A RESOLUTION, WITH ATTACHMENT(S), APPROVING
FUNDING FOR THE HIV/AIDS PLANNING & MANAGEMENT
ORGANIZATION (HAPMO), IN THE AMOUNT OF $180,000,
FOR THE PROVISION OF HOUSING RELATED SERVICES TO
PERSONS WITH HIV/AIDS OR RELATED DISEASES;
ALLOCATING FUNDS THEREFOR FROM THE 1995 HOUSING
OPPORTUNITIES FOR PERSONS WITH AIDS (HOPWA) GRANT
PROGRAM; FURTHER AUTHORIZING THE CITY MANAGER TO
EXECUTE THE NECESSARY AGREEMENT, RETROACTIVE TO
JULY 1, 1996, AND IN A FORM ACCEPTABLE TO THE CITY
ATTORNEY, TO IMPLEMENT THE PROGRAM.
WHEREAS, the HOPWA Program was created under the AIDS Housing Opportunity
Act, and revised under the Housing and Community Development Act of 1992; and
WHEREAS, Section 606(d)(2) of the Housing and Community Development Act of
1992 stipulates that the City, being the most populous unit of general local government in
an eligible metropolitan statistical area (EMSA), not including county government, must be
the applicant for the Grant and be allocated the related funding; and
WHEREAS, in March, 1995, the City of Miami applied for $7,906,000 in 1995
HOPWA Grant funding on behalf of the EMSA, which includes Dade County and the cities
of Hialeah and Miami Beach; and
WHEREAS, in October, 1995, the City was notified by U.S. HUD that due to the
rescission of $15,000,000 in Program appropriations, our EMSA's 1995 HOPWA
allocation was reduced to $7,268,000; and

WHEREAS, in June, 1996, the 1995 HOPWA Advisory Board recommended to the
City Administration, after a duly noticed public hearing, that the HIV/AIDS Planning &
Management Organization be funded to provide housing related services to persons with
HIV/AIDS, and support services to the City Administration; and
WHEREAS, the City Administration recommends funding for this agency for the
provision of housing and related services to very low income persons living with AIDS
throughout the Dade County geographic area, and support services to the City
Administration;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the Preamble to this Resolution
j are hereby adopted by reference thereto and incorporated herein as if fully set forth in this
Section.
Section 2. Funding in the amount of $180,000 for the HIV/AIDS Planning &
Management Organization (HAPMO) for the provision of housing related services to
persons with HIV/AIDS or related diseases, and support services to the City
Administration, is hereby approved, with funds therefor hereby allocated from the 1995
HOPWA Grant Program.
Section 3. The City Manager is hereby authorized' to execute the necessary
r agreement, retroactive to July 1, 1996 and in a form acceptable to the City Attorney, with
HAPMO to implement said Program.
Section 4. This Resolution shall become effective immediately upon its adoption.
' The herein authorization is further subject to compliance with all requirements that may be imposed by the City
Attorney, including but not limited to those prescribed by applicable City Charter and Code provisions.

HOUSING OPPORTUNITIES FOR PERSONS WITH AIDS (HOPWA) GRANT
RESOURCE IDENTIFICATION, PLANNING, &
INFORMATION SYSTEM INTEGRATION
HIV/AIDS Planning & Management Organization, Inc.
Goal I :To identify resources for housing and related services targeted for persons with
HIV/AIDS, and their families, throughout all of Dade County; and to facilitate
communication thereof.
Objective 1. Bi-monthly updating and maintenance the apartment listing.
Task l . Continuous gathering of information from newspapers, realtors,
and affordable housing owners, developers, and management
companies.
Task 2. Verify participation of current landlords.
Objective 2. Maintain and update, at least monthly, the Service Delivery
Information System (SDIS) resource inventory as information
becomes available.
Objective 3. Continue the availability of the "Housing Hotline."
Task 1.Continuous updating and maintenance of the "Housing Hotline"
with the most current information.
Task 2. Continue to gather information from HOPWA and non-HOPWA
providers for the "Housing Hotline."
Task 3. Advertise the "1-800 Housing Hotline."
Objective 4. Produce client / consumer information bulletins / brochures for
each HOPWA funded service, including client responsibilities.
Task LProduce the bulletins in English.
Task 2.Produce the bulletins in Spanish.
Task 3. Produce the bulletins in Creole.
Objective 5. Outreach.
Task 1. Conduct six educational meetings on available housing services,
holding one every two months in the following neighborhoods:
South Beach, Little Haiti / West Little River, Liberty City /
Brownsville, Opa Locka / Carol City, Homestead / Florida City,
and one to be determined by the City Administration during the
contract period.
Task 2. Conduct a training session on housing issues for case managers,
social workers, and service providers who deal with HIV/AIDS
population. This will be done twice, with six months in between
sessions.

