RESOLUTION NO.
A RESOLUTION ACCEPTING THE PROPOSAL OF
PENINSULA STATE LEGAL SERVICES PLAN, INC.
("PENINSULA"), FOR THE PURPOSE OF PROVIDING A
GROUP PREPAID LEGAL SERVICES PLAN, AT NO COST
TO THE CITY, TO APPROXIMATELY 260 FULL-TIME,
NON -UNION CITY OF MIAMI EMPLOYEES AS WELL AS
ELIGIBLE UNION EMPLOYEES CONTINGENT UPON
UNION AGREEMENT FOR PARTICIPATION; FURTHER
AUTHORIZING THE CITY MANAGER TO ENTER INTO AN
AGREEMENT, IN A FORM ACCEPTABLE TO THE CITY
ATTORNEY, WITH PENINSULA FOR AN INITIAL TERM
OF TWO YEARS, RENEWABLE AT THE CITY OF
MIAMI'S OPTION FOR TWO ADDITIONAL ONE-YEAR
PERIODS, AT THE SAME TERMS AND CONDITION.
WHEREAS, the City wishes to offer a group prepaid legal
services plan, at no cost to the City, to approximately 260 full-
time, non -union City of Miami employees, as well as certain
eligible union employees; and
WHEREAS, the City mailed solicitations to various firms for
the furnishing of the prepaid legal services and Peninsula State
Legal Services Plan, Inc. was selected as the most qualified firm
to provide the required services;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:

Section 1. The recitals and findings contained in the
Preamble to this Resolution are hereby adopted by reference
thereto and incorporated herein as is fully set forth in this
Section.
Section 2. The proposal of Peninsula State Legal
Services Plan, Inc. to provide a group prepaid legal services
plan, at no cost to the City, to approximately 260 full-time,
non -union City of Miami employees as well as eligible union
employees contingent upon union agreement for participation is
hereby accepted.
Section 3. The City Manager is hereby authorized-!/ to
enter into an agreement, in a form, acceptable to the City
Attorney, with Peninsula State Legal Services Plan, Inc., for an
initial term of two (2) years, renewable at the City of Miami's
option, for two additional one-year periods, at the same terms
and conditions.
Section 4. This Resolution shall become effective
immediately upon its adoption.
PASSED AND ADOPTED this 25th day of July 1996.

Recommendation for Resolution
and Award of Professional
Services Agreement for Group
Prepaid Legal Services Plan
It is respectfully requested that the City Commission adopt a resolution to award a contract to
Peninsula State Legal Services Plan, Inc. to provide a group prepaid legal services plan to
approximately 260 full-time, non -union employees as well as eligible union employees
contingent upon union agreement for participation, at no cost to the City of Miami.
BACKGROUND
In a effort to continue to provide City employees, at their expense, the option to acquire a quality
group prepaid legal services plan, solicitations were sent to fourteen (14) firms. Sealed proposals
were received from five (5) firms by the deadline of July 10, 1996. Evaluation of the proposals
were performed by staff from the Department of Human Resources, a representative from IAFF,
Local 587, and a recent retiree from the City of Miami. The top three (3) rankings of the
proposals were as follows: 1.) Peninsula State Legal Services PlanInc 2.) Legal Line Prepaid
Legal Services, Inc. and 3.) Midwest Legal Services, Inc. It is recommended by the evaluation
committee that the professional services agreement be awarded to Peninsula State Legal Services
Plan, Inc.
