Dear Mr. Foeman:
It has been a pleasure working with you in the development of the Shake -A -Leg Miami
program. Shake -A -Leg Miami has grown through a grass routs effort involving people from all
sectors of the community. You have always been there to help and we are turning to you now
for your continued support. We would like to ask you for two things.
1 As you know we have been working with the City of Miami to expand our current programs
and develop a world class water sports center. We have made significant progress and will be
presenting our plan to the City of Miami Commission on July 1 Oth. It would be a big help to us
if you would send a letter to any of the city commissioners you know or would like to petition on
our behalf. Attached is a brief explanation of the proposed water sports center and a sample
letter to the city commissioners requesting their approval of our plans. Your support on this will
give us that little extra that might mean the difference between success and delayl
2. To assist us in demonstrating our broad based community support and to insure that we
continue to have the necessary guidance and support to achieve our ambitious plans, we are
formerly creating a board of advisors. We would like you to become a member of this board.
Board members will be called on from time to time for advice and support. We will list you as a
member of the board of advisors which will be included in our proposal before the City
Commission.
We appreciate your support and I will be calling you to discuss Shake -A -Leg Miami and our

On behalf of the Board of Directors of the Dr. John T. Macdonald Foundation, Inc., I am writing to request your
affirmative vote for the Shake -A -Leg Miami plan developed for the former Coast Guard Seaplane Base in Coconut
Grove. As an organization that has provided significant programmatic support for Shake -A -Leg Miami, we have
been pleased with the return on our investment. .
Currently operating out of a temporary facility on the property, Shake -A -Leg Miami continues to do an incredible
job of serving the needs of all segments of Miami's population. While Shake -A -Leg Miami's primary mission is to
serve and improve the quality of life and capabilities of the physically and developmentally disabled in a unique
sailing program, the resulting outreach it has made into all areas of the community - at -risk children, college
students, professionals in all walks of life, community organizations, national and international sailing
professionals, and so many others - is a tribute to the organization's vision and capability.
The opportunity for the City of Miami to take advantage of Shake -A -Leg Miami's plan to develop an International
Water Sports Center for the recreational use of the residents of Miami will be a tremendous step forward for this
entire community.
Throughout its operation, Shake -A -Leg Miami has received broad -based community support. That support has
grown substantially in the past several years, not only in human resources, but through significant funding support.
Of particular note is the funding which will be available through the Trust for Safe Neighborhood Parks Act which
will help finance the construction of this Center, *and was approved by Dade County voters in November of 1996.
This proposed facility will provide quality, affordable programs to all members of the community and from around
the world and will be a showcase for the City of Miami. Shake -A -Leg Miami is uniquely suited and qualified to
take a leadership role in the development of this Center and is poised to take that step.
Your support of this project is greatly appreciated. You will be proud of the results of your vision in supporting
such a public -private partnership for the good of all our community's residents and visitors.

Please be advised this is the second time this service has been bid. The first, Bid No. 96-97-026, was rejected as only
one (1) bid was received. As a result, some specifications were slightly changed, and the bid was re -issued and a total
of 64 bid packages were mailed. The bid opened April 23, 1997.
Two (2) bids were received. One included the same firm who responded pursuant to Bid No. 96-97-026 (Cliffhanger),
and one additional firm (Sky Hanger). A bid tabulation was prepared comparing the prices submitted by the two firms,
and a close evaluation revealed a disparity in pricing. Item #1 was the item to determine award to the lowest
responvise vendor. This item included as a minimum, on a semiannual basis, the cleaning of all MRC windows and the
pressure cleaning of all exterior paver areas surrounding the facility; and on a quarterly basis the cleaning of mdin
lobby windows, cross -members and ledges in the lobby, and the pressure cleaning of the north and south
entranceways. The lowest price quoted for this service was $1,500 per year by Cliffhanger.
I contacted Steve Shaw, Facility Manager for the MRC and the individual who prepared the specifications. Upon his
review of the pricing, he advised that the low firm (Cliffhanger) could not possibly perform the required work at the
prices quoted. As a result, a conference telephone call was placed with Mr. Roberto Calderin, President of Cliffhanger.
After our discussion, he advised his firm had indeed made a mistake in the prices quoted and could not perform the
work according to specifications at that price. Subsequently, he FAX'd another price sheet to me and quoted a new
and higher price of $12,280.00 for Item #1 ($4,120.00 higher than the bid of Sky Hanger). As Cliffhanger was unable
to perform the work at the original prices quoted and prices cannot be changed after the bid opening, the bid response
from Cliffhanger, Inc. was deemed non -responsive.
Although only one firm was deemed responsive (Sky Hanger), I recommend this bid be accepted for the following
reasons: (1) This was the second time this service has been bid, and the service is required as soon as possible; (2)
There are no guarantees that any other firms will bid should a third bid be issued; and (3) Sky Hanger provided the
lowest responsive and responsible price.

