A RESOLUTION ACCEPTING THE PROPOSAL OF
COLUMBIA/CEDARS MEDICAL CENTER FOR THE
PROVISION OF DRUG SCREENING SERVICES FOR THE
DEPARTMENT OF HUMAN RESOURCES, ON A CONTRACT
BASIS FOR AN INITIAL TERM. OF TWO (2) YEARS,
WITH TWO (2) RENEWAL OPTIONS OF ONE (1) YEAR
EACH, AT AN ESTIMATED COST OF $14,384.00
DURING THE FIRST YEAR OF THE INITIAL PERIOD;
FURTHER, PROVIDING FOR A THREE PERCENT (30)
ANNUAL INCREASE, PER UNIT/TEST PERFORMED,
EFFECTIVE THE FIRST DAY OF THE FIRST OPTION
PERIOD UNTIL THE END OF THE CONTRACT PERIOD;
ALLOCATING FUNDS THEREFOR FROM THE DEPARTMENT
OF HUMAN RESOURCES GENERAL OPERATING BUDGET,
ACCOUNT CODE NO. 270101-260; AUTHORIZING THE
CITY MANAGER TO EXECUTE A CONTRACT, IN A FORM
ACCEPTABLE TO THE CITY ATTORNEY, OR TO
INSTRUCT THE CHIEF PROCUREMENT OFFICER TO
ISSUE A PURCHASE ORDER FOR SAID SERVICES.
WHEREAS, Requests for Proposals were mailed to 19 firms
seeking a broad range of responses from drug screening providers
and to afford a competitive process for the provision of drug
screening services; and
WHEREAS, said services will be used by the Department of
Human Resources for the purpose of performing drug screening
profiles on applicants for employment and designated current
employees; and
WHEREAS, one (1) response was received on July 7, 1997 to
provide said services; and

WHEREAS, the proposal received from Columbia/Cedars Medical
Center was the most advantageous to the City; and
WHEREAS, funding is available for payment of said services
during the first year of the initial period in the amount of
$14,384.00 from the Department of Human Resources General
Operating Budget, Account Code No. 270101-260;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the
Preamble to this Resolution are hereby adopted by reference
thereto and incorporated herein as if fully set forth in this
Section.
Section 2. The proposal of Columbia/Cedars Medical
Center for the provision of drug screening services for the
Department of Human Resources, on a contract basis for an initial
term of two (2) years, with two (2) renewal options of one (1)
year each, at an estimated cost of $14,384.00 during the first
year of the initial period, and further providing for a three
percent (3%) annual increase, per unit/test performed, effective
the first day of the first option period until the end of the
contract period, is hereby accepted, with funds therefor hereby
allocated from the Department of Human Resources General
Operating Budget, Account Code No. 270101-260.

Section 3. The City Manager 'is hereby authorized to
execute a contract,11 in a form acceptable to the City Attorney,
or to instruct the Chief Procurement Officer to issue a purchase
order for said services.
Section 4. This Resolution shall become effective
immediately upon its adoption.

It is respectfully recommended that the City Commission adopt a resolution awarding
Request for Proposals 96-97-102 - Part I to Columbia/Cedars Medical Center, a non -
minority proposer, to provide drug screening services to the Department of Human
Resources. The proposed contract will be effective for an initial two year period with
the option to extend for two additional one-year periods. Compensation during the first
year of the initial contract period is in the amount of $14,384.
BACKGROUND
In an effort to secure a broad range of providers, invitations to perform drug screening
services for the City of Miami were sent to 19 firms. This effort resulted in responses
from 3 firms, two of which were no -bids.
The need for laboratory services is outlined in Section 3(f) of Ordinance No. 8526
adopted by the City Commission on January 20, 1976. The Department of Human
Resources is required to establish and maintain medical services which shall include a
medical examination of employees and prospective employees. Additionally, on
January 14, 1988, the City Commission adopted Motion 88-34 which required all new
applicants to undergo a drug test.
Drug screening services will include performing drug profiles on applicants for
employment and designated current employees including but not limited to AFSCME
and LIUNA bargaining unit employees, unclassified employees, managerial employees,
etc. The need for such services is mandated by City Commission policy and the various
applicable labor agreements.
Funding is available within the fiscal year 1996-97 operating budget of the Department
of Human Resources to cover the cost of these services (Account Code: 270101-260).

Your approval is hereby requested to approve the Proposal received pursuant to the RFP issued for Drug
Screening Services, Part I on behalf of the Department of Human Resources.
Please be advised that an RFP for Drug Screening Services, RFP No. 96-97-060, included three parts:
Part I (Employment), Part II (Police and Fire), and Part III (D.O.T.). Parts II and III were approved by the
City. Commission. However, the two (2) Responses initially received for Part I were rejected by the City
Manager on or about May 19, 1997 in order to clarify and revise specifications which would result in a
more cost effective and efficient delivery of service to the City of Miami. As a result, RFP No. 96-97-102
was issued for Part I services only.
A total of nineteen (19) solicitations were mailed on June 16, 1997. One (1) responsive and responsible
Proposal was received on July 7, 1997 from Columbia/Cedars Medical Center (local firm). Two (2) "No
Bids" were received for reasons as stated: (1) Mount Sinai Medical Center (local) reported it was unable
to meet specifications; and (2) Mercy Hospital (local) reported that the turnaround time was too tight. In
addition, I had received a telephone call from OCC Health Services of America (local), after the time the
Proposals were due inquiring about the due date and time; they were advised the Proposals had already
been received by the City Clerk's Office and no others could be accepted.
As this is the second solicitation for Part I of Drug Screening Services, and as this -field is a highly
specialized area which must adhere to many State and Federal guidelines, along with meet all
specifications of the City, I recommend that the sole Proposal received from Columbia/Cedars Medical
Center, a firm located within the City of Miami, be accepted and recommended to the City Commission
for award.
APPROVED / DISAPPROVED:
NOTE: The only Proposer who would have met the NIDA certification requirements
is Cokumbia/Cedars Medical Center.
