HomeMy WebLinkAboutR-95-0352J-95-393 05/04/95 RESOLUTION NO. 9 5— 352 A RESOLUTION, WITH ATTACHMENT(S) AUTHORIZING THE CITY MANAGER TO EXECUTE A MEMORANDUM OF AGREEMENT, IN SUBSTANTIALLY THE ATTACHED FORM, BETWEEN THE CITY OF MIAMI, ON BEHALF OF THE SOUTH FLORIDA URBAN SEARCH AND RESCUE (US&R) TASK FORCE AND CEDARS MEDICAL CENTER, FOR THE PURPOSE OF CEDARS MEDICAL CENTER MAINTAINING AND PROVIDING AN EXPENDABLE MEDICAL CACHE OF SPECIFIC DRUGS AND SUPPLIES FOR DISASTER RESPONSE, CONTINGENT UPON FUNDING FOR SAID SUPPLIES USED DURING AN ACTIVATION BEING 100% REIMBURSABLE FROM STATE OF FLORIDA OR FEDERAL SOURCES. WHEREAS, Resolution No. 93-224, adopted April 15, 1993, authorized the City Manager to execute a Memorandum of Agreement between the City of Miami and the State of Florida and the Federal Emergency Management. Agency ("FEMA"), for the purpose of setting forth the parties responsibilities in connection with the ;i development and support of a Local Urban Search and Rescue Task Force sponsored by the City of Miami Fire -Rescue Department; and WHEREAS, the Task Force is required to maintain a significant amount of expendable, sensitive and time dated pharmaceutical supplies in inventory for emergency use; and WHEREAS, Dr. Shatz, JMH Trauma Surgeon/USAR Medical Team Manager, has approached the Cedars Medical Center and worked out an agreement where the Cedars Pharmacy will provide the pharmaceutical supplies for the Task Force's use as a community iATTA C�-������R� C D CITY COI1'i OSSION NiEETUgC OF MAY 1 1 1995 Resolution No. 95- 352 service with costs associated with the maintenance of the cache to be absorbed by Cedar; and WHEREAS, the City's responsibility after a mobilization and demobilization will be to return the unused portion of the cache to Cedars Pharmacy and reimburse it for the replenishment of any used supplies; and WHEREAS, said supplies will only be requisitioned during a State or Federal activation where the expense is 100% reimbursable by FEMA; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are hereby adopted by reference thereto and incorporated herein as if fully set forth in this Section. Section 2. The City Manager is hereby authorized! to execute a Memorandum of Agreement, in substantially the attached form, between the City of Miami and Cedars of Medical Center for the purpose of setting forth the parties responsibilities in connection with the provision of the Task Force's pharmaceutical cache within the Pharmacy Department of Cedars Medical Center, ! The herein authorization is further subject to compliance with all requirements that may be imposed by the City Attorney, including but not limited to those prescribed by applicable City Charter and Code Provisions. 2- 95- 352 G- contingent upon funding being secured in the form of reimbursements from State of Florida and Federal sources. DOC 3- 95- 352 MEMORANDUM OF AGREEMENT This Agreement entered into this .2 7ZZ day of 19 9S, by and between the City of Miami, on behalf of die South Florida Urban Search and Rescue (US&R) Task Force, Florida Task Force Two (FL TF 2), hereafter referred to as the City and Cedars Medical Center hereafter referred to as CMC. I. PURPOSE This Memorandum of Agreement (MOA) between the City and CMC sets forth the voluntary positioning of the task force's pharmaceutical cache within the Pharmacy Department of Cedars Medical Center and the reimbursement of any supplies used during a deployment. H. RESPONSIBILITIEES A. ne City agrees to: 1. Provide an administrating agency, which will be the City of Miami Department of Fire -Rescue, which will carry out the herein agreed upon responsibilities of FL TF 2. 2. Maintain 24 hour Task Force Alert capabilities, including a 24 hour phone number and point of contact or duty officer available at all times through the Fire -Rescue Department's Fire Central Information Office (FCIO): (305)-579-5245. 3. Provide CMC with proper shipping containers to be placed in storage at the CMC Pharmacy Department for packaging of the pharmaceutical cache. 4. Return all unused pharmaceutical supplies and shipping containers to CMC Pharmacy and reimburse CMC for the replenishment of supplies. B. CM[C agrees to: 1. Provide the pharmaceutical supplies as specified in the US&R Response System Medical Cache inventory. 95- 352 2. , Stock and store, in a rapidly accessible manner, types and amounts of specified pharmaceutical supplies. 