BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The emergency repair of the roof at the Allapattah Child Daycare Center is hereby authorized, with fund therefor hereby allocated in the amount of $3,900 from Index Code No. 450322, Minor Object 540, Project No. 450001. Attn: Mr. Orlando Urra 1836 Northwest 22nd Place Miami, Florida 33125 Re: Roofing Failures at 1836 Northwest 22nd Place A roofing field inspection was performed at the above referenced facilit.v on February 19, 1995 to investigate various areas of water intrusion into the facility and provide a general roof assessment of the center and In- dicate area's that might prove a potential problem of failure at a later date. Existing Substrate: Multi -Ply Asphalt BUR Smooth Surface Roofing System with an Aluminized Fibrated Coating over an unknown thickness of what appears to be Cellular Insulating Concrete. A visual examination of Deck A revealed various anomalies which could be attributable to a downgraded service life of the roofing system. The following anomalies were noted: A) The existing multi -ply asphaly BUR system exhibits extreme UV degradation, weathering, ponding water and organic growth or contamination to the point that felts are unadhered, exposed, brittle and fishmouthed. Aluminum coating has worn out. Existing asphalt is exhibiting alligatoring and crazing. B) Existing Stucco Parapet Wall(3'-0") exhibits many areas of cracks allow- ing the presence of water intrusion into the roofing system and CMU wall. C) All single GFI outlet which services the existing roof fans do not have pitch pans installed or any acceptable industry standard of flashing/ waterproofing pipe penetrations. D) Existing A/C condensate line is not properly supported. The existing support is a 8"x8"x16" CMU Block. This is not an industry approved - method. E) Existing A/C duct supports are steel angles and not the recommended round pipe supports as recommended by the NRCA (National Roofing Contractor's Association Manual of Roofing and Waterproofing),In addition, these angles do not have pitch pans or acceptable waterproofing. F) Multi -Ply membrane at northeast corner of deck contains one(1) blister . adjacent to existing parapet wall. G) Existing pitch pans at A/C equipment needs to be filled. Pitch pans are empty and are ponding water. A) Existing aluminized coating has been weathered off. Coating or surfacing must be reapplied in order to maintain the integrity of the membrane. 1/ 95-- 214(cont.) Allapattah-Child re Center February 27, 199_ Page 2 I) Existing parapet wall perimeter along the metal counterflashing is mis- sing portions of the stucco, exposing felts and membrane. J) Existing windows along cupula tile roof should be caulked and sealed to provide a watertight seal. K) Existing collectorheads and downspouts should be cleaned out. One(1) collectorhead has a boomerang inside. L) Existing vent through the roof are open with no vandal proof caps to prevent vandalism(optional). In addition, Vent through roof do not have lead flashing. M) Existing A/C unit is mounted directly on the roofing system deck, on a factory curb with no flashing at curb. N) Clay tile roofing system was found to be in good condition with no miss- ing or broken tile. All tiles are strongly adhered to the existing sub- strate and metal drip edge is secured. The above referenced items represent potential problems or areas of roof failures at a later date. The areas of water intrusions on the deck are a result of faulty of defective flashing, poor workmanship and installation procedures. The solution would be to correct the defective areas using acceptable industry standards and manufacturer's recommendations.
