
Request for Grant Distribution - A request form for
advance payment of awarded grant funds, signed by
the grantee (county official) and returned to the state
Office of EMS for approval and transmittal to the state
Department of Banking and Finance, Office of the
Comptroller.

DESCRIPTION OF
PROGRAM
OVERVIEW
The Department of Health and Rehabilitative Services,
Office of Emergency Medical Services is authorized by
Chapter 401, Pan 11. Florida Statutes, to distribute
..t.• e.nnAM.!! A r+nrfiOn of these funds are made
available to eligible county governments to improve
and expand prehospital emergency medical services
(EMS) systems in their county. The grant program is
an innovative process which helps boards of county
commissioners (BCC;) to improve and expand the
EMS systems of municipalities, agencies, and
organizations within their respective counties.
On -going costs for EMS and the replacement of
rc:i:rment cannot be funded under this grant program.
Tre:• remain the responsibility of the counties and
EMS agencies and organizations.
SOURCE OF FUNDING FOR COUNTY
GP,4NT PROGRAM
Counry grant funds are derived from surcharges on the
fines for various traffic violations. Funds are not
obtained from any tax upon the general public or
private businesses.
ELIGIBILITY
County grants are awarded only to BCCs. However,
each BCC should determine its counrv►vide Eh:S needs
before submitting a grant application. The assessment
should be coordinated with area EMS councils, when
thev are available. The county- may use county grant
funds to help municipal and other agencies and
organizations to improve and expand prehospital EMS
including licensed EMS services, first responder
organization, the American Red Cross, the American
Heart Association, EMS training centers, and
acadenic institutions.
GRANT PROCESS
?he grant application process is quite simple:
1. The BCC completes and returns the county
grant application along with the necessary resolution
(see Item S in the application) to the department. No
other documents are needed to apply.
2. The BCC retains this booklet because it
contains grant requirements and other informnation
and forms needed to comply with the grant.
3. The department processes the application and
sends a Notice of Grant Award (Appendix A) to the
BCC.
4. The grant will begin October 1, 1992. After
we send you the Notice of Grant Award, we request
the Department of Banking and Finance, Office of the
Comptroller, to issue your advance payment check
Completed applications and resolutions should be
received by the department no later than October 1,
1992.
SUBMIT YOUR COUNTY
GRANT APPLICATION
Complete and submit the grant application to us, with
the resolution requested by Item S of the application.
Remember, the resolution must cent that the funds
you receive from this program will improve and
expand your county's prehospital EMS system, and
that the funds will not be used to supplant the existing
county EMS budget allocations. Items two and eight
of the application must have the original signatures of
the authorized county official. We cannot process any
application which is incomplete. Mail your
application, and resolution to us by certifued mail,
return receipt requested:

GRANT AIVARD PAYMENT AND
REPORTING
After the grant begins, changes in the original grant
application must be requested on the Grant Change
Reouest document in this booklet (Appendix B). Any
change must receive our written approval before
making expenditures which are rot consistent with the
current approved budget.
Tne grantee is required to maintain complete and
accurate accounting and program records. These

records must be readily available for inspection by
state and independent audit personnel.
The grantee is required to submit program activity and
expenditure reports to the department as requested
(Appendix D).
CONTIAT17"Y OF GRANTS
It is important to ensure your next grant begins after
this one ends, with no unfunded time between them,
contact your state EMS grant* officer for special
procedures.
CHECK FACSIMILE PRESEATATION
The grantee may request a public presentation by us to
you, of the county's grant check at the time the grant
is awarded. This could result in favorable publicity
for the BCC, your EMS system, and the store's EMS
grant program. If interested, please contact your state
EMS grant officer for further details.
EMS MATCHING GRANTS
All counties are also eligible for the state EMS
matching grant program. Contact your state EMS
grant officer for information. County grant funds
cannot be used to fulfill any state EMS matching grant
program requirements.

GENERAL CONDITIONS
1. The grantee herebv agrees to: (a) improve the
existing qualiry of prehospital EMS activities, services,
or to decrease patient mortality and morbidity, and (b)
to expand the extent, size or number of existing
prehosphal EMS activities or services. The activities
and services to be provided are described in thee EMS