DRUG -SCREENING SERVICES
Items 28 & 29 of the June% 1997 City Commission Agenda
The Department of Human Resources in conjunction with the Departments of
Fire -Rescue and Police developed a Request for Proposal (RFP) to secure drug
screening services, The RFP was divided into the following three parts;
1) Employment Drug Screening Services
2) Police and Fire Drug Screening Services
3) DOT Alcohol and Controlled Substances Testing,
Part I'for Employment Drug Screening Services has been rejected and will be
reissued. The RFP will be clarified and revised and should result in a more.cost
effective and efficient delivery of service.
The Evaluation Committee comprised of Ruben Camp, RN, Mercy Outpatient
Center, Mike Edwards, Records Administrator, Metro Dade County and Renee
S. Tones, City of Miami reviewed the responses to Parts li & Ill. Veldora Arthur,
Fire -Rescue, Sharon Simpkins and Sgt. Audrey Brown served as non -voting
members of the Committee. It was determined in both cases, that Toxicology
Testing Service was non -responsive due to the fact that the RFP required that
the provider's collection site be in the City of Miami. Toxicology's proposal
recommended utilizing Government Center has a collection site, however, after
hours Mt. Sinai on Miami Beach would be the collection site.

Part If is to provide drug screening services for sworn police and fire personnel
pursuant to the labor agreements.

Part Ill is to provide drug screening services pursuant to the federal
government's Omnibus Transportation Employee Testing Act of 1991 which
requires that certain employees with a commercial driver's license, who operate
a commercial vehicle weighing over 26,001 pounds, transports 16 or more
passengers or transports hazardous materials undergo random drug testing.
Approximately 260 employees throughout the City are subject to this testing.
Cedars/Columbia Medical Center was deemed most responsive to the RFP for
both Parts 11 & III. 42 RFP invitations were sent out and only 2 responses were
received.

RECOMMENDATION:
It is respectfully recommended that the City Commission approve the attached resolution
authorizing the lease/purchase of a Microfilm Reader Printer equipment for the Microfilm
Office of the Building and Zoning Department. This equipment will be leased/ purchased
from PMI Imaging Systems Inc., (Gary Schwartz and David J. Vollmers, owners), a non-
minority/non-local vendor, located at 2100 Park Central North, Suite 300, Pompano
Beach, Florida. The annual cost of the lease/purchase, including maintenance, will not
exceed $11,460.96. Funds are available in the Fiscal Year 1996-1997 budget of the
Building and Zoning Department.
BACKGROUND:
The Department of Building and Zoning has determined that this equipment is needed
since the present printer is over nineteen (19) years old and in constant need of repairs.
Eighteen (18) invitations for bids were mailed and two (2) bids were received. It was
determined that PMI Imaging, Inc. submitted the lowest, most responsible and responsive
bid for the leasing of this equipment. Due to the rapid change in technology, a
lease/purchase for four (4) years at a cost of $11,460.96 per year, which includes
maintenance, is being recommended rather than an outright purchase of $39,445. If the
system does not meet the department's expectations, PMI Imaging Systems, Inc. will take
the unit back at no cost to the City, within the first thirty (30) day period. The Microfilm
Office is a revenue producer. It charges the public for copies of plans, tax cards, and
blueprints. The new Microfilm Canon NP-700 Reader Printer has the capability of reading
and printing 16 mm / 35 mm rolls of films, in addition to a two (2) hours response time for
maintenance servicing.