3. Periodically check pharmaceutical supplies on a rotation deemed appropriate by Gary L. Hershey, Pharm. D. , Director of Pharmacy Services, or his designee, and replace as necessary, to ensure that supplies are maintained within appropriate expiration periods. 4. Secure controlled substances in a narcotic cabinet under the standard protocol of CMC until such time that FL TF 2 is mobilized. ffi PROCEDURES A. Alert & Activation 1. Upon request of State or Federal disaster assistance, or the pre -positioning of the US&.R Task Force when deemed prudent, the State of Florida or the Federal Emergency Management Agency (FEMA) shall request the activation of forces necessary to respond to an emergency or disaster situation. 2. Upon activation of the US&R Task Force, the medical component will become operational under the US Public Health Service. The City of Miami Department of Fire - Rescue will notify the Pharmacy Services Department at CMC, via Gary Hershey, Pharm.D., or his designee: (305) 325-5722. Additionally, the CMC Administration Department will be notified by the Task Force Medical 'Team Manager via the Vice President/Operations, or appointee: (305) 325-5683. The pharmaceutical supplies and storage containers will be retrieved from the CMC pharmacy by David Shatz, M.D., Medical Director FL TF-2, or his appointee. At that time, controlled substances will be accounted for and properly released. Once released, accountability of these substances becomes the responsibility of the Task Force. 95- 352 B. Mobilization and Demobilization 1. FL TF-2 shall mobilize and move their task force personnel and equipment cache to the pre -designated point -of - departure for pickup by Department of Defense aircraft or other specified aircraft, or be enroute via ground transportation, within six hours of official activation. 2. Upon return and demobilization of the 'Task Force, unused pharmaceuticals and shipping containers will be returned to CMC Pharmacy Services for restocking in preparation for future deployments. CMC will prepare and submit an invoice to the City within fifteen days, or as otherwise agreed upon, for reimbursement. The invoice will include i all expended supplies listed by name, amount and price. Reimbursement will be based on current acquisition costs. IV. CONDITIONS, AMENDMENTS AND TEIt1YENATION A. This agreement will end on December 31, 1996, at which time both parties may agree to renew the association. Renewal will be based on evaluation of the Task Force and CMC Pharmacy Services' ability to conform with procedures and equipment standards as described in this MOA. B. This agreement may be modified or amended only on written agreement of all parties, and all amendments shall be attached hereto. QUINN JONES, III CITY ATTORNEY 95- 352 TO FROM : CITY OF MIAMI, FLORIDA CA=9 INTER -OFFICE MEMORANDUM The Honorable Mayor and Members DATE ; MAY - 3 1995 FILE of the City Commission City RECOMMENDATION SU6JECT : Resolution Authorizing USAR Memorandum with Cedars Medial Center REFERENCES: ENCLOSURES: It is respectfully recommended that the City Commission adopt the attached Resolution authorizing the City Manager to execute an agreement with Cedars Medical Center (CIVIC), which has agreed to maintain and provide the expendable medical cache, on two hour notice, for the South Florida Urban Search and Rescue Task Force. BACKGROUND The South Florida Urban Search and Rescue Task Force is required to maintain an extensive expendable medical cache of specific drugs and supplies for disaster response. Through an agreement with the Federal Emergency Management Agency (FEMA), an alternative to purchasing these items is to enter into an agreement with a local hospital or pharmaceutical supplier that is capable of maintaining and supplying the required supplies on short notice. Cedars Medical Center has agreed to stock, rotate critical time items and provide the required supplies through their Pharmacy Department on short notice at no charge to the City. When activated for a State or Federal response to a disaster, the items used during the activation would be replenished by their organization and a bill will be presented to the Task Force for reimbursement. The supplies used during an activation are 100% reimbursable by FEMA. _ The South Florida Urban Search and Rescue Task Force will benefit from this agreement by not having to purchase expensive and time critical supplies. The Agreement will provide the necessary guidelines to coordinate the Task Force's acquisition of the specific expendable drugs and supplies required by FEMA. There are no costs associated with the execution of this document. 95- 352