revisions on file in the State of Florida, Department of
Health and Rehabilitative Services, Office of EMS,
Tallahassee, Florida.
2. Prehospital EMS activities will be rendered,
by the grantee in a manner consistent with Chapter
401. Florida Statutes, Chapter 10D-66 Florida
Administrative Code and the work plan in the
grantee's EMS county grant application and its
department approved revisions on file in the state's
Office of EMS.
3. The grantee agrees to implement provisions of
this grant in accordance with federal, state, and local
laws, and rules, regulations. and policies that pertain
to EMS.
4- The grantee ensures that EMS county grant
funds will not be used to supplant or replace any other
funds. The EMS eounry grant funds will not be used
for any state EMS marching grant requirements.
5. The grantee shall not use or disclose any
information concerning a recipient of prehospital
activities under this grant for any purpose not in
conformity with state regulations and federal
regulations (45CFR. Parr 205.50) except upon written
consent of the reeipiera or his responsible parent or
guardian when authorized by law.
6. Incident Reporting:
a. Client Risk Prevention
If services to clients will be provided under this grant
the provider and any subcontractors shall, in
accordance with the client risk prevention system,
report those reportable situations listed in
HRSR 215-6. Paragraph 5, in the manner prescribed
in HRSR 215-6 or district operating procedures.
b. Abuse, Neglect and Ecploitation Reporting
in compliance with Chapter 415, F.S., an emplovee of
the grantee who knows, or has reasonable cause to
suspect, that a child, aged person or disabled adult is
or has been abused, neglected, or exploited, shall
immediately report such knowledge or suspicion to the
central abuse registry and tracking system of the
department an the single statewide toll free telephone
number (1-800-96ABUSE).
7. EMS county grants are subject to the
availability of funds. The department's performance
and obligation to pay under this grant is contingent
upon a sufficient annual appropriation by the Florida
Legislature.

8. The grantee is not restricted to staying within
the line item amounts reflected in the approved
budget. However. the grantee must stay within the
approved total grant budget and not exceed any
budgeted individual salary more than 10 percent.
9. Travel and per diem expenses shall be in
compliance with section 112.061, F.S.
10. The grantee shall be responsible for any
amount reflected in the approved budget which is in
excess of the grata: award. The grantee shall be
responsible for all expenses and cost which occur
before and after the grant begins or ends.
11. The department shall provide the grantee
HRS logos and decals. The grantee shall affix, ir a
prominent location, the HRS logos and decals to all
items purchased as pan of the approved budget line
items designated by the department as requiring them.
12. All computer equipment (hardware and
software) must be compatible with the state Office of
EMS computer systems. Costs for computer equipment
and activities which are not compatible with the state
Office of EATS computer system shall be disallowed.

13. The grantee agrees that all communications
activities and equipment shall be approved in writing
by the Florida Department of General Services,
Division of Communications as required by section
401.024, F.S. Any costs for communications activities
and equipment which do not receive such approval
shall be disallowed. This approval must be dated
after the effective date of the grant and prior to any
commitment to purchase the requested equipment
and/or serviar or both. This approval is required in
addition to any previous Division of Communications
recommendation, review, conceptual or other

14. The grantee shall own all items including
facilities, vehicles, and equipment purchased by EMS
county grant funds unless otherwise described in the
grant application and/or award. The grantee shall
clearly document the assignment of facilities, vehicles,
and equipment ownership and usage, and maintain
this documentation so it is available during any audit.
Tire owner shall be responsible for the proper
insurance, licensing, permitting, and maintenance of
facilities, vehicles, and equipment purchased with
county EMS grant funds.
15. The grantee must ensure facilities, vehicles,
and equipment purchased by EMS county grant funds
shall be used solely for improving and expanding pre -
hospital EMS throughout their useful life whether
owned by the grantee or other recipients.
16. 7-he grantee agrees tha: a complete copy of
the EMS Notice of Grant Award, department approved
budget, approved grant changes if any, and grant
application, must be on fik with the county person
responsiblefor directly administering the grant.
17. The grantee, his sub -grantees) or
contractor(s) and assignees shall provide access to,
and furnish whatever information is necessaryfor the
department to monitor the grant including access to
all client records.
18. A Vendor Omb::dsman has been established
within the Department o; Banking and Finance. The
duties of this individual include acting as an advocate
for vendors who may be experiencing problems in
obtaining timely pavmem(s) from a state agency. The
vendor ombudsman may be contacted at (904)
4W-2924 or by calling the State Comptroller's
Plotkin, 1-800-848-3792.
19. The Grantee agrees to comply .within the
provisions of section 216.347. Florida Statutes, which
prohibits the expenditure of grant funds for the
purpose of lobbying the Legislature or a state agency.
EMS COUNTY GRANT
GENERAL REQUIREMENTS
77sese requirements are an integral part of the EMS
county grant. 1n the event of a conJIict, the following
requirements shall always be controlling:
1. A final financial report shall be submitted
detailing all expenditures of this grant. Bills for fees
or other compensation for services or expenses shall
be maintained by the grantee in sufficient detail for a
proper pre -audit and post -audit of the grantee's
records.
2. To the extent provided by law, the grantee
hereby agrees to indemnify and hold the department
harmless from any and all claims or demands for any
personal injury or property damage resulting or
occurring in connection with any ac:ivities conducted
under the grant agreement and shall investigate all
such claim: of every nature at its expense. In
addition, to the extent provided by law, the grantee
hereby agrees to be responsible for any injury or
property damage resulting from any activities
conducted under this EMS grant.
3. The grantee agrees to use the facilities,
vehicles, and equipment purchased under this grant,
solely for EMS throughout their useful life, or the
grantee must return to the department funds equivalent
to the remaining value of the facilities, vehicles and
equipn, ens .
4. All tour. y grants are made in accordance
with section 401.113(2)(a), F.S., and shall be made
through an EMS Notice of Grant Award. The Notice
of Grant A►,•ard and Appendix D of this booklet
contain by reference all regulations, rules, and other
conditions governing this grant. The Department
agrees to request advance distribution (payment) of
the grant amount when the Notice of Grant Award is
sent to the grantee. Tnis payment is usually sent to
the grantee by the Department of Banking and