Task 3.Conduct a monthly training session for new case managers, social
workers, HIV/AIDS service providers.
Task 4.Conduct on -site training for HIV/AIDS service provider
agencies' employees. This will be done based on client referrals
and in agreement with, and with prior written approval of, the City
Administration.
Task 5. Contact landlords to explain the HOPWA housing assistance
program and the necessary client confidentiality requirements.
Objective 6. Information sharing.
Task 1. Send out, via fax, notices with information on housing and related
services, at least bimonthly.
Task 2. Assist clients over the telephone by making referrals to various
AIDS service providers and/or legal help agencies, working with
HOPWA funded service providers or landlords, working with
client's case manager, or other assistance needed by client within
HOPWA's scope of activities.
Task 3. Maintain a log of client calls from Task 2 (above).
Task 4.Deliver a monthly written report to the HOPWA Advisory Board
on systemic HOPWA problems and recommendations on
solutions.
Goal2:Assist the HOPWA Advisory Board and the City Administration with their
recommendations for the use of future resources for housing and related services
for persons with HIV/AIDS.
Objective 1. Ensure that housing issues are included in the 1996 HIV/AIDS
Community Needs Assessment.
Task l . Housing issues will be discussed in the Needs Assessment's focus
groups.
Task 2.Housing issues will be discussed in the Needs Assessment's public
meetings.
Task 3. Housing issues will be discussed in the Needs Assessment's client
satisfaction surveys.
Objective 2. Update the 1995-1996 Housing Needs Assessment.
Goal 3.Implement the Service Delivery Information System (SDIS) for HOPWA.
Objective 1. Have all fiscal year 1995 HOPWA funded agencies, and the City,
up and linked to the SDIS by January 1, 1997.
Objective 2. Program SDIS to accumulate data on HOPWA and non-HOPWA
clients, and other information, as required to be reported in the

HOPWA Annual Progress Report (APR), for time specific periods,
by March 1997.
Objective 3. Conduct HOPWA SDIS Users Group monthly meetings, at least
six, beginning January 1997.
Goal 4.Provide support to City Administration.
Objective 1. HOPWA Advisory Board (HAB)support.
Task I.Take notes and produce written minutes of the HAB meetings.
Task 2.Transmit meeting notices and agenda packages to HAB members.
Task 3."Mega-Pax" HAB meeting notices.
Objective 2. Assist City Administration in the implementation of the HOPWA
Grievance Procedure.
Objective 3. Assist HOPWA funded agencies in the implementation of the
HOPWA Grievance Procedure.
Objective 4. Assist HOPWA assisted clients with accessing and using the
HOPWA Grievance Procedure.

The attached proposed Resolution is being submitted for your
consideration to authorize a settlement, without any admission of
liability, in full and complete settlement of any all claims and
demands against the City of Miami, in Dade County Circuit Court
Case No. 95-09412 CA (30) Fermin Domenack vs. City of Miami, in
the amount of $40,000.00. Funds are available from the City of
Miami Self -Insurance and Insurance Trust Fund, Index Code No.
620101-653.
A complete analysis and description of this case is fully set
forth in the memorandum to the Members of the Tort Committee
previously transmitted to you.
This claim has been investigated by the Division of Risk
Management and evaluated by the Tort Committee and it is agreed
that the settlement is in the best interest of the City.

RECOMMENDATION
It is respectfully recommended that the City Commission adopt the attached Resolution
authorizing the City Manager to execute a memorandum of agreement with the HIV/AIDS
Planning & Management Organization (HAPMO) for the provision of housing related services to
low income persons with HIV/AIDS, and for the provision of support services to the City
Administration. Funding for these services is being provided by the U.S. Department of Housing
and Urban Development from the fiscal year 1995 Housing Opportunities for Persons With
AIDS (HOPWA) Grant Program.

NET recommends that the City Commission adopt the attached resolution, approving the funding
of the HIV/AIDS Planning Organization (HAPMO) in the amount of $180,000 and authorizing
the City Manager to execute a memorandum of agreement with HAPMO, in connection with the
provision of housing related services to low income persons with HIV/AIDS or related diseases,
and for the provision of support services to the City Administration.
In March of 1995, the City of Miami, on behalf of Metropolitan Dade County and the cities of
Hialeah and Miami Beach, applied for $7,906,000 in Housing Opportunities for Persons With
AIDS (HOPWA) Grant Program funding from U.S. HUD. In October of 1995, the City was
notified by U.S. HUD that its application for 1995 HOPWA Grant Program funding had been
approved; however, because of a federal budget amendment/rescission to the fiscal year budget,
the City's award was decreased from $7,906,000 to $7,268,000.
At the March 1995 City Commission meeting, the City Commission, through Resolution No. 95-
179, authorized the City Manager to accept the 1995 HOPWA Grant Program award from U.S.
HUD and to execute the necessary implementing agreements, in a form acceptable to the City
Attorney, and applicable City Code provisions.

In June of 1996, the City's HOPWA Advisory Board recommended to the City Administration,
after a duly noticed public hearing, that the HIV/AIDS Planning & Management Organization
(HAPMO) be funded in the amount of $180,000, from the 1995 HOPWA Grant Program, to
provide housing related services to persons with HIV/AIDS, and support services to the City
Administration.
Based on the above recommendation of the City's HOPWA Advisory Board, NET recommends
the HIV/AIDS Planning & Management Organization (HAPMO) for funding in the amount of
$180,000, from the 1995 HOPWA Grant Program, to provide housing related services to persons
with HIV/AIDS, and support services to the City Administration. Funding for these services is
available from the 1995 Housing Opportunities for Persons With AIDS (HOPWA) Grant
Program.
City Commission ratification of the attached resolution is recommended.
