A RESOLUTION, WITH ATTACHMENTS, ACCEPTING THE
PLAT ENTITLED JEFFERSON REEVES HEALTH CENTER
SUBDIVISION, A SUBDIVISION IN THE CITY OF MIAMI,
SUBJECT TO ALL OF THE CONDITIONS OF THE PLAT AND
STREET COMMITTEE, AND ACCEPTING THE DEDICATIONS
SHOWN ON SAID PLAT; AUTHORIZING AND DIRECTING
THE CITY MANAGER AND CITY CLERK TO EXECUTE THE
PLAT; AND PROVIDING FOR THE RECORDATION OF SAID
PLAT IN THE PUBLIC RECORDS OF DADE COUNTY,
FLORIDA.
WHEREAS, the City Department of Public Works recommends the
acceptance of the plat;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The plat entitled Jefferson Reeves Health Center
Subdivision is a resubdivision of Lot 16 and a portion of each of
Lots 4, 5, 17 and 18, Block 13 N, A.L. Knowlton's Map of Miami,
Plat Book B at Page 41, and a portion of Lot 4, Parry's Division
of Lots 1-2-19-20 in Block 13 N, Plat Book 1 at Page 75, and a
portion of each of Lots 1 and 2 and all of Lots 3 through 36,
A.T. Carters Resubdivision of the West Half of Block 13 N, Plat
Book B at Page 167, and Northwest 4 Court between Northwest 10
Street and Northwest 11 Street, all lying in Section 36, Township
53 South, Range 41 East, City of Miami, Dade County, Florida,
being more particularly described on the attached ATTACHMENT 1,
which plat by reference is made a part hereof as if fully
incorporated herein, and, subject to all of the conditions
required by the Plat and Street Committee as set

"A" attached hereto is hereby accepted. The dedications shown on
the plat together with the dedications to the perpetual use of
the public of all existing and future planting, trees and
shrubbery on said property, are also hereby accepted and
confirmed by the City of Miami, Florida.
Section 2. The City Manager and City Clerk are hereby
authorized and directed to execute the plat and cause the same to
be recorded in the Public Records of Dade County, Florida.
Section 3. This Resolution shall become effective
immediately upon its adoption.

City Manager
The City of Miami Plat and Street Committee, at its meeting of June
10, 1993, approved the above tentative plat subject to the
following revisions being made to the tentative plat, additional
information being provided and/or variances being granted. Please
be advised that the processing of your tentative plat cannot
proceed until these conditions have been satisfied.
- Remove 2-foot high concrete block wall encroachment
into right of way at northwest corner of Tract "A".
Provide radius information for the face of the 2-foot
high concrete block wall at this location.
- Miami -Dade Water & Sewer Authority Department will
require the installation of a 12-inch water main
surrounding the site on three sides: N.14. 10 Street,
N.W. 11 Street and N.W. 5 Avenue.
- Miami Fire Department will require the installation of
two (2) new hydrants: one (1) hydrant at the northeast
corner of N.W. 5 Avenue and N.W. 10 Street and a second
hydrant approximately 300 feet east of this hydrant. A
retap of one existing hydrant will be required also.
- Miami Fire Department will also require a review of the
site plan. Contact Lt. George Scott at 350-7836.
- Florida Power & Light Company will require a 10-foot
wide easement adjacent to and parallel with the
exterior plat boundary lines. The only exception is a
16-foot long strip at the southeast corner of the plat,
which will be required to be 5 feet wide.

- The Plat and Street Committee has reviewed the public
interest requirements set forth in Section 54.5-16 of
j the City Code for the street vacation and closure. It
is the recommendation of the Committee that the
application for the vacation and closure N.W. 4 Court
meets the spirit and intent of this Code Section and
should be approved. However, the Committee does not
recommend the vacation and closure of street right-of-
way at the southeast corner of N.W. 5 Avenue and N.W.
I 11 Street - the area between the existing 25 foot
s radius and the existing 2 foot high concrete block
wall.
- The question of ownership of the property must be
resolved before the 'time of final plat submittal.
Contact Linda Kearson, City of Miami Law Department, at
579-6700.
- Indicate the current zoning designations of the
adjacent subdivisions on both the location and main
sketches.
The preservation of large oak trees on site is
encouraged. Contact Sarah Eaton, Planning, Building
and Zoning Department, at 579-6086.
- Remove any reference to temporary D.O.T. easement from
plat.
Show proposed name of plat in the location sketch.
- Remove retaining wall encroachment into N.W. 5 Avenue.
- Show plat book and page numbers for all underlying
legal descriptions in the location sketch.
In addition to the above requirements, you should be aware of
the following:
1. State and local laws require the installation of various
physical improvements in the public rights -of -way when
property is platted. These subdivision improvements
include paving, drainage, landscaping, sidewalks, etc. In
some cases this could represent a substantial investment
on your part.

2. The alteration, relocation or installation of utilities
such as storm and sanitary sewers, electric, telephone,
water, etc. , caused by this plat will be at the property
owner's expense. Also, utility easements may be required
on the property being platted.
3. A building permit will not be issued on the property being
platted until the final plat is record--d. Also, the
Certificate of Occupancy for any building construction will
be issued only after all the required subdivision
improvements have been completed.
4. Approval for fire flow requirements must be obtained from
the Fire, Rescue and Inspection Services Department prior
to the issuance of a building permit.
5. Additional items must be provided to the City of Miami
Department of Public Works before the final plat is
submitted to the City Commission for approval. You will be
notified in writing as to what these items are after the
amount of the bond has been determined for the necessary
subdivision improvements.
6. Tentative plat approval is only valid for one (1) year From
the date of the Plat and Street Committee Meeting at which
time it was approved.
If you have any questions concerning these requirements, please
refer to the attached sheet for the appropriate person to contact.

The City Zoning Board of Miami, Florida, after Public
Hearing, has recommended the closing of Northwest 4 Court

between Northwest 10 Street and Northwest 11 Street. This
action was confirmed by City Commission Resolution No. 94-
53.

The Certificate of Title Examination, dated March 10, 1995,

signed by Thomas Goldstein, Attorney, indicates that the fee
simple title to the property platted is correctly vested in
the City of Miami, Florida, and the Plat has been correctly
executed.

As a result of a field investigation by the Department of
Public Works it has been determined that no subdivision
improvements shall be required.

The attached Resolution has been prepared for the acceptance

examined the title searches for Overtown Health Center by Dade
County dated February 4, 1993, November 18, 1993, April 11, 1994
and March 10, 1995, covering the period from the beginning to

RECOMMENDATION:
It is respectfully recommended that the City Commission adopt a

BACKGROUND:
! The Department of Public Works has reviewed this plat, and has
determine that it is now in order for acceptance by the City
Commission.
The proposed record plat entitled Jefferson Reeves Health Center
Subdivision is a resubdivision of Lot 16 and a portion of each of
Lots 4, 5, 17 and 18, Block 13 N, A.L. Knowlton's Map of Miami,
Plat Book B at Page 41, and a portion of Lot 4, Parry's Division
of Lots 1-2-19-20 in Block 13 N, Plat Book 1 at Page 75, and a