Finance, Office of the Comptroller, after the notice of
award is mailed.
5. Any payments due the grantee under the terms
of this grant may be withheld until all special and
financial reports due from the grantee, and necessary
adjustment thereto, have been approved by the
department.
6. All terns, conditions, and provisions of this
grant and other applicable documents in this booklet
are hereby reaffirmed. Acceptance of the grant terms
and conditions is acknowledged by the grantee when
funds are drown or otherwise obtained from the grant
payment system.
7. All EMS county grant funds shall be
deposited by the grantee in an account maintained by
Me grantee, and shall be assigned unique accounting
code designator for all grant deposits and
disbursements or expenditures thereof. Al! EMS
county grant funds in the account maintained by the
grantee shall be accounted for separately from all
other grantee funds.
& All EMS county grant funds shall be used
solely for prehospital activities as outlined in the
attachments to the Notice of Grant Award. Any
unused EMS county grant Rends remaining in the
assigned grantee account at the end of the grant
period, including accrued interest, shall be reported to
the department, remain in the grantee's account and
shall be applied toward the next year's EMS county
grate funding requirements. Any disallowed county
grant expenditure shall be returned to the county grant
account maintained by the grantee within 45 days
after the issuance of the department's notice of
determination. The cost of the disallowed hem(s)
shall be the responsibility of the county.
REVISIONS TO EMS
COUNTY GRANT
The grantee shall obtain written approval from the
department's EMS grant officer on the EMS County
Grant Change Request document (Appendix B) in this
booklet prior to any of the following changes to the
grant budget:
1. Introducing a new line item in the approved
budget.
2. increasing any budgeted individual salary
more than 10 percent.
3. Introducing a sub -grantee or contractor
relationship where none is specifically identified in the
approved
budget.
REPORTS AND
DOCUMENTATION
Tile grantee shall send to the department one original
and two copies of each required report as follows:
1. Tne Expenditure Report (Appendix C)
indicating all agreed upon budget expenditures. The
required deadlines for providing the department with
Expenditure reports are as follows:
a. April 30, 1993, for activities October 1, 1992
through March 31, 1993
b. July 31, 1993, for activities April 1, 1993
through June 30. 1993
C. October 31. 1993, for activities July 1, 1993
through September 30, 1993 (final report)
2. Tile Program Activity Report is due
October 31, 1993. It must contain a written summary
indicating the degree to which the EMS system was
improved and expanded through this grant as follows:
a. Communications Equipment and Systems
If the county grant includes communications
equipment and systems, the grantee agrees to submit a
brief one page report to the department describing the
improvement and expansion brought about by the
purchase of communications activities, services. and
equipment purchased with grant funds and identified
in the grantee's original application.
b. Emergency Transport Vehicles
If the county grant includes the purchase, licensing,
basic life support (BLS) or advanced life support
(ALS) permitting of an emergency transport vehicle, or
the refurbishing of an existing BLS or ALS permitted

emergency transport vehicle, the grantee agrees to
provide the department the following information
about vehicles purchased with grant funds: number of
emergency and nonemergency responses; total mileage
accrued; number of days in service; number of days
out of service; mean response time (measured from the
time an EMS dispatcher is notified until the crew
arrives on scene) for emergency calls.
c. Medical/Rescue Equipment
If the county grant includes purchase and use of any
or all equipment which meets standards in Chapter
IOD-66, F.A.C., or is approved by the provider's
rr-d;rn! d;r-crcr, the grantee agrees to provide the
department the following information about equipment
purchased with grant funds; the number of individual
instances where defibrillators and power rescue tools
were used.
d. Public Education
If the county grant includes public education activities
or services, the grantee agrees to provide the
department the following information about activities
or services conducted with grant funds: the number of
participants and number of participants who
successfully completed the training course.
e. Continuing Professional Education
If the county grant involves continuing professional
education activities or se -vices, the grantee agrees to
provide the department the following information
about activities or services conducted with grant
funds: type of training conducted; number of
participants; and the number of individuals
successfully completing any training course.
f. Svstem Evaluation/Quality Assurance
If the county grant invol.rs system evaluationlaunliry
assurance, the grantee agrees to submit a brief one
page report describing the improvement and expansion
brought about by any quality assurance or evaluation
activities conducted with grant funds.
Program Activity and Expenditure reports will be used
to ensure adequate monitoring and auditing of the
grantee. These reports must be submitted within the
time frames specified. The grantee, his sub -grantees)
or contractor(s), and assignee(s) shall provide access
to and furnish whatever information is necessary for
us to monitor this grant including access to all client
records.
ASSURANCE OF
COMPLIANCE
CREDIT STATEMENT
The grantee assures that where activities supported by
this grant produce• original writing,- sound record •rg,
pictorial reproductions, drawings, or other graphic
representations and works of any similar net -re,
notices, informational pamphlets, press releases,
advertisements, descriptions of jhe sponsorship of the
program, research reports, and similar public notices
prepared and released by the provider shall include
the statement:
'Sponsored by
Grantee Organization
and the State of Florida, Department of Health and
Rehabilitative Services, Office of Emergency Medical
Services. If the sponsorship reference is in written
material, the words 'State of Florida, Department of
Health and Rehabilitative Services, Office of
Emergency Medical Services' shall appear in the same
size letters or type as the name of the grantee's
organization.
One copy of any such materials will be sent to the
department.
COPYRIGHTS
Tne grantee assures that where activities supported by
this grant produce original writing, sound recordings,
pictorial reproductions, drawings or other graphic
representation and works of any similar nature, the
department has the right to use, duplicate and disclose
such materials in whole or in par:, in any manner, for
arri purpose M•hatsoe.vr and to have others acting on
behalf of the department to do so. If the materials so
developed are subjeci to copyright, trademark, or
patent, legal title and every right, interest, claim, or
demand of any kind in and to any patent, trademark or
copyright, or application for the same, will vest in the
State of Florida, Department of State, for the exclusive
use and benefit or the state. Pursuant to secion
286.021. F.S., no person, firm or corporation,
including parties to this grant, shall be entitled to use
the copyright, patent or trademark without the prior

CIVIL RIGHTS CEn^TIE ICATION
The granite ordinarily gives this assurance in
consideration of and for the purpose of obtaining
federal grants, loans, contracts (except contracts of
insurance or guaranty), property, discounts, or other
federal financial assistance to programs or activities
receiving or benefiting from federal financial
assistance.
No federal funds are provided by this state grant
program, but compliance with civil rights legislation
is required. The grantee assures that it will comply
with:
1. Title V/ of the Civil Rights Act of 1964. as
amended, 42 U.S.C. 2000d et seq.. which prohibits
discrimination on the basis of race, color, or national
origin in programs and activities receiving or
benefiting from federal financial assistance.
2. Section 504 of the Rehabilitative Act of 1973.
as amended, 29 V.S.C. 794, which prohibits
discrimination on the basis of handicap in programs
and activities receiving or benefiting from federal
financial assistance.
3. Title N of the Education Amendments of
1972, as amended, 20 U.S.0 1681 er seq., which
prohibits discrimination on the basis of sex in
education programs and activities receiving or
benefiting from federal financial assistance.
4. The Age Discrimination Act of 1975. as
amended, 42 U.S.C. 6101 a seq., which prohibits
discrimination on the basis of age in programs or
activities receiving or benefiting from federal financial
assistance.
5. The Omnibus Budget Reconciliation Act of
1981. P.L. 97-35, which prohibits discrimination on
Me basis of sex and religion in programs and activities
receiving or benefiting from federal financial

6. All regulations, guidelines. and standards

7. U.S. Department of Health and Human
Services Regulations for Protection of Human Subjects

The grantee agrees that compliance with this
assurance constitutes a condition of continued receipt
of, or benefit from state EMS funds, and that it is
binding upon the grantee, successors, transfers, and
assignees for the period during which such assistance
is provided. The grantee further assures that all
contractors, subcontractors, sub -grantees, or others
with whom it arranges to provide services or benefits
to participants or employees in connection with any of
its programs and acti;-ities are not discriminating
against those participants or employees in violation of
the preceding statutes, regulations, guidelines, and
standards. In the event of failure to comply, the
grantee understands that the department may, at its
